Dan Westphal Construction Services Project Manager EXPERTISE Mr. Westphal has more than ten (10) years of relevant experience in providing and managing construction inspection, observation and materials testing both in the field and laboratory. Mr. Westphal s responsibilities include the supervision of the laboratory and field testing of asphalt, earthwork, concrete, aggregate, foundations and masonry. Since his employment with CTL Engineering, Mr. Westphal has went through extensive training in construction observation for both field and laboratory asphalt testing, concrete testing, field density, nondestructive testing and many aspects of subgrade/foundation. EXPERIENCE A partial listing of CTL s relevant experience includes: Independent Assurance Testing / City of Indianapolis DPW / 2013 CTL is contracted with the DPW to provide Independent Construction Observation and Materials Testing Services. CTL performs laboratory and field testing of construction materials in accordance with ASTM and ASHTO standards in adherance to the DPW and INDOT Frequencey of Testing Manual. Additinonally, CTL is responsible for interpreting laboratory reports and interpret Field Reports and consultation in the event of unforseen conditions. Our duties include inspecting and testing of soils, concrete, grout and asphalt concrete material. Also, our duties involve pavement coring and providing reports. Crooked Creek Watershed Study / Indianapolis DPW / 2013 Geotechnical Engineering. The purpose of the project was to evaluate potential flood mitigation features as part of an alternative improvement initiative of the Crooked Creek Watershed in Indianapolis Indiana. CTL Engineering was contracted to assist in the analysis. The scope of CTL s work involved review of available geotechnical information for about ten (10) sites along Crooked Creek, perform a field geotechnical investigation of these sites and support the designer with geotechnical concerns. The subsurface soil investigation provided critical information necessary to assess constructability of three promising alternatives and to help determine the most preferred alternative before the owner can enter into the future design phase. CTL performed percolation testing of 4 (four) sites with three depth levels. The findings of the representative samples were characterized and provided soil infiltration parameters. Indianapolis Airport Authority (IAA), Stormwater and Deicing Controls and Capacity Phase 2, Indianapolis, Indiana, 2013. Geotechnical Engineering Services. The project consisted of the design of a 72 Gravity Storm Sewer, Lift Station, 14 Force main and 3 million gallon New Storage Basin. CTL performed all of the drilling and laboratory analysis for this project. CTL s geotechnical report included; Existing subsurface conditions at the boring locations, Site preparation and earthwork recommendations, foundation recommendations and soil parameters required for the design of the proposed gravity storm sewer, force main, lift station and storage basin. Southport AWT Capacity Expansion Project / Citizens Energy Group / 2013 CTL Engineering was contracted to perform the geotechnical engineering services on this project including field investigations and associated laboratory testing. CTL drilled over 2,200 linear feet of soil borings for the proposed improvements. Soil samples obtained from the drilling operation were preserved in glass jars and visually classified in the field and laboratory. Representative soil samples were tested for Natural Moisture Content, Grain Size, ph, Atterberg Limits, Loss on Ignition and Unconfined Compressive Strength. CTL s geotechnical engineering report included; site preparation, earthwork, seismic considerations, corrosion protection, groundwater management recommendations and foundation recommendations for all of the proposed structures and associated piping.
Harbour Waters Groundwater Treatment Plant Replacement / Citizens Energy Group / 2013. The project involved the design of an upgrade to the existing Harbour Water Treatment Plant in Cicero, Indiana. CEG plans to demolish the existing facility and constructing new Filters and Main Treatment Building, two Aeration and Retention Tanks, two Finished Water Tanks and Backwash Storage Basin and Water Pump Station. CTL Engineering was contracted to perform the geotechnical engineering services on this project including field investigations and associated laboratory testing. CTL s geotechnical engineering report included general site preparation and earthwork, groundwater management and foundation recommendations. Indianapolis Airport Authority / Old Terminal Demolition / Indianapolis, Indiana, 2013. Construction Observation and Materials Testing Services. The project involves the demolition of the old Indianapolis Airport Terminal. CTL is contracted with the IAA to perform Construction Services including; construction materials testing services for Demolition of Old Terminal and Associated Structures. CTL is providing the following services: Perform QA/QC on demolition contractor s methods, materials placement, and testing in accordance with plans and specifications; Perform compaction testing and construction observation of fill material, and Report field and laboratory test results. Marion County Bridge 2106L / City of Indianapolis DPW / 2012. Geotechnical Engineering. The project included improvements to the bridge. The existing 3- span bridge on 42 nd Street over Indian Creek is planned to be widened on both sides of the existing structure. CTL s role consisted of drilling, soil sampling, laboratory testing, and foundation recommendations for the proposed bridge widening. Marion County Bridge 0101F / City of Indianapolis DPW / 2012. Geotechnical Engineering. The project included rehabilitation to the Moore Road over Sheets Creek Bridge in Marion County. The existing single span arch bridge on Moore Road over Sheets Creek will be rehabilitated to include possible widening of the existing bridge about 2 feet on each side and construction of new wingwalls. CTL s scope consisted of concrete coring through the bridge arch, drilling, soil sampling, laboratory testing, and foundation recommendations for the proposed widening. Marion County Bridge Pleasant Run Parkway Bridge over Pleasant Run / City of Indianapolis DPW / 2011. Geotechnical Engineering. The project involved replacement of the existing bridge with a single-span structure. Additionally, the project involved roadway improvement approximately 200 feet west and 350-400 feet east of the stream crossing. CTL s work consisted of drilling, soil sampling, laboratory testing, and foundation recommendations for the replacement of the existing bridge and roadway improvements. Drilling, soil sampling, and foundation recommendations were performed following LRFD and INDOT guidelines for geotechnical investigation. Allisonville Road Rehabilitation / City of Indianapolis DPW / 2011. Geotechnical Engineering. The project involved roadway rehabilitation on Allisonville Road approximately 5,900 feet in length, starting at about 100 feet south of 86 th Street to 100 feet north of 96 th in the City of Indianapolis. The proposed pavement treatment was patching and
resurfacing of the existing PCC pavement. Additionally, the existing roadway was widened six feet into the medians. CTL s scope consisted of drilling, soil sampling, laboratory testing, and foundation recommendations for the proposed roadway rehabilitation. Camden and Thompson STEP Project / City of Indianapolis DPW / 2012. CTL Engineering provided geotechnical drilling, laboratory testing, and site preparation and foundation recommendations for this project. The field investigation consisted of drilling a total of 13 test borings to an average depth of 25 feet each for a total drilling length of 325 feet. Additionally, the existing pavement was cored at the thirteen (13) test boring locations. The existing pavement at or near each boring location was cored, photographed and evaluated. Soil data obtained from field and laboratory testing was used to determine the existing soil conditions, and to prepare recommendations for site preparation, earthwork and foundation support. CTL Engineering provided Construction Observation and Materials Testing services for this project. Testing services include: Testing compaction and sampling of B Borrow backfill and No. 53 aggregate sub-base and Concrete sampling and testing. Mr. Karaki was responsible for technical review of written testing reports. Broad Ripple Dam Rehabilitation / Citizens Energy Group / 2013. The project consisted of rehabiliation of the existing dam on White River. CTL provided the construction observation and materials testing sevices on this project. Fall Creek/Kestone Avenue Dam Rehabilitation / Citizens Energy Group / 2013. The project consisted of rehabiliation of the existing dam on Fall Creek. CTL provided the construction observation and materials testing sevices on this project. Multiple Hearth Improvements / Belmont AWT / Citizens Energy Group / 2013 The project involved design and construction of the multiple hearth incinerator complex consisting of four multiple hearth incinerators (MHF) at the Belmont AWT. CTL s scope included geotechnical engineering of the four (4) pad foundations for new equipment on the north side of the existing Incinerator building. Beech Grove Water Tank / Water Line and Booster Station / Citizens Energy Group / 2013 The project included the design and construction of a 16-inch water line under I-465 from the water tank to the existing 16-inch water line along the south side of I-465. Also, a booster station was constructed on a concrete slab at or near the existing grade, southwest of the water tank. CTL s scope included all of the geotechnical engineering services on this project. 38th Street and Kessler Boulevard STEP Project / City of Indianapolis DPW / 2012. Geotechnical Engineering. CTL Engineering provided geotechnical drilling, laboratory testing, and site preparation and foundation recommendations for this project in 2010. The field investigation consisted of drilling a total of 11 test borings to an average depth of 25 feet each for a total drilling length of 275 feet. The existing pavement at or near each boring location was cored, photographed and evaluated. Soil data obtained from field and laboratory testing was used to determine the existing soil conditions, and to prepare recommendations for site preparation, earthwork and foundation support.
