SCHOLARSHIP PROGRAM 2015-16 Attached is the list of various scholarships which are available at this time. Please note the following procedures as you select those for which you wish to apply. 1. Scholarships listed in this packet are available to all Milton High School seniors. Unless otherwise noted, applications are available on the school district website at www.milton.k12.wi.us. Click on the counseling link. 2. BEWARE OF DEADLINES. Scholarships are due in the counseling office by deadlines listed in this packet, not deadlines printed on the scholarship. We have to allow time for processing and mailing. LATE SCHOLARSHIPS WILL NOT BE ACCEPTED NO EXCEPTIONS! 3. The scholarship packet is updated frequently. Check emails for announcements, stop in the counseling office or check the counseling website for updated lists. 4. Some scholarships require essays, photographs, letters of recommendation, etc. and it is THE STUDENT S responsibility to take care of this. Don't expect letters of recommendation to be completed in a few days. Contact people for letters of recommendation well in advance so they have adequate time to write the letter. DIRECTIONS FOR INDIVIDUAL APPLICATIONS (All scholarships that follow require individual applications) Unless otherwise noted, individual applications must be picked up in the counseling office. Some scholarships may require additional materials such as essays, letters of recommendation, a photograph, etc. Make sure you read each scholarship s criteria carefully. 3. MHS DRAMA SCHOLARSHIP DEADLINE: MARCH 16, 2016 AWARD: $500 1. Graduating senior planning to attend a four year college or university. 2. Must have participated in at least one theater production. 3. Must maintain at least a 2.0 GPA during the first semester to collect the scholarship award. 7. ROCK COUNTY 4-H FAIR ALUMNI SCHOLARSHIPS DEADLINE: FEBRUARY 15, 2016 AWARD: Thirteen of $500 1. Graduating senior who is a current or previous member of 4-H or FFA who have participated in the Rock County 4-H Fair. 2. Transcript, two letters of recommendation, and photograph required. 3. Students must mail application themselves directly to the address listed on the application.** 11. HARRY H. BLISS MEMORIAL SCHOLARSHIP DEADLINE: MARCH 15, 2016 AWARD: Two of $1,000 1. Graduating senior who is in the top half of the class and preparing for a career in communications or journalism. Preference will be given to those who express special interest in news writing and reporting, editorial writing, editing, news photography, advertising, broadcast announcing and production, broadcast engineering, or other areas having application in communications media. 2. Essay, one letter of recommendation, photograph, transcript, and ACT score report are required. 12. BADGER STATE ETHANOL SCHOLARSHIP DEADLINE: FEBRUARY 24, 2016 AWARD: One of $1,000 1. Graduating senior going into an agricultural related field. 2. May be used at any post-secondary school in Wisconsin. 3. The scholarship will be paid upon successful completion of first semester and proof of enrollment for second semester. 4. One letter of recommendation required (must not be from a relative).

13. ST. MARY KNIGHTS OF COLUMBUS SCHOLARSHIPS DEADLINE: MARCH 15, 2016 AWARD: Four of $400 1. Graduating senior and member of St. Mary parish who will be attending a college or vocational school. 2. Selection based on scholastic achievement; need; character; and involvement in church, school, and community. 20. ROCK ENERGY COOPERATIVE SCHOLARSHIP DEADLINE: JANUARY 5, 2016 AWARD: Undetermined number of $500 1. Graduating senior who will be continuing his/her education at an accredited school. 2. Based upon general merit, cooperation, leadership abilities, financial need, and class rank. 3. Parent or guardian must be an active member of REC. 4. Transcript required. 5. YOU must ask your counselor to fill out the second page of the application. 6. AB NICHOLAS SCHOLARSHIP DEADLINE: APRIL 30, 2016 AWARD: Up to 60 of $10,000 for four-year programs renewable or $ 5,000 for two-year programs renewable 1. Live in Wisconsin and attend a Wisconsin public high school. 2. Participate your senior year in your school s boys or girls basketball program, as a player or manager. 3. Must plan to attend a University of Wisconsin System school. 4. Based on financial need. 5. Apply online at www.abnicholasscholars.org. 27. MILTON MARLIN SWIM CLUB SCHOLARSHIPS DEADLINE: MARCH 15, 2016 AWARD: Undetermined # of $200 1. Graduating senior pursuing any post high school program who is a present or former Marlin swimmer and a member of the high school swim team for all four years. 30. PAGEL GRAPHIC ARTS SCHOLARSHIP DEADLINE: FEBRUARY 24, 2016 AWARD: Undetermined number of $500 to $1,500 Eligibility Criteria: 1. Graduating senior planning to attend school full-time in Wisconsin and pursue some form of post-secondary education in the graphic communications industry. 2. Scholarships will be awarded based on academic performance, outside activities, and financial need. 3. Transcript is required.

32. CHEWNING LEGAL, LLC SCHOLARSHIP DEADLINE: APRIL 30, 2016 AWARD: One of $500 Eligibility Criteria: 1. Must have a minimum 3.5 GPA. 2. Scholarship is for graduating high school seniors in Wisconsin. 3. Must be attending a 4-year college or university. 4. A transcript with counselor's or school representative's signature must be submitted for consideration. 5. A 1,000 word minimum/2,000 word maximum essay explaining the manner in which the applicant s parent s disability has impacted their life, education and goals. 6. Proof that parent(s) currently receives Social Security Disability or SSI benefits (e.g., a copy of a letter or notice from Social Security regarding the benefits). 7. Winner will be notified via mail and announced on our Facebook page and our blog page. 8. Although not required, applicants are encouraged to submit recommendation letters and/or resumes to substantiate their application. 9. Online applications only. AXA ACHIEVEMENT SCHOLARSHIP DEADLINE: DECEMBER 15, 2015 AWARD: One of $10,000 (an extra $15,000 possible if chosen as one of ten national winners) 1. Graduating senior planning to enroll full-time in an accredited two-year or four-year college or university. 2. Student must demonstrate ambition and self-drive as evidenced by outstanding achievement in a nonacademic activity or project in school, the community, or the workplace. 3. THE FOUNDATION OF THE WISCONSIN AUTOMOBILE & TRUCK DEALERS SCHOLARSHIP AWARD: Vary and may be valued up to full tuition DEADLINE: JANUARY 15, 2016 1. Be a Wisconsin resident. 2. Have a valid driver s license with a good driving record. 3. Be a high school graduate or high school senior graduating in 2016. 4. Plan to attend or currently be enrolled (with an anticipated graduation date no earlier than the spring of 2017) in an automotive, auto collision or diesel technician program at a NATEF-certified Wisconsin technical college. 5. Plan to pursue a professional career in the automotive, diesel or auto collision industry. 6. Successfully complete a series of assessment evaluations at one of the testing sites listed on the Scholarship Assessment Form 37. BOWER CITY GARDEN CLUB GREEN SCHOLARSHIP DEADLINE: MARCH 16, 2016 AWARD: Two of $1,000 1. Graduating senior pursuing a career in one of the following fields: horticulture, environmental studies, conservation, botany, resource management, forestry, landscape design, or other garden related field. 2. Letter of recommendation required. 3. Selection based on past performance and recommendations.

38. J.P. CULLEN & SONS, INC. SCHOLARSHIP DEADLINE: MARCH 28, 2016 AWARD: One of $1,000 1. Must be entering the construction field (Bachelor of Science in Engineering/Construction or Technical College Associates Degree). 2. Photograph and brief essay required. 39. SFM FOUNDATION SCHOLARSHIP DEADLINE: MARCH 28, 2016 AWARD: Up to $10,000 per year (for up to five years) 1. Graduating senior who is the natural, adopted, step-child, or full dependent of a worker injured or killed in a work-related accident during employment with a Minnesota, Iowa or Wisconsin-based employer and entitled to receive benefits under the Minnesota Workers Compensation Act or Worker s Compensation Act of Wisconsin. 2. Must provide the following: transcript, verification of college acceptance, estimate of school costs, onepage explanation letter (educational goals and need for financial assistance), copy of your recent school identification card or driver s license, two original letters of recommendation, brief description of nature and extent of parent or legal guardian s injury, death certification of parent or legal guardian (if applicable), and first report of injury. 3. Must be a resident of the United States. 2. Must be enrolled full-time at an accredited four or two-year College or trade school. 3. Students must hold a minimum GPA of 3.0. 4. WISCONSIN PROFESSIONAL POLICE ASSOCIATION SCHOLARSHIP DEADLINE: JANUARY 28, 2016 AWARD: Undetermined-Committee will decide 1. Scholarship is for graduating high school seniors in Wisconsin. 2. Graduating senior who is planning to enroll in a full-time course of study at a college, university, or vocational/technical school in police science, criminal justice, or a law enforcement-related program. 3. Must provide the following: Transcripts and 3 letters of recommendation. 4. Brief essay is also required.

42. AXA ACHIEVEMENT COMMUNITY SCHOLARSHIP DEADLINE: FEBRUARY 1, 2016 AWARD: Up to twelve of $2,500 1. Graduating senior planning to enroll full-time in an accredited two-year or four-year college or university. 2. Student must be ambitious and motivated, as evidenced by outstanding achievement in school, community, or work activities. 3. Only the first 10,000 applications will be accepted. Apply early! 4. ENGINEERING SCHOLARSHIPS DEADLINE: DECEMBER 17, 2015 AWARD: Two of $2,000 (Engineers Foundation of Wisconsin Scholarships) One of $2,000 (WSPE Past Presidents Scholarship) One of $2,000 (Behling Scholarship) Three of $2,000 (Louis C. Geiger Scholarship) One of $2,000 (Yu-Hwa Terry Ni FREE Program Scholarship) 1. Graduating senior with a minimum GPA of 3.0 (on a 4.0 scale) and minimum ACT score of 24 who is a U.S. citizen and Wisconsin resident. 2. Must plan to study engineering at a four-year ABET accredited program leading to a baccalaureate degree in engineering, and intend to enter the profession of engineering upon graduation. 3. Essay (250 words), transcript, a copy of ACT score and counselor signature area required. 4. Fill out application online at www.wspe.org/efwi.shtml, print it out and turn in to the counseling office. 44. SOCIETY S ASSETS SCHOLARSHIPS DEADLINE: JANUARY 28, 2016 AWARD: Undetermined number up to $25,000 1. Graduating senior who has a permanent and substantial (as opposed to a minor) physical or sensory disability and plans to enroll on a full-time basis in an accredited college, university, or technical school. 2. Must be a resident of Racine, Kenosha, Walworth, Rock, or Jefferson counties. 3. Transcript (need unweighted GPA for application; ask counselor to re-figure), physician s verification form, three recommendation forms, and essay required. 45. AMERICAN BUSINESS WOMEN S ASSOCIATION PARK CITY CHAPTER AWARD: Up to Five of $750 DEADLINE: JANUARY 27, 2016 1. Graduating senior who is a resident of Rock County, Wisconsin; is a U.S. citizen 30 days prior to application and will be attending a college or technical school full time. 2. Must have at least a 2.5 GPA (on a 4.0 scale). 3. Transcript (including this year s first semester grades if available), three letters of reference (one each from three of four categories listed), and a typewritten student profile statement (not more than 300 words) required.

46. VIETNAM VETERANS OF AMERICA ROCK RIVER CHAPTER #236 SCHOLARSHIPS DEADLINE: MARCH 16, 2016 AWARD: Eleven of $300 (Vietnam Veterans of America Scholarship) Two of $300 (Tim Kettle Memorial Scholarship Fund) One of $300 (Albert H. Kath Memorial Scholarship Fund) Eligibility Criteria: 1. The applicant s father, mother, or grandparent must be Vietnam-Era Veterans who were on active duty between January 1, 1959 and May 7, 1975. Please note: service did not have to be in the Republic of Vietnam. 2. Application must be accompanied by proof of service dates (DD-214 or equivalent document). 3. Must include photograph. 47. ROCK COUNTY ASSOCIATION FOR HOME AND COMMUNITY EDUCATION SCHOLARSHIPS AWARD: One of $500 DEADLINE: MARCH 11, 2016 1. Graduating senior who is interested in going into a community service field (education, extension, nursing, police science, fire safety, social work, etc.). 2. Transcript and three letters of recommendation are required. 3. First considered a relative in the organization. 48. BADGERLAND FINANCIAL SCHOLARSHIP DEADLINE: JANUARY 26, 2016 AWARD: Twenty-two of $1,000 Eligibility Criteria: 1. Graduating senior attending either a 4 year university, university short course, or a technical/trade school. (An Additional 5 points will be given to those who choose to study an Ag related field) 2. Must be the child/dependent of a Badgerland Financial customer in good standing. 3. Must have a 2.0 or higher GPA. 4. One letter of recommendation from a non-family member, photo and transcript required. 49. DELTA THETA SIGMA SCHOLARSHIP DEADLINE: FEBRUARY 15, 2016 AWARD: $1000 In-House Resident (up to 10 awarded) & $500 Academic Scholarship (up to 10 awarded) Eligibility Criteria: 1. Graduating senior attending either University of Wisconsin-Madison or Madison Area Technical College 2. Apply online at http://www.deltathetasigma.com/scholarship. 50. ROCK COUNTY AGRICULTURAL SCHOLARSHIP (UWE) DEADLINE: FEBRUARY 24, 2016 AWARD: Between ten and twelve of $300 to $500 1. Senior pursuing a career in agriculture or a related field at a college or technical school. 2. Transcript and three letters of recommendation are required.

FARM KIDS FOR COLLEGE AND STEVE PAVICH MEMORIAL SCHOLARSHIP AWARD: One of $750 (Steve Pavich Memorial Scholarship) DEADLINE: FEBRUARY 24, 2016 One of $250 (Steve Pavich Memorial Scholarship) Three of $1,000 (Farm Kids for College Scholarships) 1. Graduating senior who will be majoring in an agricultural field at an accredited college or university. 2. Two essays, two letters of reference, transcript, and a photograph are required.

DIRECTIONS FOR STANDARD APPLICATION (All scholarships that follow require the standard application) The standard application is found on the counseling office webpage at www.milton.k12.wi.us. You must save a copy to your computer before you complete it. Once the application is complete, you will need to print a copy for EACH scholarship to which you are applying. Be sure you print the name of the scholarship at the top of the application. Some scholarships may require additional materials such as letters of recommendation, a photograph, etc. Make sure you read each scholarship s criteria carefully. 4. MHS FRENCH SCHOLARSHIP DEADLINE: MARCH 16, 2016 AWARD: Two of $200 1. Graduating senior who is currently a French student and has been an active member of either the French Club or the French Honor Society. 2. Must include short essay on How do you plan to use French in the future? 5. VOCAL MUSIC SCHOLARSHIP DEADLINE: MARCH 16, 2016 AWARD: Ten of $200 1. Graduating senior who has been involved with choirs in high school and other outside musical activities. 2. Attachment required. Pick up form in counseling office. 6. MHS FORENSICS TEAM SCHOLARSHIPS DEADLINE: MARCH 16, 2016 AWARD: Three of $100 1. Graduating senior currently active in Forensics. 8. KEVIN EKEDAHL CANCER SURVIVOR/CAREGIVER SCHOLARSHIP DEADLINE: MARCH 15, 1016 AWARD: Two of $500 1. Graduating senior who is a survivor of cancer or a caregiver for an immediate family member (mother, father, or sibling) affected by cancer. 2. Must be planning to pursue further education/training at a college or technical college or serve in the military armed forces beginning in the fall of 2015. 3. Essay, physician verification form, transcript, and two letters of recommendation are required. 9. HAL AND MARTHA DEAL MEMORIAL SCHOLARSHIP DEADLINE: MARCH 15, 2016 AWARD: Two of $500 1. Senior furthering education at a college or vocational school as a full time student. 2. Selection criteria will include goals, purpose, engagement in school, extracurricular activities, and community service. 3. Short essay What would it mean to me to receive this scholarship?

MILTON OPTIMIST CLUB SCHOLARSHIP DEADLINE: MARCH 16, 2016 AWARD: Six of $300 1. Senior furthering education at a college or vocational school as a full time student. 2. Must have a 3.0 GPA or better. 3. Selection based on extracurricular involvement, including community service and school activities, and goals and purposes. 4. Transcript required. 5. Attachment required. Pick up form in counseling office. 15. PAUL EDWARD CAMPION MEMORIAL SCHOLARSHIP DEADLINE: MARCH 15, 2016 AWARD: Four of $2,000 1. Graduating senior who is planning to enroll at a college and is in need of financial assistance. 2. Candidate should be in the top half of their graduating class. 3. Should have been active in extracurricular activities such as athletics and community service. 4. Candidate will be selected based on the application, financial need, and their future goals and objectives. 5. Must include financial income information. Pick up form in counseling office. 16. AMERICAN ASSOCIATION OF UNIVERSITY WOMEN SCHOLARSHIP AWARD: One or Two of $500 DEADLINE: MARCH 18, 2016 1. Graduating senior who is planning to attend either a college or a technical school. 2. Photograph required. 3. Attachment required. Pick up form in counseling office. 17. MILTON YOUTH BASKETBALL SCHOLARSHIP DEADLINE: MARCH 16, 2016 AWARD: Two (1 Boy and 1 Girl) of $500 1. Must have participated in high school basketball all four years. 2. Monies will be paid after successful completion of first semester and proof of enrollment for second semester.

18. ROBERT WENHAM MEMORIAL AGRICULTURAL SCHOLARSHIP DEADLINE: MARCH 15, 2016 AWARD: One of $500 1. Graduating senior in good standing at Milton High School who has participated in at least three agricultural classes. 2. Transcript and photograph required. 19. ROBERT WENHAM MEMORIAL VOCAL MUSIC SCHOLARSHIP DEADLINE: MARCH 15, 2016 AWARD: One of $500 1. Graduating senior in good standing at Milton High School who has participated in at least three vocal music classes. 2. Transcript and photograph required. 21. The criteria focuses first on need, second on community and church activities, and finally on your GPA. 3.
