An example of an acceptable qualification for this position: Bachelor s degree in nursing or graduate of accredited school of nursing preferred, plus three (3) years experience in public health nursing and/or medical/surgical nursing, including palliative care and/or home health care experience with supervisory experience or equivalent combination of education and/or experience; must be a Registered Nurse; bilingual (Spanish) skills preferred. LICENSURE OR CERTIFICATION REQUIREMENTS: Must be a Registered Nurse licensed in the State of Ohio; must have a valid State of Ohio driver s license and maintain insurability under Health District policy; must have cardiopulmonary resuscitation certification (American Heart Association); CHPN (certified hospice/palliative care nurse) preferred; may be obtained after employment. POSITION DESCRIPTION Inside Page 1 of 3 JOB DESCRIPTION AND WORKER CHARACTERISTICS: JOB DUTIES in order of importance ESSENTIAL FUNCTIONS OF THE POSITION: For purposes of 42 USC 12101: 1. Develops, coordinates, and oversees Henry County Hospice program for the care of terminally ill patients and their families; provides professional nursing care by utilizing established nursing processes and procedures in accordance with regulations, policies, and procedures; assesses and evaluates patients; develops, writes, and implements plan of care; prepares and accurately maintains records; compiles and prepares reports for submission in a timely manner; regularly evaluates patient, family, and caregiver needs in accordance with policy and procedure; serves as quality assurance/performance improvement coordinator to assess, plan, make, and implement changes through utilization of data, research surveys, etc. in order to improve the quality of care and maintain compliance; acts in place of Home Health/Hospice Clinical Manager as required. 2. Assists Home Health/Hospice Clinical Manager in verifying compliance with state and Medicare guidelines (e.g., providing staff in-service on changes in regulations, etc.); provides strong knowledge base for all hospice nurses and staff (e.g., able to answer questions, provide resources materials and support, assist in problem solving, etc.); conducts and obtains condition reports on all hospice patients for IDT meetings and maintains hospice patient database. 3. Assists with negotiating contracts with long term care facilities; conducts in-service sessions with contracted facility staff on hospice care and regulations as requested and/or needed; orients hospice staff to contracted facilities; serves as liaison between contracted facility and hospice staff; serves as liaison between nursing staff and physicians and DME, should problems arise. 4. Works with Medical Social Worker, Home Health/Hospice Administrative Manager and Scheduler, Home Health/Hospice Clinical Manager and hospice volunteers to coordinate respite for families/caregivers; assists with training and assigns volunteers to patient care; trains new staff in hospice philosophy, care, routine medications charting, and forms. 5. Participates on quality improvement teams and/or employee work groups (e.g., identifies opportunities for improvement, contributes ideas for improving Health District operations and services, etc.). 6. Maintains all required licensure and certifications. 7. Demonstrates regular and predictable attendance; must be able to work assigned evenings, weekends, and holidays. 8. Meets all job safety requirements and all applicable OSHA safety standards that pertain to essential functions.

Attends meetings, conferences, seminars, and community events as directed; serves on boards and committees as assigned; promotes hospice services (e.g., gives presentations) in the community and secures donations through fundraising and the hospice bazaar. 10. Maintains confidentiality in accordance with Health District policy and all applicable laws and regulations. OTHER DUTIES AND RESPONSIBILITIES: 11. Serves as preceptor for students and orientees as needed; maintains supplies for hospice patients. 12. Participates in Health District disaster preparedness activities; may be required to report for work outside normal working hours during an emergency; carries a cell phone when on call. 13. Performs any and all other related duties as assigned or directed in order to promote, further, and ensure the effective and efficient operation of the Henry County Health District.
