Proof of immunizations must be submitted during the school enrollment process. Parents of Incoming 7 th Graders - Your 7 th grader will need to have had a Tdap booster immunization prior to starting 7th grade in the 2014-2015 school year if they have not had one since they turned 7 years of age. Student may be excluded from school until proof of required immunization is provided. Student Information Change If your family moves from one Perrysburg address to another, updated proof of residency must be provided to the office (copy of lease, purchase agreement, or property tax statement). Please print clearly when appropriate and review and/or fill out each of the nine (9) sections completely before signing and dating the form. Permission to Display Photographs, Audio, Video or Electronic Images, Artwork and Stories I give consent (or do not give consent) for photographs, audio, video or electronic images of my student; original written materials, artwork or other work created by my child during the course of instruction; as well as quoted statements by my child to be used by the School District outside the school setting for exhibition, public display, publication, publicity materials, advertising, news media stories, video, audio or other electronic media such as the Internet, television, CD-ROM or DVD. I understand that my student s full name may also be used with such display. NOTE: This form will not impact a parent s choices concerning ordering school portrait photographs. Permission for use of School Technology My student and I have read and agree to abide by the Staff and Student Network and Internet Acceptable Use and Safety Policy and Guidelines 7540.03. We understand that any violation of the terms and conditions set forth in this policy and these guidelines is inappropriate and may constitute a criminal offense. As a user of the Board s computers/network and the Internet, my student agrees to communicate in an appropriate manner, honoring all relevant laws, restrictions and guidelines. I understand that the Acceptable Use Policy and Guidelines are posted on the school s web page and can be obtained from the school office. Violation of the AUP may result in disciplinary action. Upon the consent signatures on this document, we verify that we have reviewed the policy and give consent. 4. Permission for Directory Information I give consent (or do not give consent) for my student s name, grade level, address and phone number to be listed in the school directory and district listing. Directory information shall not be provided to any organization for profitmaking purposes. Student directory information may be disclosed without prior written consent unless the parents have withdrawn their consent using this form. Other than requests for directory information as described above, school officials release information from, or permit access to, a student s education records only with the prior written consent of a parent, guardian or eligible student, except that the Superintendent or a person designated in writing by the Superintendent may permit disclosure in certain limited circumstances. Student Handbook Certification My student and I understand the rights and responsibilities pertaining to students and agree to support and abide by the rules, guidelines, procedures and policies of the School District as reflected in the student agenda/ handbook. 7. High School Students Only Transcript Requests Upon student request, I give consent (or do not give consent) for Perrysburg High School to send transcripts to postsecondary institutions as well as scholarship organizations. Transcripts include all previous courses with semester grades and credits earned, senior schedule, grade point average, attendance and all standardized test scores reported to Perrysburg High School. Transportation Responsibilities of Students & Parents for Regular Transportation & Field Trips Pupil Transportation Management Policies Pupils shall arrive at the bus stop at least 5 minutes before the bus is scheduled to arrive. Pupils must wait in a location clear of traffic and away from the bus stops. Pupils must go directly to an available or assigned seat so the bus may safely resume motion. Pupils must remain seated, keeping aisles and exits clear. Pupils must observe classroom conduct and obey the driver promptly and respectfully. Pupils must not throw or pass objects on, from or into the bus. Pupils may carry on the bus only objects that can be held in their laps. Pupils must board and depart the bus at locations to which they have been assigned unless they have a note signed by their parent and building principal for authorization to do otherwise. Responsibilities of Parents Parents of students being transported by Perrysburg Exempted Village School bus should expect their children to be home anytime after the last dismissal bell rings at their respective school building. Parents have responsibility of seeing that students are orderly while awaiting the bus. Parents will be responsible for any damage done to a bus by their children and will be required to make restitution to the Perrysburg Board of Education. All half-day kindergartners must have a parent, guardian or caregiver present and visible to the bus driver in order to be released from the bus midday. If no one is present and visible, the kindergartner will remain on the bus and return to their building of attendance. The building secretary will contact parent/guardian. Emergency Dismissal It may become necessary to close school early due to weather conditions or other emergency conditions that would make staying in school unsafe for students. At such time, it is extremely difficult to contact parents to determine where each child should go. We are asking you to make advanced plans in case this situation occurs. Vacations are unexcused absences. Therefore, if this form is not completed, work is not accepted for credit. Homework Policy: A sixth grade policy has been established and is explained in the Junior High Handbook. However, the homework policy may vary according to the teacher. Each teacher will post weekly activities/homework on Schoology, http://lms.perrysburgschools.net. This website is a helpful tool for parents and students. Ultimately, it is the student's responsibility to write their assignments in their assignment notebook. Parent Communication: One of the most exciting aspects of teaming is the opportunity it provides for better communication among students, parents and teachers. These supply requests will be given to students on the first day of school. Seventh grade is much different from what you have experienced in previous years. You will find it necessary to be more independent and responsible for yourself, your things, and your actions. Included in this newsletter is some basic information that will help you to feel confident and prepared. You will also know what some of our expectations are. Please read over this information carefully with your parents. Expectations: HONESTY and RESPECT at all times! Students are expected to be honest and respectful not only towards their teachers, but also to their peers. Absences: Be sure to turn in all homework that was due the day you were absent AND get anything that you missed. Find out when it is due, and then TURN IT IN! Schoology: We encourage you and your parents to check your grades and assignments online with Schoology. If you have forgotten or misplaced your password, parents can obtain this from the junior high office.
