Guiding Light School of Massage Therapy Enrollment Application Please submit a completed application to Guiding Light School of Massage Therapy at the above address along with a $50 application fee check or money order. Once the application has been received an interview will be arranged with the schools director. The interview process will determine the acceptance of each applicant into the program. The interview process will also allow the student to ask questions regarding the school and education style. The goal is to have a well-matched relationship between school and student. What do you plan to do with the training you receive from our program? A commitment to the class schedule is an essential part of this program. Every class is a vital part of this program. Consistent class attendance is a requirement. Will you be able to commit to the class time schedule? Do you have any physical, mental or emotional health issues? If so, please explain:
Emotional and spiritual health issues will be brought up within the class content; do you have any objections or issue with this? The course content recognizes the contributions of both western and eastern health practices; do you have any objection with this? Can you work independently with projects and assignments? It is necessary to read textbook chapters or other materials on your own to prepare for class, do you have the time and skill to do this? The class is a co-ed setting. Privacy screens and conservative draping practices will be used. Do you have any issues or reservations with a co-ed class? Guiding Light is a private learning institution which is not governed by the Iowa State Education Department. It is regulated by the Iowa Massage Therapy board which is part of the Department of Public Health. Ultimately, it is governed by the Iowa legislature. Because of this status, federal
education funds that are available to other higher learning institutions are not available for students of Guiding Light. It will be necessary for students to find their own funding. Banks will give personal loans if qualified, but banks will not give educational loans for this course. If you are already in a health related field, then there may be programs available for continued learning. Ask your employer if these fees would be covered if funds are available to you for educational purposes. If you have recently been unemployed or are currently on disability, then programs may exist for re-training. Are you financially prepared to commit to this course of education? Guiding Light is regulated by the State of Iowa; however, in order to obtain a license to practice massage in the State of Iowa, it is necessary to take a MBLE State Board Exam (Massage & Bodywork Licensing Examination), which is governed by the Federation of State Massage Therapy Boards. A 70% score is required to pass this exam. Guiding Light will assist in helping students prepare for the Massage & Bodywork Licensing State Board Examination by providing materials and council. Guiding Light is passionate about training massage therapists in a holistic view of health. We are different in our approach. The main theme is recognizing that a person is more than muscle tissues and that the body communicates what it needs. As a practitioner, your job will be to learn to listen and then respond. If there is a preconceived idea of what is best for someone or if you are someone with narrow vision or strong intellect, this course of study may not be a good match. It is sometimes necessary to forget what you already may think you know to be able to learn a new way of perceiving or thinking. Our class structure is holistic in nature. Every course is interrelated and takes on an inclusive learning style. This is unlike traditional classroom education, which dissects the learning into compartments. If you are more comfortable in a traditional classroom setting, then this course of study may not suit your needs. Our program is designed to give you practical knowledge. Textbook study is a part of the program; however, the reality of the profession is not always captured in the textbook. Our instructors have a wealth of practical knowledge, which will aid you in the learning process. It is important that the training you receive prepares you for entering the massage profession and making a living at it. It is a business and a career that requires more skill than intellectual knowledge. It is necessary to learn how to interact with people, to understand that trends occurring in our economy will affect your livelihood and to possess the ability to track the decisions made by organizations that will also affect your livelihood. There is much more to this career than just giving someone a good massage. It is important to understand the majority of Massage Therapy professionals are self-employed as employee options can be limited in the state of Iowa. Most health-related professionals are hired as employees to a particular health business. In this profession it will be necessary for you to create your future. Although self-employment status is the norm; many self-employed massage therapist are contracted by other businesses. It is important for you
to have personal discipline and motivation. Do you believe you have the patience and fortitude to create a future for yourself? Are you ready to take an exciting new path in life? It must be realized that once your have committed to this course of study you are financially committed. A full refund will be given with written withdrawal two weeks prior to the first day of class. Once class is in session NO REFUND will be given. (Refer to the two-page document titled, Guiding Light s Refund Policy in this packet). Do you understand the school s refund policy? Professional behavior of students in an expectation of this school. Personal situations will need to remain outside the classroom unless it pertains to the course of study within the classroom. If a student s behavior is disruptive to the teacher, fellow students or the classroom environment he or she may be dismissed. Students need to be respectful of the teacher and fellow students. Remember this is a classroom of others and not just for one. Learning to be in a group setting is part of the training of interpersonal professional relationships. The fellow students are your peers. We encourage students to first address issues on their own. If an issue within the classroom or with the school remains unresolved, then please schedule a time to meet with the director to discuss the situation. Personal issues that arise outside of the school setting are your personal responsibility. If a decision or situation will affect your ability to perform in the classroom, then please discuss the circumstances with the director. Transfer Credits will be $1.50 per credit. Students must test out of the class at the time of registration to receive those transfer credits. This assures the school that the knowledge is still active in the student s awareness. First Aid & CPR are required by the State of Iowa for license. If the student has a current certification at the time of enrollment, then it will not be necessary to take the 10 credits. However, if the certification expires within the time the student is in class, the student will be required to take the course for renewal. If the certification expires after the course is completed, then it is important for the student to know the state will require a current certification. Keep an eye on the expiration dates. Photo Release Form As a student of Guiding Light School of Massage Therapy s certification program, I understand that videos and photographs will be taken of me and my classmates throughout the year. These photograph and video opportunities may occur during class activities, lectures and any event affiliated with Guiding Light School of Massage Therapy. By signing this form, I give Guiding Light School of Massage Therapy the absolute right and permission to use these photographs in archival and/or promotional materials and publicity efforts. I release Guiding Light School of Massage Therapy any employee of Guiding Light School of Massage Therapy from liability for any violation of any personal or proprietary right I may have in connection with such use. Guiding Light School of Massage Therapy Refund Policy Withdrawal from the Guiding Light School of Massage Therapy Certification course: Students are required to give a hand written notice of withdrawal from the program and request for a refund. Notice must be signed and dated. A FULL refund of money paid (minus application fee, printed materials and book fee) will be given if: Written request is made two weeks (14 business days) prior to the first day of class. Student did not complete transfer credit eligibility tests or practicum. (A fee will be charged for instructor and/or directors time) Money paid has cleared our bank or credit card account. Any fees that were incurred such as credit card charges or bank fees will be taken from the refund. Student has not signed a re-enrollment form which requests the money paid for the program is applied to the next class session. Students who have met the above requirements will receive a refund, minus any expenses, within three weeks of refund request. If written request is not received within 14 business days prior to the first day of class, no refund will be given. Student would be eligible to transfer fees to the next class session offered. There will be no refund for printed materials and books. Student has the option to sell his or her books to future classes. Student is financially responsible for each day school is in session whether student attends class or not.
Students on payment plans are responsible for the entire program fee regardless if they decide not to continue the program and if they miss scheduled class hours. This is a financial agreement/contract and commitment. In the event it is necessary for Guiding Light School of Massage Therapy to pursue legal action, student will be responsible for the total dollar amount of legal costs and any other incurred costs. The massage therapy program is limited in the amount of students it can give the instructional training to; therefore, students are given a space in the program when the program fees have been paid or a down payment has been made. If the spaces are thus filled, Guiding Light School of Massage Therapy will inform other potential students no room is available.
