FSC102 Non-Sponsored Firefighter Academy Application Estrella Mountain Community College offers a Firefighter Operations Certificate which consists of a group of college level courses in Firefighting Operations, Hazardous Materials First Responder and Emergency Medical Services. With the completion of the MCCCD General Education Core requirements and some additional restricted electives in related areas, a student may continue to earn an Associate of Applied Science (AAS) in Emergency Response and Operations. Fire Department Operations (FSC102) is a required class for the certificate and Track I of the AAS degree completion options. The EMCC Firefighter Academy follows the National Fire Protection Association (NFPA) 1001 Professional Firefighter Standards both in the classroom and in extensive hands-on experience, including live fire training. Upon successful completion of the Firefighter Academy, students are eligible to sit for the AZ State Firefighter I and II exam, which is IFSAC accredited. It is extremely important to begin class in good physical condition. Due to the nature of this program, students have the option and are encouraged to seek out fire department or other agency sponsorship. The student will serve in the capacity of Reserve, College Mentor, Recruit or Volunteer Firefighter with a Fire Department or Agency. STUDENTS WHO CHOOSE TO BE SPONSORED, MUST BE SPONSORED BY THE DEPARTMENT OR AGENCY FOR THE ENTIRE PROGRAM. STUDENTS WHO, FOR WHATEVER REASON, ARE NO LONGER EMPLOYED OR SPONSORED BY A DEPARTMENT OR AGENCY WILL BE DROPPED FROM THE PROGRAM. The Academy consists of the following college courses: FSC102 Fire Department Operations (11 credit hours) Upon successful completion, students will test for Firefighter I & II Certification from the Arizona Center for Fire Service Excellence (AZCFSE) Prerequisites: EMT104 Emergency Medical Technology FSC105 Hazardous Materials First Responder - 40 hour (3 credit hours) FSC130 CPAT(1 credit hour; CPAT from other certified agencies or businesses will be accepted) CPAT must be completed and passed within 6 months prior to first day of class Co-requisites: FSC134 Fitness and Conditioning for Firefighters - FSC134 is a co-requisite and must be taken during the same semester as FSC102 Fire Department Operations (3 credit hours) Important Notes: Classes will meet two nights a week, Tuesday & Thursdays, 6:00 PM to 10:00 PM and every Saturday from 8:00 AM to 5:00 PM. Sundays may be scheduled to meet class competencies. Textbooks for FSC 102 are as follows. One (1) complete set of NFPA approved structural firefighting personal protective equipment: hood, turnout coat, bunker pants, boots, and gloves INCLUDING positive pressure self-contained breathing apparatus (SCBA) and PASS unit are required. 2

EMCC will have turnout and SCBA equipment available for a limited number of students on a first come first serve basis. Any remaining students will need to have agency sponsorship and must complete the Sponsored Application Packet for FSC102 admission. Each student must have a physician signed medical release at time of registration. This medical release must be performed within six (6) months of the academy start date. FSC102 Physical/Medical Release Form Clearance for Participation in the Estrella Mountain Community College Fire Academy Entire Form to be completed by the Licensed Healthcare Provider (MD, DO, NP, or PA) The EMCC Fire Academy Class is a physically demanding hands-on class that requires 100% participation by the student every time the class meets. Activities that are involved with this class include, but are not limited to lifting, pulling, pushing, and carrying heavy objects. This may be done while crawling, climbing, running, and walking in bulky clothing. Environments in which students are expected to perform these activities include, but are not limited to dark, smoky, hot, crowded, confined, tight, and loud. It is essential that EMCC students be able to perform all of the above physical activities in this program. A health care provider MUST sign the Fire Science Physical/Medical Release Form prior to program start date (within six months) and indicate whether the applicant will be able to function as a Fire Science Program student. Health care providers who qualify to sign this declaration include a licensed physician (M.D.), (D.O.), a nurse practitioner, or physician s assistant. FSC102 Enrollment Process and Payment Options Guidelines for Admissions: Applicants seeking admission to the FSC102 Firefighting Operations I & II must contact the Occupational Education Department for assistance in completing the application process. The following original documents must be submitted for verification; copies will be made by the Occupational Education Department. Firefighter Academy Application Fire Science Physical/Medical Release signed by a licensed healthcare examiner (within six months of FSC102 start date) Proof of successful completion (C or better) of EMT104 Emergency Medical Technology Proof of successful completion (C or better) of FSC105 Hazardous Materials First Responder-40 hour (8 credit hours) Proof of successful completion (C or better) of FSC130 CPAT within six months of program start (CPAT from other certified agencies or businesses will be accepted) Signed FSC102 Prerequisite Verification Form Concurrent enrollment in FSC134 Enrollment Process: Once all forms have been completed and you have collected all required documents as described above please follow the steps below to ensure the timely processing of your paperwork and to get registered for FSC102. 1. Contact the Occupational Education department to schedule an appointment (An appointment is required ) to drop off your complete admission packet. Bring your COMPLETED Admission Packet to the Occupational Education Division. Bring original documents; photocopies will be made in the office as necessary. The program assistant will verify that all admission requirements have been met by completing your FSC102 Prerequisite Verification Form. 3. You will receive a stamped EMCC registration packet from the Program Assistant indicating your eligibility to enroll in the FSC102 course. 4. Please complete your required registration information and take to EMCC Enrollment Services office to enroll in the FSC102 course. 7

ERMA/STARS Instructions For Students Arizona Center for Fire Service Excellence In order to be eligible to test for state certification, or receive a state training certificate from AzCFSE, students must have an account in the ERMA registration system (Event Registration & Management Application). This database was formerly known as STARS. When students activate their accounts, they will receive an EIN (Electronic Identification Number) that must be given to the instructor to include on the class roster. The EIN also allows students to see all training certificates they have received from AzCFSE or the State Fire Marshal s Office in the past, or to enroll in a Fire School class. Students must retain their EIN to access their training record or enroll in future programs or workshops. Getting an account in ERMA: If you don t know your EIN, you may look it up on the ERMA website: Go to www.erma.az.gov & click Log In and then Lookup your EIN. Type your name and click Search. Do NOT select an agency and ALL agency codes will be searched (your account may have been created under several possible agencies). ERMA will send your EIN to the email address on file. If ERMA does not have the correct email address then see the instructions below (Updating Your Email Address). Once your email address has been updated you may log into ERMA to activate your account. If you don t have an EIN and you have never received a state certificate before: Go to www.erma.az.gov and click Log In, then click on the option for first time users. Type your name in the boxes provided and click Search. Then click New to create a new account. Fill in all requested information. It is very important that a valid email address be entered into ERMA because this is where your EIN and password will be sent. Students enrolling in fire school: If you are taking a Fire School class you must have an EIN from the ERMA system in order to enroll in the class. This is required to get credit for completing the training. Students in programs sponsored by fire departments or colleges: If you are participating in a program or workshop sponsored by a fire department or community college then you must give your EIN to the instructor to include on the roster so you will get credit for the training through AzCFSE. How to log into ERMA after getting your EIN: If you know your EIN and ERMA password, then go to the ERMA website (www.erma.az.gov) and click on Log In, then enter the information requested. If you know your EIN but don t have a password (or can t remember your password): Go to the website (www.erma.az.gov) and click LogIn and select Reset Your Password. Type your EIN in the space provided and the account will be activated. The password will be sent to your e-mail address. If ERMA does not have a valid e-mail address then see the instructions below for updating your email address. After the email address has been corrected, go back to ERMA (www.erma.az.gov) and activate the account by following the directions above. 10
