Cardiac Rhythm Disease Management Products Medicare C-Code List for hospital outpatient device reporting Updated January 1, 2012 Documentation/Coding and Reimbursement Resources

Cardiac Rhythm Disease Management Products Medicare C-Code List for Hospital Outpatient Device Reporting Updated January 1, 2012 Procedure to device code edits apply to a subset of device-dependent APCs and their corresponding C-Codes (see Table 1). Device (C-Codes) to procedure edits were implemented by the Centers for Medicare & Medicaid Services (CMS) to ensure that both the procedure and the device components were appropriately billed. These edits are shown in Table 2. Table 3 includes the Add-on (+33225)/primary procedure pair requirements effective January 1, 2012. This information can be found in the January 2012 Integrated Outpatient Code Editor, transmittal 2370 dated 12-16-2011 at: http://www.cms.gov/transmittals/downloads/r2370cp.pdf CMS continues to release quarterly updates to their OCE (Outpatient Code Editor) at: http://www.cms.gov/outpatientcodeedit/02_oceqtrreleasespecs.asp#topofpage. These changes may include revisions to Table 1 and Table 2. This edit information can be found on the CMS website at: http://www.cms.gov/hospitaloutpatientpps/02_device_procedure.asp#topofpage. Medicare Learning Network articles can be found at: http://www.cms.gov/mlnmattersarticles/. While the use of most C-Codes will not facilitate immediate payment for the device, the codes and the charges assigned to each will be used by CMS in the development of future payment rates for procedures involving devices. For rate setting purposes of device-dependent APCs (Ambulatory Payment Classification), CMS eliminates claims that include a token charge, a device furnished without cost, with a full credit, or partial credit. CMS has indicated that it is important that hospitals continue to provide complete and accurate coding and charge information in order to ensure appropriate APC payment rates in the future. 1 Disclaimer: These coding suggestions and coverage guidelines do not replace seeking coding advice from the payer and/or your coding staff. The ultimate responsibility for correct coding lies with the provider of services. Please contact your local payer for interpretation of the appropriate codes to use for specific procedures. Medtronic makes no guarantee that the use of this information will prevent differences of opinion or disputes with Medicare or other third party payers as to the correct form of billing or the amount that will be paid to providers of service. Determining which C-Code is applicable for a specific item is the responsibility of the hospital. All products included in this listing are placed in the appropriate CMS category based on their intended function. For further information, please contact your Medtronic sales representative. This information is also available at http://www.medtronic.com/crdmreimbursement. 1
