Guidelines for Applying to the Chiba University Graduate School of Medical and Pharmaceutical Sciences Based on the 2014 Chinese Government Graduate Student Overseas Study Program As described below, the Chiba University Graduate School of Medical and Pharmaceutical Sciences (hereafter, the Graduate School ) is recruiting students sponsored by the Chinese Government Graduate Student Overseas Study Program operated by the China Scholarship Council (CGOP). 1. Eligibility Students from universities that have an inter-university or inter-departmental academic exchange agreement with Chiba University are eligible. (Note) Students who wish to apply to the Graduate School must confirm that their university has an academic exchange agreement with the Graduate School. To do so, they should inquire with the admissions office for the field in which they wish to enroll (see Attachment). 5. Application requirements and conditions The applicant must satisfy all four of the following qualifications. (1) The applicant must be enrolled in and recommended by a university that has an inter-university or inter-departmental academic exchange agreement with Chiba University and must be a candidate to receive a scholarship as a CGOP student; (2) An applicant to the 4-year doctoral program in the medical field must meet the requirements and conditions stated in Application Guidelines for the International Frontier Medical and Pharmaceutical Program: the Chiba University Graduate School of Medical and Pharmaceutical Science s 4-Year Doctoral Program in the Medical Field (October 2014 Enrollment), and an applicant to the 4-year doctoral program in the 1
pharmaceutical field or the 3-year doctoral program must meet the requirements and conditions stated in 2014 Student Application Guidelines for the Chiba University Graduate School of Medical and Pharmaceutical Sciences Doctoral Program (October Enrollment, 1st Selection). (3) The applicant must have language proficiency deemed sufficient to receive education and research guidance at Chiba University; (4) The applicant has received permission to apply from the applicant s desired academic advisor. Before applying, students should contact the University s Graduate School faculty member from whom they wish to receive academic guidance, confirm the content of the research offered and the conditions for acceptance into the program, and obtain permission to apply. 6. Application documents (1) A certificate issued by the candidate s home university which proves that the applicant is a candidate for the CGOP (2) A letter of recommendation from the candidate s home university signed by the president of the university or the dean of the candidate s graduate school (3) For the 4-year doctoral program in the medical field, the documents mentioned in Application Guidelines for the International Frontier Medical and Pharmaceutical Program: the Chiba University Graduate School of Medical and Pharmaceutical Science s 4-Year Doctoral Program in the Medical Field (October 2014 Enrollment) are required, while the documents mentioned in 2014 Student Application Guidelines for the Chiba University Graduate School of Medical and Pharmaceutical Sciences Doctoral Program (October Enrollment, 1st Selection) are required for the 4-year doctoral program in the pharmaceutical field and the 3-year doctoral program. (Note) Contact the admissions office for the field in which students wish to enroll if there is any document that they are unable to submit at the time of application. 7. Application period and procedure (1) Application period The application period for the 4-year doctoral program in the medical field is as stated in Application Guidelines for the International Frontier Medical and Pharmaceutical Program: the Chiba University Graduate School of Medical and Pharmaceutical Science s 4-Year Doctoral Program in the Medical Field (October 2013 Enrollment), and the application period for the 4-year doctoral program in the pharmaceutical field and the 3-year doctoral program is as stated in 2013 Student Application Guidelines for the Chiba University Graduate School of Medical and Pharmaceutical Sciences Doctoral Program (October Enrollment, 1st Selection). (2) Method of application The application documents mentioned in Section 6 should be sent by mail (using EMS or some other service that provides mail tracking) to the admissions office for the field in which students wish to enroll. 8. First selection The Graduate School will comprehensively assess applications according to the selection criteria that it has established (see Attachment), and decide on the applicants who pass the first selection. Applicants will be notified in early March 2014 of the results of the first selection, and a Letter of Conditional Acceptance and a Certificate of Tuition Exemption will be issued to successful applicants. However, acceptance will be cancelled if the applicant is not granted a CGOP scholarship. 9. Final selection Once an applicant accepted in the first selection passes the China Scholarship Council s screening and it is 2
decided that the applicant will receive a scholarship through the Chinese Government Graduate Student Overseas Study Program, the applicant should promptly send a Certificate of Qualification to Receive Aid from the National Overseas Study Fund and to Leave China and Study Abroad to the organization in charge of admissions at the Graduate School (or program) in which he or she wishes to enroll. Upon receiving the abovementioned certificate, the Graduate School will make a final decision about whether to accept the applicant. A Notification of Acceptance and a Certificate of Eligibility will be issued to applicants accepted in the final selection 10. Admission procedures Documents required for admission procedures (including the Certificate of Eligibility, an immigration document required to obtain College student visa) will be sent to successful applicants, who then must carry out the procedures by the date designated by the Graduate School. 11. Tuition and other fees Successful applicants will be exempt from examination, admission, and tuition fees. Note that the exemption period for doctoral students will be limited to the standard period required to complete the program. 12. Essential considerations (1) Prior to applying, all applicants should contact the admissions office for the field in which they wish to enroll (2) Incomplete applications may not be accepted. (3) Application documents will not be returned and may not be replaced or modified once submitted. (4) The admission may be cancelled if the application documents contain false information. (5) Personal information collected in the applicant selection process may be used for such activities as managerial and administrative activities, academic guidance activities, and activities related to research and study of applicant selection methods, in addition to screening purposes. 3
