II. How to Log into the PQRS Quality Reporting System
Logging into the PQRS Quality Reporting System 1. Direct your web browser to www.mshregistry.com 2. Click on the Select button located under the heading of the US Oncology Network to login as a US Oncology Network Member
Your Personal Account Page Your PQRS reporting has been pre-registered Find your selection under Reports in Progress Click the title to launch
III. PQRS Quality Reporting System Walkthrough A portion of your data has been pre-loaded into your PQRS module. There may be additional data that you need to add in order to complete your report. The deadline for entering all of your data and completing your report is March 13, 2015. NOTE: You do not have to enter in all of this data in one session. You can continually log in and out of the PQRS Quality Reporting System to add data to your report.
Review and Confirm Your Individual NPI and TIN The NPI that should be listed on this screen is your individual NPI number The TIN should be the one you are using to submit your 2014 Medicare claims.
Validate Individual NPI and Billing TIN Verify your information including : Name, NPI and Tax ID
Online Attestation This attestation grants the registry permission to submit your data to CMS on your behalf.
Enter Email The email address that you enter on this screen will be sent to CMS along with your PQRS data and may also be used by CMS to distribute feedback reports or other information pertinent to your PQRS submission.
Data Entry
Progress Monitor The total number of patients entered versus the total required The Medicare patient count Measure reporting compliance - CMS requires that each measure reported have a performance score greater than 0% (or less than 100% for inverse measures). The system will track and display this to you in the progress monitor. If you need to see which measures have not yet been met, refer to the details section of the progress monitor. Those with a green check have been satisfied, and those without a green check still need to be addressed.
Your PQRS Data Has Been Pre-loaded Your PQRS data has been pre-loaded into your account for your preferred reporting module. The data which has been loaded into your account will contain your total denominator eligible patients for the reporting module you have preselected. This example is specific to the Oncology Measures Group, your selected reporting module may be a Measures Group or an Individual Measures Set. You may still need to add additional data to the registry to complete your report. To determine if additional data is needed, please review the high-level milestones presented in the Progress Monitor. To complete your report, you will first need to locate the encounters that are incomplete. These will be listed in the section labeled My PQRSwizard Patient Registry towards the bottom half of the screen. If you are reporting a Measure Group and the Progress Monitor indicates that your report is ready to submit and that no additional data is needed, you may simply click Continue to finish your report. If you are reporting on Individual Measures, you will need to ensure that you have reported on 50% of your Medicare patients before you proceed.
Registry Data The registry is tabulated to help you easily toggle between views of your patient data. The All tab contains ALL patient records that have been loaded into your registry. The Completed tab contains those records that do not require any additional data. The Incomplete tab contains the records that are incomplete and require your attention. Please note that any encounter information labeled with the status of Information Needed will not be included in measure scoring. The columns that display in the registry include: Patient ID use this field to identify the patient Last Updated this field indicates the most recent date when your patient data was uploaded into the registry Date of Birth Status records labeled Complete do not require any additional data entry. Records labeled Information Needed are incomplete and you will need to add additional data to the records to complete it. and Action the available actions are: Review: To review the patient record, you may click on the magnifying glass icon. Edit: To edit the patient record, you may lick on the pencil icon. Delete: To delete the patient record, you may click on the x icon.
Example: Editing and completing a patient record with status: Information Needed Clicking the edit icon will open an online form for the respective patient encounter. The data that you need to enter to complete the records will be displayed on the form and you simply need to select the appropriate response for to the question posed based on the patient s medical record. You ll need to do this for each incomplete patient record to satisfy the reporting criteria for your PQRS reporting method.
Remember To qualify for the incentive and avoid penalty, report one of the following: 1 measures group for a sample of 20 patients 9 individual measures, across at least three (3) NQS domains for fifty percent (50%) of eligible Medicare patients. To just avoid penalty: Individual Providers should report on three (3) measures, across at least one (1) NQS domain for fifty percent (50%) of eligible Medicare patients
Check Your Progress Monitor for Completion Status Update Check your Progress Monitor to track your progress. The PQRS Quality Reporting System will automatically track your reporting guidelines.
Advance to the Next Screen to Finalize Report Once are ready to finalize your report and mark it as ready for CMS submission, click on the Continue button in the bottom right corner of the screen.
Performance Scores Instantly Calculated Once you have submitted all of your patients, you ll be able to review your performance scores for each measure reported. If any data needs to be updated at this time, you can click the back button to update data; click on the Continue button in the bottom right corner of the screen to advance to the final step.
