Ultra-Sensitive Exchange Consent Management Ultra-Sensitive Exchange Consent functionality has been added. HIPAA Privacy HIPAA privacy functionality has been updated to facilitate the electronic release and receipt of client documentation related to Ultra-Sensitive Exchange requirements. Updates include moving setup to the assessment level rather than the (hard coded) page level, such that any assessment can be configured as a PHI disclosure authorization, an acknowledgement of privacy practices, and/or a PHI disclosure form. (Multiple settings for the same assessment are allowed.) An option for PHI exchange has been incorporated. New questions have been created to capture additional information related to PHI disclosure authorizations, and columns to display this information have been added to the PHI Disclosure Authorizations pane in the Display HIPAA Privacy Documents Panel. A utility will run automatically when the promotion is applied that updates active assessments containing the current HIPAA pages such that they reflect the correct document type. Transmit Continuity of Care Documents A new utility has been added as Transmit Continuity of Care Documents (AZ2395UA). This utilty generates CCDs for Clients that meet the selection criteria and have a record that has been triggered based on settings for a designated External Entity in Ultra-Sensitive Exchange Setup such that the CCDs can then be batched for the External Exchange Entity. Updates for Meaningful Use Functionality has been updated to accommodate Meaningful Use attestation requirements. Meaningful Use Launchpad Fields and links have been added to the following branches of the Meaningful Use Launchpad (AZ2219EC): Setup > Global Settings > Continuity of Care Documents Setup > Measure Calculation Reporting In addition, multiple branches of the Meaningful Use Launchpad have been updated to include or remove a container around the fields that display on each branch. Core Objective 7: Patient Electronic Access Provider Portal Embedded access to the Cerner Network Provider Portal solution has been added. Client registration invitations for the Cerner Network Patient Viewer are initiated through the Provider Portal. A new view has been added as Measure Calculation - View, Download, Transmit Report (AZ2287RU) to report data according to the new 2014 requirements for Core Objective 7: Patient Electronic Access, Confidential and Proprietary to Cerner Corporation Page 4

which replaces Stage 1 Core Measure 12: Electronic Copy of Health Information and Menu Set Measure 5: Patient Electronic Access. Core Objective 8: Clinical Summaries Continuity of Care Documents The continuity of care document (CCD) functionality in the HomePage solutions has been updated to capture and render the following information to a CCD for CDA (Clinical Document Architecture) Release 2: IHE (Integrating the Healthcare Enterprise) Health Story Consolidation, Release 1.1 for Meaningful Use Stage 2: Care Plan Care Team Members Clinical Instructions Cognitive Status Diagnostic Tests Pending Encounter Diagnoses Ethnicity Functional Status Future Appointments Future Scheduled Tests Immunizations Medications Administered During Your Visit Preferred Language Procedures Race Reason for Referral Reason for Visit Recommended Client Decision Aids Referrals to Other Providers Smoking Status Summary Purpose Export Clinical Summary The export continuity of care document (CCD) functionality in the HomePage solutions has been updated to allow staff to export specific sections (data elements) of a clinical summary. These changes meet MU Stage 2, 170.314(e)(2) - Clinical Summary requirements whereby for each visit with an EP, a clinical summary CCD is to be provided to a client within a single business day. Core Objective 9: Protect Electronic Health Information (EHI) Auditable Events and Tamper-Resistance Continuity of Care (CCD) Type Audit Events An audit record is now created upon successful display, print, or void of an imported or exported CCD in the Continuity of Care Documents pane in the HomePage solutions. Additionally, an audit record is created upon successful reconciliation of a non-void, non-reconciled imported CCD. All audit records can Confidential and Proprietary to Cerner Corporation Page 5

be displayed in Display Audit Events (AZ2218EA) and reported on using through the Audit Events Listing (AZ2218RA). Core Objective 10: Clinical Lab-Test Results Lab Orders The Lab Order Detail panel in the Doctor's HomePage Lab Orders Pane/Lab Order Form has been updated such that up to three active diagnoses can be related to each lab order as required for Meaningful Use Stage 2 certification and HL7 guidelines. Lab Order Results Two new tables have been added to allow for the manual entry of lab results: Lab Specimen Types Table (AZ243EIG) and Lab Abnormal Flags Table (AZ243EII). Printed Lab Order Forms Printed lab order forms now include the location test code and diagnoses (if entered) for a lab order. Core Objective 14: Medication Reconciliation Clinical Reconciliation Maintenance Panel A new Clinical Reconciliation Maintenance panel has been added to meet Meaningful Use Stage 2 guidelines (specifically, 170.314(b)(4) clinical information reconciliation), whereby designated users are able to create reconciled lists of a patient s active medications, a patient s active problems (diagnoses), and a patient s active medication allergies by simultaneously displaying data from two list sources (a Continuity of Care Document (CCD)/Clinical Document Architecture (CDA) document and the client's chart) and allowing modifications to the lists. Diagnosis Entry Forms The diagnosis entry forms have been updated to include new External and External Source fields, allowing the source of diagnoses added to the system through the clinical information reconciliation process to be saved, as well as enabling users to document this information manually in the course of completing a diagnosis review assessment. Core Objective 15: Summary of Care Transition of Care Messages Functionality to support transition of care (TOC) messages that allow the staff to send a continuity of care document (CCD) as an attachment to a secure message has been added. These messages can only be sent to external entities (not clients) and each CCD assessment can only be sent once. Confidential and Proprietary to Cerner Corporation Page 6

Core Objective 16: Immunization Registries Data Submission Create HL7 v2.5.1 Immunization VXU Messages A new Clients field has been added to the Selections1 tab of Create HL7 v2.5.1 Immunization VXU Messages (AZ2299UA). Additionally, the Immunization Date Range now considers the Administration Date of the non-voided, final approved Immunization record that is included in the batch rather than the Final Approval Date of the Immunization record, as it did prior to this promotion. Core Objective 17: Use Secure Electronic Messaging Secure Electronic Messaging Secure messaging has been incorporated to allow staff to send/receive encrypted messages to/from clients and external entities through the HomePage solutions. Three new data files, Secure Messages (AZSECMES) and Secure Messages Body Text File (AZSMSGTX), and Secure Messages Attachments (AZSMSGAT), have been added to store the information about and the contents of these messages. Secure messaging is subject to audit logging, as established in Security Parameters Setup (AZ001EA). Menu Set Objective 3: Imaging Results Measure Calculation Imaging Results Report A new Lab Code Desc print element has been added to the Print Columns tab in the Measure Calculation - Imaging Results Report (AZ2287RS). Updates for Iowa HFS County Invoices An update has been made such that services going to the same county (pay source) that are provided under the same agreement number can be included on the same invoice. Requested by Hillcrest Family Services, Inc., IA. Updates for Oregon Updates have been made to support Oregon Measures and Outcomes Tracking System (MOTS) state reporting. Updates for Texas Assessment questions have been added to support Texas intellectual and developmental disabilities (IDD) prescription drug plan (PDP) pages for the Department of Aging and Disability Services (DADS). Requested by multiple Texas centers. Confidential and Proprietary to Cerner Corporation Page 7
