Duty to have fall protection Protection at or above 6' level. Walking/working surfaces inspected before work begins. Employees can only work on surfaces strong enough to support them. Page 1
Protection required for Unprotected sides and edges Leading edges Ramps, runways and other walkways Dangerous equipment Overhand bricklaying and related work Steep roofs Residential construction Page 2
Protection required for Walking/working surfaces not otherwise addressed Hoist areas Holes Formwork and reinforcing steel Excavations, wells, pits, shafts Roofing work on low-slope roofs Precast concrete erection Wall Openings. Page 3
Guardrails Top rail height - 42 +/- 3 Midrails 21" Intermediate members/ openings - no greater than 19" Withstand 200# downward/outward force Midrails 150# No steel, plastic banding Wire rope toprail flagged every 6' Manila, plastic or synthetic rope rails inspect often All wire rope guardrails 1/4" diameter or greater. Page 4
Guardrails Guardrail Around Floor Opening Page 5
Open Sided Floor Protection Fall protection systems and practices Covers Vehicular traffic - 2 times maximum axle load of the largest vehicle. Foot traffic - 2 times weight of expected load. Secured to prevent displacement. Marked "HOLE"/"COVER" or color coded. Page 6
Positioning device systems No more than 2' freefall. Inspected before each use. Safety belts. (Safety belts are not approved as a personal fall arrest system) Page 7
Personal fall arrest systems Body belts are no longer allowed for fall arrest (This was effective January 1, 1998) Page 8
Personal fall arrest systems Harness arrest force 1,800# maximum. Rigged to fall no more than 6' nor contact lower level. Provide for prompt rescue. Inspect prior to each use. Page 9
Personal fall arrest systems Dee-rings, snaphooks 5,000# tensile strength Proof-tested to 3,600# Only locking type. Page 10
Page 11
(Personal fall arrest systems) Lifelines Horizontal lifelines which may become vertical require locking capability in both directions. (Rope Grabs). Horizontal lifelines installed by qualified person. Safety factor of at least 2. Free fall no more than 6'. Each employee must have own lifeline. SELF RETRACTING LIFELINES: Limit Fall to 2 feet, then tensile load 3,000 lbs. Allow fall greater than 2 feet, then tensile load 5,000 lbs. Page 12
Controlled access zones Leading edge work. Overhand bricklaying. Precast concrete. Residential Construction Work. Accessible only to those qualified employees necessary to perform work. Page 13
Controlled access zones GUIDELINES: Erected not less than 6' nor more than 25' from the unprotected or leading edge. Must extend along the entire length of & be approximately parallel to the unprotected or leading edge. Control line must be connected on each side to a guardrail system or wall. Must consist of ropes, wires, tapes or equivalent materials, & supporting stanchions as follows: Each line fagged or otherwise clearly marked at not more than 6' Intervals with high visibility materials. In general, each line must be rigged & supported such that its lowest point is not less than 39" & its highest point is not greater than 45" from the working surface. Page 14
Protection from falling objects Toeboards 3 1/2" tall. Screens - install where needed. Only masonry equipment / materials within 4 of edge. Material stored within 6' of edge unless guardrails have been installed and material is stable and self-supporting. Canopies must be strong enough not to collapse and prevent penetration of objects. Barricade areas and prohibit entry by employees. Page 15
Safety net systems As close as practicable, no more than 30' below. Bridges - fall area to net unobstructed. Sufficient clearance to objects below. Drop tested or certified. Outward extension of safety nets Vertical Horizontal Distance up to 5 8 >5 to 10 10 >10 13 Page 16
Safety net systems No defective nets. Inspected weekly and after impact. Removal of objects daily or before next shift. No greater than 6" openings on any side. Border ropes 5,000# breaking strength. Connections not more than 6 apart. Page 17
Safety net systems Testing 400 Pound Bag of Sand. Dropped Highest Work Surface. After Installation. After Relocation. After Major Repairs. Every 6 Months. Inspected Weekly. After Any Occurrence which way effect integrity. 0 Page 18
Warning line systems Erected around all sides of roof work area. No mechanical equipment in use outside the warning line. Roof access points to be guarded. Flag with high visibility material. No lower than 34" nor higher than 39". Page 19
Safety monitoring systems Competent person to monitor. On same level, in visual sight of workers. Close enough to communicate, orally. No mechanical equipment to be used or stored in area. Only employees engaged in roofing work or covered by a fall protection plan in area. Employees must comply with warnings from monitors. Monitor shall have no other duties. Page 20
Safety monitoring systems The employer shall designate a competent person to monitor other employees who shall be: Competent to recognize fall hazards. Able to warn the employee who appears unaware of the hazard or is acting in an unsafe manner. On the same walking/ working surface & within visual sighting distance. Page 21
