1 International Civil Aviation Organization HLSC/15-IP/39 27/1/15 INFORMATION PAPER SECOND HIGH-LEVEL SAFETY CONFERENCE 2015 (HLSC 2015) PLANNING FOR GLOBAL AVIATION SAFETY IMPROVEMENT Montréal, 2 to 5 February 2015 Theme 2: Future approach to manage aviation safety Topic 2.1: State safety programme 2.2: Safety information protection AVIATION SAFETY DATABASE SYSTEM SUPPORTS IN THE STATE SAFETY PROGRAMME IMPLEMENTATION (Presented by Indonesia) SUMMARY This information paper explains the role of the Aviation Safety Database System developed by the Indonesian Directorate General of Civil Aviation (DGCA) in order to support the State safety programme implementation. The objective of the Aviation Safety Database is to support and facilitate harmonization of the various safety policies of various civil aviation stakeholders such as aerodromes, air operators, NTSC, Civil Aviation Medical Centre and other parties. 1. BACKGROUND 1.1 As part of its regulatory system concerning the Indonesian National Aviation Safety Programme, the Ministry of Transportation of the Republic of Indonesia had developed a National Aviation Safety Database System infrastructure to support the State safety programme implementation. The database system is based on information technology to harmonize aviation safety data from various stakeholder sources as one of the material to improve the aviation safe policies. 1.2 The National Aviation Safety Database System Programme was developed in phases, starting in 2010 up to 2014 with the central data located at the Directorate General of Civil Aviation, Ministry of Transportation. This system is used as a database on aviation occurrences covering both mandatory and voluntary reports. 1.3 The National Aviation Safety Database System Programme is developed as an online framework, with continuous 24 hours, 7 days a week operation to provide all aviation stakeholders with use of the system wherever and whenever required. The reporting system for each individual occurrence category / type is directly connected to the Ministry of Transportation for any required follow up actions. 1.4 The use of the aviation safety database system is regulated in a step by step manner with the scope of authorities of each level. It is regulated by referring to Accident/Incident Data Reporting (11 pages) HLSC.2015.IP.039.en.doc

2 HLSC/15-IP/ (ADREP) and European Co-ordination Centre for Aviation Incident Reporting System (ECCAIRS) taxonomy standards. 1.5 The security of the data in the system gets full attention from the Government as the civil aviation regulator by developing a secured Data System Center, with an online Backup Data Center which is located in a separate location from the Main Data Center System. 1.6 The reliability of the National Aviation Safety Database System is also getting full attention, where continuous connectivity is assured and protection against disruption, such as power failure is provided through a reserve data centre that overrides the main data centre functionality, if necessary. 1.7 To assure the use of National Aviation Safety Database System to be operated effectively, the Indonesian Government periodically conducts a National Aviation Safety Programme promotion programme in relation to all aviation stakeholders. The promotion programme is conducted in the form / method of information and familiarization through various media channels to access all aviation stakeholders. 2. NATIONAL AVIATION SAFETY DATABASE OPERATIONAL FEATURES 2.1 The National Aviation Safety Database System is developed with reference to existing applicable civil aviation regulations and ADREP and ECCAIRS taxonomy category. The system is available continuously for 24 hours a day, 7 days a week to be accessed by the aviation stakeholders at any time and from anywhere. The aviation stakeholders can access it through the webpage The stakeholders who are connected into the database system must have access authority with user login and password. Currently, the system is connecting active users from aerodromes, airlines, ATSs, flying schools, AMOs, NTSC and other Technical Directorates. 2.3 Each individual stakeholder will be connected to the type of services in accordance with the aviation occurrences in the system to be able to file a report of any occurrence to the Ministry of Transportation. The reporting system on the stakeholders are grouped in the Mandatory Occurrence Report (MOR). 2.4 The MOR currently consists of air traffic services (ATS) occurrence; bird-strike occurrence; dangerous goods occurrence; accident/serious incident; service difficulty report; and wild-life occurrence. The reports in the MOR are provided with an Alert Management System which is connected to Authorities to take any necessary follow up corrective actions on each received report. On the MOR group it is provided with the additional facility of a management chart which contains executive summary graphs to know the trend from time to time. 2.5 The system facilitates special modules to be used by aviation stakeholders to harmonize the aviation safety regulations and policies. The special module is the Airworthiness Directive module which is to be used to manage and distribute the Airworthiness Directive Documents to the aviation stakeholders. 2.6 Each aviation occurrence data that requires special attention will be managed through a special facility in the Safety Data Driven Module. This module will store incoming reports to be specially handled.

3 - 3 - HLSC/15-IP/ The system also has the ability to specially manage the type of hazard which is related to each incoming report. This facility is grouped in the risk management module, that can be used to conduct hazard identification, risk assessment and risk mitigation. 2.8 Besides the mandatory reporting, the system facilitates a safety assurance function through Surveillance and DGCA Internal Audit Modules. The scheduled surveillance process can be accessed online and is used by aviation safety inspectors to file reports of surveillance results. This system can classify each audit finding status whether it is closed or open. 2.9 Each airline/aircraft operator has a facility to report the flight hours and flight cycles for each individual aircraft they operate. This module allows the Ministry of Transportation to know the flying hours and cycles of each individual aircraft operated by the aircraft operator Each individual incoming aviation accident and serious incident report to the database system, after investigation, will receive comment or feedback from NTSC. To implement this function, the system has a special module which is named Safety Recommendation. This module will pass the safety recommendations from NTSC to the related or responsible stakeholders One of the crucial requirements in the management of the system is to keep and maintain the data in the master data module updated. The change of data in the master data module is conducted by automatic and manual systems. In the automatic system, the system has an agent facility that can take up updated data from other information systems outside the National Aviation Safety Database System, such as aviation personnel data; medical assessment data; aircraft registration and others In order to harmonize existing regulations, the system has a facility named ICAO State letter. This module is used to update the regulation system and store various changes to regulations through a automatic versioning system. This system is separately accessed from the main module on the webpage The system currently is provided with an additional facility to accommodate incident report of return to apron, return to base and diversion. This system is expected to improve the effectiveness and efficiency in response to a particular aviation occurrence The system also provides a facility for the general public to report any aviation occurrences voluntarily. This system is grouped into voluntary confidential reporting modules. A person (information source) can file a report through the webpage Beside the online facility, the information source can also send the report by using direct message through a short message service (SMS) (texting). Each individual incoming report will be responded by the system by giving a randomly unique number, where the information source can be traced back actually through the reporting process that has been filed. 3. EXECUTIVE REPORTING FEATURES OF NATIONAL AVIATION SAFETY DATABASE SYSTEM 3.1 The system is not only providing an operational service facility to the aviation safety stakeholders, but also providing a system to conduct analyses and executive reporting to the aviation safety decision makers. 3.2 The analyses of the received reports is done through an advanced analytic facility that can be used to do comparisons between existing data. The comparison can be done freely either in units of time, units of data or units of data sources.

4 HLSC/15-IP/ The results of the analyses can be used by the decision makers to set safety targets that have to be achieved to support the State safety programme. 3.4 Besides the advance analytic facility, the system also provides a comprehensive executive reporting system facility through the business intelligent application. The business intelligent system is developed in the database with large scale based on operational database which is named Datawarehouse. 3.5 The business intelligent system displays the whole reports based on the category/type of occurrence with drilldown level to find the root cause of an issue based on available data. 3.6 The business intelligent system is restricted/limited to access at analyses and key person levels that are used as one of the materials to decide on aviation safety policy. 3.7 The business intelligent system is developed with a user friendly concept to be able to access data anywhere and anytime. This system is available online through access to webpage The executive leaders/key persons use the business intelligent system on ipad tablet which allows the decision makers to give response effectively and efficiently against any existing aviation occurrences. 4. CHALLENGES 4.1 The development of the National Aviation Safety Database System in a step by step way allows the Ministry of Transportation to have a comprehensive aviation safety database with working harmonization principles among aviation stakeholders. 4.2 The National Aviation Safety Database System requires legal provisions, in the form of laws and regulations of non-punitive policy except if there is any evidence of criminal or terrorism acts, and also regarding the protection of safety information against misuse/improper use of safety information other than for the purpose of improvement of safety. 4.3 The main challenge to face next is to utilize the database system by advancing the good will and role in filing safety reports of all aviation stakeholders, needed to continuously familiarizing and encouraging them, and when necessary rewarding to those who actively file safety reports. 4.4 Continuous evaluation against the National Aviation safety Database System will be required in order to assure the continuous implementation of State safety programme, while improving and adding the capability of the system. 4.5 Provision and upgrading of human resources to reliably support the system operations is a must, e.g. personnel for database custodian/administrator, safety analysts, auditors, etc., so that the incoming and stored reports/ safety data will be processed, analyzed and followed up accordingly. 4.6 The authorities of the Republic of Indonesia are giving a serious attention to promoting aviation safety, including the use of the National Aviation Safety Database System.

CONCLUSIONS 5.1 The Government of the Republic of Indonesia has developed a National Aviation Safety Database System as part of the commitment to State safety programme implementation. 5.2 The National Aviation Safety Database System is in operational condition and is part of State safety programme implementation. 5.3 Improvement and upgrading of human resources qualifications, continuous evaluations and safety promotions are intensively conducted to support the utilization of the National Aviation Safety Database effectively and efficiently.
