Longlands Child Development Centre Pre-School Referral Information Leaflet

What is Longlands Child Development Centre? Longlands Child Development Centre is a place where the Early Years Services are based and where NHS staff can assess your child who may have additional needs. Why do I need to go there? You will have been referred to us by your own Healthcare Professional, for example your GP or Health Visitor, to see another more specialist Healthcare Professional who may help and advise you further. Can I take someone with me? Absolutely! If you feel you need support when attending an appointment, you can ask a relative, partner, close friend or support worker to go with you. What will happen in the appointment? Your appointment will take between 1 hour and 1 1/2 hours where you will have the opportunity to talk to a Healthcare Professional and discuss your childs needs. During this time your child's level of development will assessed. From this assessment you will receive a report.

Who am I likely to see? The Early Years Services based at Longlands Child Development Centre have the following Healthcare Professionals available: Consultant Paediatric Team Clinical Psychology Team Speech and Language Therapists Occupational Therapists Physiotherapists Specialist Health Visitor. Things to Consider before your Appointment Whilst you are waiting for your appointment, if you would like to; keep a diary of your child's behaviour / triggers / reactions to things / dietary problems / anything you have tried, etc. as it will help with your appointment. It would help if you tell anyone who is involved with your child about your referral. Ask them to write down any relevant things so you can take this with you to your appointment. Please bring your child's 'Red Book' as this can be useful too.

Who can I talk to about this before my appointment? A Healthcare Professional may wish to contact you before your first appointment. Ask any Healthcare Professional for help or you can contact local support groups using this website www.northlancsdirectionsgroup.com. If you have any difficulties with your referral and would like to talk to someone about it, a Healthcare Professional from Longlands Child Development Centre or your local children's centre can help you. Travel Information - By Car From King Street, Lancaster, turn left onto Westbourn Road (via Market Street), and over the railway bridge. In approximately 300m Turn Left, onto Westbourne Drive which is sign posted Longlands Child Development Centre.
