2 About the Sheep River Health Trust The Sheep River Health Trust is dedicated to supporting the provision of comprehensive health services in the Foothills area. The Health Trust aims to promote health and wellbeing throughout our local communities by working in partnership with the residents of the area. SRHT, a registered charitable organization, raises funds to enhance health and wellness programs for the residents of Longview, Black Diamond, Turner Valley, Millarville, DeWinton, Okotoks, and the surrounding rural area. 3 The Trust in Action Programs Spiritual Care Program In 2011 The Spiritual Care team based at the Oilfields General Hospital carried out 3,653 visits to patients in need School Meal Program 1,500 Meals provided to children in need through our school meal program 3

4 Palliative Care Program We have one of the best Rural Palliative Care Teams in Canada in our area Donations have helped to support additional training need for the team to continue their care at a time in life when it so needed In 2011, the team cared for 58 patients in our community Books for Babies Books for Babies is a great program. It gets parents and children interacting more and learning to value books and reading. It gives bonding time which is so important for healthy development. Books for Babies Participant Home Care Program 600 patients received help through Homecare in 2011, a program we help to fund with your donations 4

5 Mental Health We have supported and helped us fund these Mental Health programs; Thrills & Skills Group Effective Co Parenting Heartmath Project Mental health often thought about, rarely spoken about Foothills Children s Wellness Network Partners include 17 Community Agencies, 5 AHS Departments, 2 School Divisions, Local Paediatricians & Physicians Started from research by the Sheep River Health Trust, the Network has launched with the Trust s support 5

6 Seniors Rising Sun Long Term Care Unit The Fun Bus II Purchased last year by the Sheep River Health Trust for the residents of the Rising Sun Long-Term Care Unit Recreation Therapy at the Rising Sun Long Term Care Unit Baking is one of the activities the residents now get to enjoy in their newly renovated kitchen, supported by your donations 6

7 Equipment Purchases With donations we purchased a holter monitor for patients to use in their own homes, saving them waiting lists for this equipment in Calgary Donations helped us purchase a CR Reader for the Okotoks Health and Wellness Centre 7

9 Home Comforts and Functional Help Donations helped us to refurbish the family room at the Oilfields General Hospital and purchase a lift for patients in the community Pediatric Cart for Urgent Care Donations have helped us purchase a specialised cart equipped specifically for children, ensuring fast care when needed for a large group in our community 9

10 Testimonies about our work Our new kitchen came first in the spring of And what a treat it is!!!! Here at the Rising Sun we try to create a very homelike atmosphere. When our residents walk into our facility we want them to feel as if they are not walking into a nursing home but rather a new home with lots of new friends. We are fortunate to have a kitchen right on the unit, but our old kitchen was quite old and outdated and did not look a kitchen you may find in your own home. Our concerns were brought forth to the Sheep River Health Trust and we were not let down. We were given a complete new kitchen with new cupboards, new appliances including 2 fridges, a microwave, an ice maker, a dishwasher, a double oven and of course our gorgeous granite counter tops. Our kitchen is used weekly for baking and cooking programs where the residents are encouraged to help out and create homemade goodies for all to enjoy. Nothing makes a home smell like a home better then fresh baking and cooking!!! Thank you SRHT for keeping our home a home!! Rising Sun Long Term Care New Kitchen Richelle Flitton ~ Books for Babies is a great program. It gets parents and children interacting more and learning to value books and reading. Books for Babies Participant ~ The breakfast that we are now able to offer our students helps them start their day in a positive way by providing essential nutrition as well as a feeling of togetherness and family because we all sit down together to eat. Colleen Vincent, School Meal Program Coordinator, Cameron Crossing ~ I was able to utilize donation dollars to attend the Shades of Grey conference in June I choose this conference to attend as I was particularly interested in furthering my understanding of End Of Life care with dementia patients. One of the more challenging areas of clinical practice within senior care is recognizing advanced dementia verses symptoms of delirium. This is where I can provide support to the front line staff. I felt that attending the conference was very beneficial for my practice and came away with an increased general knowledge of senior care. Thank you once again for your donation generosity as it has been greatly appreciated by myself and my rural colleagues who I was able to share my new insights and learning with. Catherine Jill Uniacke Rural Palliative Clinical Nurse Specialist ~ Our new bus arrived in late August and what an exciting day that was. Our new bus has state of the art strapping system allowing for ease of strapping and un strapping of the wheelchairs. It has the capacity to transport 5 wheel chairs, 8 regular seats, plus the driver. As well it has 4 flips seats allowing us to interchange our regular seats and wheelchairs depending on how many wheelchairs and regular seat are needed for a particular outing. We love our new bus and every time we pull out of the parking spot with a bus full of happy seniors. I thank the SRHT and the generous donators who make our outing possible!! Over the past year our team of volunteers donated over 3000 hours to the Trust. Over the past year our donors have given $ to the Trust to help in their community.
