1 Support Clusters Definitions and Pricing The following price list helps DisabilityCare Australia to estimate and approve the cost of individualised plans. The price list includes prices for the most commonly used supports. Terms UoM : Unit of Measure. The Unit of Measure varies with the type of support being provided - for e.g. per year/per hour Bis: Based on Individual Situation Max$: Some frequently used supports have a set maximum price. For these supports DisabilityCare Australia will not pay higher than this rate for the support. $: prices are based on market research and the price listed is an estimate for planning purposes. The price paid will need to be consistent with the benchmark price. Standard supports, with lower benchmark prices, do not need a quote. Prices are GST exclusive For highly specialised or higher cost supports a quotation will be required - this may be supplied as a general quotation for a suite of specialists services such as early childhood intervention or specifically for a customised piece of equipment. Travel Travel within 10kms is included in the price of the support. Where no support is available locally travel for longer distances will be included in the participant s plan. Equipment DisabilityCare Australia will also provide rental equipment and repairs, maintenance and delivery of equipment. These are not included in the prices below and are quoted separately. Remote and very remote Loadings will apply to the provision of supports in remote and very remote areas in accordance with the rates set by the Independent Hospital Pricing Authority.

4 Personal supports and training Accommodation/ tenancy assistance Assistance provided to guide, support and prompt participants, or to undertake on a participant's behalf, activities to ensure they obtain or retain appropriate accommodation.
