Parent/Guardian Satisfaction Survey FY This report is the annual Blue Water Developmental Housing, Inc. summary regarding parent/guardian satisfaction as of May. METHOD Satisfaction surveys were sent to all parents/guardian of individuals served by Blue Water Developmental Housing, Inc. The survey was conducted for the Program from January 10 through February 10,. This resulted in 40 surveys being sent via mail to parent/guardians of individuals served in the Program. Surveys for were conducted from March 31 through April 30,. This resulted in 136 surveys being sent via mail to parent/guardians of individuals served in the Program. As of 5.21.15 Parent/Guardians Completed Participation Rate Residential Surveys 40 23 57% Community Based Surveys 136 46 34% Total Surveys 176 69 39% Surveys 2014 152 33 22% The parent/guardian satisfaction survey identifies issues of concern and to help prioritize future goals for the organization. It is important to note that overall parent/guardians who took the survey reported satisfaction with the programs and services provided by Blue Water Developmental Housing. The data in this report reflect survey results for the organization as a whole with comparison to the prior year. As well as looking at both and Programs individually.
Parent/Guardian Satisfaction Survey FY 1. Does your son/daughter/brother/sister feel safe & comfortable with BWDH staff? 4% 96% 2014 2% 98% There was a slight decrease from 2014 to. Although the comments were very positive towards staff a few individuals does not feel safe and comfortable with other residents in the home. of the responses for feel safe & comfortable with BWDH staff. COMMENTS: She likes it better than when she had to climb stairs. Likes his home Is very safe and secure I can only go by their actions, seemingly they have not shown a problem With staff, yes however, not so safe with other peers. My son was injured twice this year. My brother is calm when at home with the staff. I believe he feels comfortable and safe with staff, not with another resident in the home though.
Parent/Guardian Satisfaction Survey FY My son has had his staff for a long time. He is non-verbal and they always come to his home in a positive manner, he feels safe and comfortable. My daughter and I are very pleased and happy with the services we receive. With most, at present very good He seems to feel safe. If there is something amiss, we can usually tell by his behaviors. He used to be hesitant in telling us of his problems at the house, but, now he is vocal. We are a satisfied with our worker I guess, can t talk She loves her staff My son loves his respite worker She loves her respite worker

The comment section found that many of the parent/guardians choose the services the individual BWDH serves. COMMENTS: We plan together I and his team choose services at IEP t capable I choose them as her guardian He is unable to choose on his own I do She needs assistance with decisions The staff and myself choose services
Parent/Guardian Satisfaction Survey FY Her care lady always asks her where she would like to go and what she would like to do. When, What, How? We work with him informing him what is available; however, he is not afraid to ask questions and request some activities. But I feel choices of services are persuaded at times. With our help He likes to walk With help from us I as a parent decide He likes a lot of the same things. Routines are important when he goes out with his female staff. She does contribute (somewhat) to the decision making. Guardian Somewhat Pretty much he does Somewhat the staff usually handles any problems We work together with our CMH case manager

Does the staff treat you and your son/daughter/brother/sister with dignity and respect? 2014 of parent/guardians of the individuals BWDH supports feel that the staff treats themselves and their son/daughter/brother/sister with dignity and respect. COMMENTS: More than ever, they take very good care of more than you expect From what I have seen yes, hope all staff is doing the same. They treat him with great care and affection I ve had instances that were debatable. Helpful The staff are wonderful, feels they are his family and is home to him Very well Staff is always welcoming to us, caring, attentive and they communicate well with us. Great, couldn t ask for better staff
Parent/Guardian Satisfaction Survey FY Very much so. You all do an outstanding job! As a rule they do They all show him the utmost dignity and respect. You can see it in how he smiles to be with his staff. Staff is super! He loves his time with staff. I ve never seen him so excited to spend time with anyone as he does with his staff.

COMMENTS: She has no problems with staff. The home supervisor is the best manager you could have. I think these meetings are quite helpful. Still working on goals. Can t really say either way, seems the same. In September 2014, after not saying a word for 44 years my brother twice said I Love you very clearly. Wow!!! He gets out more and does more than he ever has.
Parent/Guardian Satisfaction Survey FY Fantastic Very pleased with everything without you all, I don t know where we would be. If followed We recently had a meeting regarding the PCP goals and carry through in his home. There has been some improvement. We want him to be as independent as possible; however, we are not blind, willingness to do certain things around the house. Coaching is key for him. Goals are set but sometimes the goals can t be met in the time allowed because of his developmental abilities. t really The process and goals set up are great, but I wish the goals were enforced more. The goals are worked on every outing. They help work on his goals in the home and out of the home in the community. As long as there is follow through. Menus for staff to help them and checked to make sure follow through. Staff meetings for houses so all staff can communicate. BWDH needs to check each facility monthly not just see it on paper. My son doesn t do this

Residential received of the individuals surveyed are happy with BWDH services. While response was 98%. The comment section showed that the parent/guardian that was not happy with BWDH services was due to the need for more hours for care. COMMENTS: Residential Very I am comfortable at knowing that I can truly feel that I can communicate with BWDH Daughter seems happy at her group home Need more hours for care than 13 hours a week As long as my daughter is happy! Never had any issues with services

6. Suggestions for new services? Parent/Guardian Satisfaction Survey FY Residential They seem to be happier in the new house. The group home and I have discussed adding some things to the goals for the summer (so far). I would have to look into what could be offered. Something fun to get them out of the house more. YMCA passes A class- goes to the movies/mall/lunch/museum/park/trolley ride once a month. Maybe more community based places or programs for our challenged kids. All staff needs to be in-serviced not online classes. Medical appointments It would be nice if there were some sort of monetary fund to use to help clients learn about money and how to shop. We are pleased

COMMENTS: Residential Has wonderful management and staff! They do a wonderful job! Staff is amazing caregivers and works well with my daughter. I am very satisfied with BWDH services. We find working with BWDH is easy. They are reasonable and are open to suggestions to provide a quality life for those they serve. Everything is going great Very much so
Parent/Guardian Satisfaction Survey FY 8. Suggestions for doing things differently/improving services? Residential Everything is going great Staff is top notch professionals and our families are fortunate to know them. If can be set in place that the temperature at the home be at least 70 degrees. As it is now winter and the back of the house is colder than the front area. It was cold for too long and winter had set in. Like they say, if it is not broke don t change it. Next hire, younger, older 35-45 years I have made my suggestion already which was welcomed and responded to. BWDH admin inspections once a month to see if all follow through such as menus posted, cleaning, schedules posted for continuity. Trying to have more public places we might be able to take our challenged kids to. More open communication with staff and not just home coordinators. Things can be handled without getting so many people involved. I would also like to see a member of BWDH administrative staff at the person centered planning meeting. I feel that would improve the process and advent questions that may arise. Special Olympics offered & transportation provided for participation at Marine City site. A class should be offered that provided training practices for Special Olympics sports. I feel Special Olympics should be available for Olympians no matter where they receive services. 9. Would you like someone to contact you regarding your supports/services? I work with the case manager mostly on this and plan to work closer this summer. I would like someone to contact me if there are other services that the boys can receive that I am not aware of. need, we are very pleased., only if you want to contact us.
Parent/Guardian Satisfaction Survey FY RECOMMENDATIONS Overall, the responses to this survey, including the comments sections, were very positive and reflected a high degree of satisfaction. Blue Water Developmental Housing, Inc. should receive recognition for the high percentage of positive responses to the survey questions. The Parent/Guardian Satisfaction Survey results will be reviewed with the Management Team at our Program Services monthly meetings as a part of its continuous quality improvement efforts. At that time the Management Team will develop an Action Plan for any areas of concern. ACTION PLAN In 2014, the action plan was for all BWDH employees to complete a two day internal training in Working with People. To ensure effective communication and interactions with the individuals we support. As of February, 209 of 348 employees or 60% have attended Working with People.
