Our school s code is Submit this scholarship application to the Cossatot Community College Financial Aid Office. Fulfill the criteria of the particular scholarship(s) for which you are applying. Please note that some scholarships have additional requirements. Attach a copy of your most recent transcript, ACT score (if available), GPA in high school, or GED scores. Unless otherwise noted, a minimum GPA of 3.0 is required, both to apply and to maintain your scholarship. Provide two letters of recommendation: one from an instructor or teacher and one from a non-relative. Provide an essay entitled, Why I Would Like to Receive this, explaining college and career goals. Priority deadline for scholarships is April 1. Must be a U.S. citizen. Student must have intent of obtaining a four-year degree in a technical field such as physics, chemistry, math, engineering, accounting or education. If no student meets this requirement, other majors will be approved at the discretion of the scholarship committee. Provide proof of U.S. citizenship, transcript for past three academic years, along with ACT score. Declare intended major. Student will receive $1000 each semester for tuition, books and/or fees.
