3 Beginning EP Meaningful Use This is a step-by-step guide for the Medicaid Eligible Professional (EP) Electronic Health Record (EHR) Incentive Program Meaningful Use MEIPASS Attestation. To begin your attestation visit the MEIPASS log in page at: Enter your User Name and Password that was created in your epaces account. Check the I accept the terms and conditions box. Click Submit.
4 CMS Registration ID Enter your CMS Registration ID. This was issued when registration on CMS.gov was completed. If you do not have your CMS Registration ID contact the CMS Help Desk at Once you have entered your CMS Registration ID click Submit.
5 Confirm Your Information If any information needs to be corrected, return to correct your information, and resubmit your registration. Do this before proceeding with your attestation. Check your contact phone # and address. Check your Payee NPI and TIN/SSN If any information needs to be corrected, return to the CMS Registration & Attestation System, correct the information and submit your registration again. EPH22 If all the information on the page is correct click Begin Attestation.
6 EP Summary Page This is the EP Summary Page. Each section of the attestation is listed on this page. As you successfully complete each section a green check mark will appear under the Pass/Fail/Incomplete heading. Without a green check mark in each section MEIPASS will not allow you to submit the attestation. Begin with Eligibility. Click Edit.
7 Eligibility Answer the first question. FAQ EP12 When you answer YES to the first question the second question will appear. If you answer NO to either question, then you are NOT eligible for this program. Click Save at any time to save your progress. Select a Payment Year. Answer the Federally Qualified Health Clinic or Rural Health Clinic question. FAQ EP29 & EP 31
8 Eligibility Practice at FQHC or RHC If you answered YES to practicing at an FQHC or RHC follow these steps. If you answered NO to practicing at an FQHC or RHC continue to the next page. If you answered YES to practicing in an FQHC or RHC, select a reporting Start Date. MEIPASS will generate the End Date. Enter the FQHC or RHC Name. Enter both Patient Encounters fields.
9 Eligibility Eligibility Information Select the Eligibility Reporting Year. FAQ EP06 The Eligibility Reporting Year field will not appear if you do not select a payment year. Enter your Patient Volume Reporting Period Start Date. FAQ EP06 MEIPASS will generate the End Date. Answer the remaining questions in the Eligibility Information section.
10 Eligibility Eligibility Information After YES or NO is selected, some fields will expand requesting additional information. For Practice as a Pediatrician information refer to FAQ EP28 Practice as a Physician Assistant will auto select No unless you selected Yes to practicing at an FQHC or RHC. For Hospital Based Provider information refer to FAQ EP22
11 Eligibility Eligibility Information Selecting NO to Organizational Encounters & Alternative Patient Panel Volume is considered the Standard Patient Volume Method. FAQ EP06 For group aggregate data, select Yes to Include Organizational Encounters. MEIPASS will ask for the organization (group) NPI. FAQ EP06, EP19 & EP36 For information about Encounters Outside NY refer to FAQ EP25
12 Eligibility-EHR Certification Information Enter your EHR Reporting Period Start Date. This reporting period must come from within the payment year. FAQ EP24 MEIPASS will generate the End Date. EHR Certification Numbers for all systems used within the reporting period must be listed. FAQ M04 To add an EHR system, enter the EHR Certification Number & click Add.
13 Eligibility-EHR Certification Information If you are using two or more different systems add their Certification Numbers as well. When you have finished adding your EHR Certification Number click Save. To obtain your EHR Certification Number please visit the ONC website.
14 Eligibility-EHR Certification Information When you click save, MEIPASS will calculate your Medicaid patient volume. FAQ EP06 & EP28 Click OK and you will be taken to the EP Summary page. Click Cancel to stay on the page.
15 EP Summary After successfully completing the Eligibility portion of the attestation you should see a green check mark in the Pass / Fail / Incomplete column. You will now be able to complete the Meaningful Use sections of the attestation. FAQ EP41 & EP42 Click Edit to continue to the Core Measures.
16 Core Measures You may click the EP Summary button at any time to return to the EP Summary page. You must meet all Core Requirements. Begin by clicking the requirement underlined in blue.
17 Core Measures Select the Measure. Select the applicable Patient Records statement. If the Exclusion applies select YES. If you select YES for the Exclusion, there is nothing further to enter. If you select NO for the Exclusion, the data fields will open. If there are multiple measures, move to the next measure.
18 Core Measures Some of the Core Requirements only require a YES or NO answer. Click Next to save your answer and move to the next Core Requirement.
19 Core Measures Some of the Core Requirements only require a YES or NO answer to an exclusion. Click Next to move to the next Core Requirement.
20 Core Measures You have successfully completed the Core Measures when all Core Requirements have a green check mark in the Status column. When all requirements have been met you will have the ability to click the Proceed to Meaningful Use Menu Requirements button.
21 Menu Measures You must complete a total of 3 Menu Requirements. Complete exclusions will not count towards the minimum Menu Requirement, unless 4 or more objectives are excluded and you meet all remaining objectives.
22 Menu Measures First select if you will complete this measure. If YES, complete the additional questions. Select Patient Records. Decide if the Exclusion applies. If the exclusion does not apply, select NO The data fields will open. Enter the Numerator and Denominator.
23 Menu Measures This is an example of when you do not meet all 3 Measures. Family Health History does not meet the requirement. Any time you select a measure and do not meet the requirement you will see a red X. Click on the Menu Requirement you did not meet.
24 Menu Measures Select NO to I will complete this measure. Click Next Click Return to Menu Requirements. Family Health History now has a status of Not Applicable. There need to be 3 Green check marks. Select another Menu Requirement to complete.
25 Menu Measures You have completed the Menu Requirements when 3 green check marks appear. OR you are excluded from 4 menu measures and 2 Green check marks appear Click Continue to Clinical Quality Measures to continue to the CQM portion of the attestation.
26 Clinical Quality Measures You must complete at least 9 Clinical Quality Measures (CQMs), including measures from at least 3 of the 6 National Quality Domains. You may choose to complete the recommended measure sets. Or you may choose individual CQMs by clicking All Clinical Quality Measures or a specific domain.
27 Clinical Quality Measures This example shows the pathway of doing the set of Recommended Adult Measures. Choose Yes to I will complete this measure. Enter all required data for the measure chosen. Click Next Measure to proceed to the next CQM.
28 Clinical Quality Measures You will see green check marks for all measures completed When all Requirements have been met you will be able to click on the Return to CQM Landing Page button.
29 Clinical Quality Measures This example shows the result of completing the set of Recommended Adult Measures which has 9 CQMs from all 6 domains. NOTE: You are only required to complete 9 CQMs from at least 3 domains. The completed measures will show on the CQM Landing Page. When all Requirements have been met you will be able to click on the Return to EP Summary to Complete Attestation button.
30 EP Summary Complete Attestation On the EP Summary Page, you will see a green check mark for each section, which indicates Pass. When all sections have green checks, you will then be able to click the Complete Attestation button.
31 Signature Register Read the terms and conditions. Check the I accept the terms and conditions box. Click Register.
32 MEIPASS Attestation Click the link to the PDF. Print the PDF. The PDF must be signed by the provider and mailed before your attestation review will begin. Mail ALL the pages of the PDF.
