The first sectin is intended t assist yu in preparing fr an adequate respnse t an influenza pandemic utbreak f a magnitude that may affect ne r several f ur peratins. The secnd sectin is meant t assist yu in preparing yur respnse in the event f an utbreak affecting ur business. Backgrund n Pandemic Influenza Infectin in Humans Althugh avian influenza A viruses usually d nt infect humans, mre than 100 cnfirmed cases f human infectin with avian influenza viruses have been reprted since 1997. Fr example, the Wrld Health Organizatin (WHO) maintains situatin updates and cumulative reprts f human cases f avian influenza A (H5N1). Mst cases f avian influenza infectin in humans are thught t have resulted frm direct cntact with infected pultry r cntaminated surfaces. Hwever, there is still a lt t learn abut hw different subtypes and strains f avian influenza virus might affect humans. Fr example, it is nt knwn hw the distinctin between lw pathgenic and highly pathgenic strains might impact the health risk t humans. (Fr mre infrmatin, see Lw Pathgenic versus Highly Pathgenic Avian Influenza Viruses n the CDC Influenza Viruses Web page). Because f cncerns abut the ptential fr mre widespread infectin in the human ppulatin, public health authrities clsely mnitr utbreaks f human illness assciated with avian influenza. T date, human infectins with avian influenza A viruses detected since 1997 have nt resulted in sustained human-t-human transmissin. Pandemic Characteristics and Impact Imagine if: 10 t 30% f yur emplyees are t sick t g t wrk n any given day; Cumulatively, a quarter f yur wrkfrce culd be ut fr three t fur mnths; Other businesses yu rely n are facing the same absentee rates A pandemic will nt be like a physical disaster. A pandemic has unique characteristics when cmpared with a mre typical disaster. Fr example: Widespread impact: The impact f a pandemic wuld likely be widespread, and nt lcalized t a single area; therefre there may be little utside assistance. Our Disaster Plan assumes that sme parts f ur rganizatin will be unaffected and can take up the required capacity. Nt a physical disaster: A pandemic is nt a physical disaster. It has sme unique characteristics that require implementatin f activities t limit cntact such as restrictin f mvement, quarantine, and clsure f public gatherings. Shuld pandemic influenza spread thrugh the United States, it culd be sme weeks befre the full impact n the wrkfrce was felt, althugh there culd be sme early impacts resulting frm clsure f schls r ther similar cntainment measures. Primary effect is n staffing levels: Unlike natural disasters, disruptin t business peratin in the event f a pandemic is anticipated t be mainly human-resurce riented. Sme experts have advised that business shuld plan fr up t 50% staff absences fr perids f abut tw weeks at the height f a severe pandemic wave, and lwer levels f staff absence fr a few weeks either side f the peak. Overall, a pandemic wave may last abut 8 weeks. Staff absences culd be due t many reasns: Illness/incapacity Sme emplyees may need t stay hme t care fr the ill Peple may feel safer at hme Sme peple may be fulfilling ther vluntary rles in the cmmunity Others may need t stay hme t lk after schl-aged children (if schls are clsed) Other impacts Supplies f materials needed fr nging activity culd be disrupted Availability f services frm sub-cntractrs may be impacted Demand fr services may be impacted Assumptins Pandemic preparedness planning is based n assumptins regarding the evlutin and impacts f a pandemic. The fllwing pandemic planning assumptins are frm the HHS (Health and Human Services) Pandemic Influenza Plan: Susceptibility t the pandemic influenza subtype will be universal. The clinical disease attack rate will be 30% in the verall ppulatin. Illness rates will be highest amng schl-aged children (abut 40%) and decline with age. Amng wrking adults, an average f 20% will becme ill during a cmmunity utbreak. Of thse wh becme ill with influenza, 50% will seek utpatient medical care. Estimates differ abut 10-fld between mre and less severe scenaris. Risk grups fr severe and fatal infectins cannt be predicted with certainty. During the annual fall and winter influenza seasn, infants and the elderly, persns with chrnic illnesses and pregnant wmen are usually at higher risk f cmplicatins frm influenza infectins. In cntrast, in the 1918 pandemic, mst deaths ccurred amng yung, previusly healthy adults. The incubatin perid (the time between acquiring the infectin until becming ill), fr influenza averages is usually 1-3 days. We assume this perid wuld be the same fr a nvel strain that is transmitted between peple by respiratry secretins. Persns wh becme ill may shed virus and can transmit infectin fr ne-half t ne day befre the nset f illness. Viral shedding and the risk fr transmissin will be greatest during the first 2 days f illness. Children will shed the greatest amunt f virus and, therefre are likely t pse the greatest risk fr transmissin. On average abut 2 secndary infectins will ccur as a result f transmissin frm smene wh is ill. Sme estimates frm past pandemics have been higher, with up t abut 3 secndary infectins per primary case. In an affected cmmunity, a pandemic utbreak will last abut 6 t 8 weeks. At least tw pandemic disease waves are likely. Fllwing the pandemic, the new viral subtype is likely t cntinue circulating and t cntribute t seasnal influenza. The seasnality f a pandemic cannt be predicted with certainty. The largest waves in the U.S. during 20th century pandemics ccurred in the fall and winter. Experience frm the 1957 pandemic may be instructive in that the first U.S. cases ccurred in June but n cmmunity utbreaks ccurred until August and the first wave f illness peaked in Octber. Phase 2 N new influenza virus subtypes have been detected in humans. Hwever, a circulating animal influenza virus subtype pses a substantial risk f human disease.
