1 Heartwood House: Au coeur de la vie 404 McArthur Avenue, Ottawa, Ontario - K1K 1G8 PERMANENT SPACE APPLICATION FORM BACKGROUND INFORMATION Our history: Heartwood House was incorporated on March 12, Its birth is the vision of a group of highly dedicated people who believed in the benefits of non-profit organizations working together under one roof. Heartwood opened its doors at 153 Chapel Street on September 1, 2001 and its development has grown to a home of its own now located at 404 McArthur Avenue in the Vanier/Overbrook community. Heartwood House owns a 26,000 square foot facility just east of downtown Ottawa. Heartwood Board of Directors: 2015 Isobel Bisby (co-chair), Pat Day (treasurer), Marg Fallis, Anthony King, Kim Oastler, Hoppy Roy (secretary), Barbara Merriam (co-chair), Tara Templin and Tanya Wayne. Heartwood Executive Director - Maureen Moloughney Mission: Heartwood House is a non-profit charitable organization dedicated to supporting small non-profit and charitable organizations. We believe that we are far better together under one roof, achieving our respective missions in a community environment that provides the opportunity for member organizations to learn from each other, while serving clients more creatively, efficiently, and cost-effectively than operating in isolation. Vision One roof, many voices; a community model for social change. Values Community: We are welcoming, inclusive and respectful, and share a common set of values with our member organizations in our quest to serve the community and their clients. Collaboration: We are far better together than we could be working in isolation. Through cooperation and collaboration, member organizations are able to better serve the community and their clients. Accountability: We are accountable to our member organizations, our community partners, the community we serve and those that fund our organization. Responsibility: We take our responsibility to our member organizations seriously and know our members in turn understand their responsibilities to Heartwood House. Sustainability: We work to ensure Heartwood House is sustainable for the future. We consider the long-term effects of our planning decisions. 1

2 Community Collaboration: Our Member Groups commit to: a) Create a welcoming and hospitable environment; b) Define an evolving process through which we will share and grow together; c) Determine a process to achieve financial independence including fundraising. Rental Information and other details about Heartwood House: square footage rate is $17.24 (to increase to $18.10 April 1, 2016.) 2. Membership fees - spaces under 300 square feet - $50 per month - spaces over 300 square feet - $100 per month 3. Internet - $12 per month - internet service through Rogers. 4. Phone system is with Rogers. 5. Meeting rooms available to accommodate up to 200 people - community room (1-20) - $5 per hour - Heartwood room - (1-15) - $5 per hour - conference room (1-35) - $7 per hour - sanctuary (35 to 200) - $12 or $15 per hour 6. Parking - full-time - $50 per month - part-time parking also available. On-street three hours parking also available. 7. Community print shop - 9 cents black and white - 30 cents colour - Fax machine - available in reception office. 8. Reception office operating hours: September 1 to June 30 Monday - Tuesday: 8:00am to 7:00pm Wednesday - Thursday: 8:00am to 7:30pm Friday: 8:00am to 5:00pm Summer Hours - July 1 to August 31 Monday - Tuesday - Thursday: 8:00am to 5:30pm Wednesday: 8:00am to 7:30pm Friday: 8:00am to 5:00pm 9. Mail and courier packages received by reception staff each day. Member group mailboxes are in the reception office. 10. Daily garbage pickup - garbage left outside offices at 3p.m. each day for pickup. Heartwood recycling program in place. 11. House common spaces - washrooms, hallways and meeting rooms are cleaned each day from Monday to Friday. 12. Community kitchen and kitchenette available for members. 13. On-site Confectionary with snack bar, Indian restaurant as well as in-house community Souper (homemade soup) Wednesdays. 14. Monthly member meetings and HHcommunity system for on-going communication. 15. Security camera and fire alarm system. 16. Elevator at front entrance - fully accessible building 17. OC Transpo bus access - number 14 stops at our building. This bus travels to the Rideau and St. Laurent Centres. 2
