3 Introduction to the MiPACS Storage Server The Server Using LEADTOOLS Medical developer toolkits, Medicor Imaging has created the MiPACS Storage Server, a scalable and affordable image repository and management system for most DICOM-compliant modalities and client applications. The MiPACS Storage Server is an ideal starting point for any facility migrating towards a film-less environment. The MiPACS Storage Server is the backbone of a DICOM, (Digital Imaging and COmmunications in Medicine), network. MiPACS Storage Server Manager The DICOM 3.0 Standard: DICOM is an open standard that allows medical equipment and software from different manufacturers to communicate with one another and share medical data, improving patient care. DICOM is the most common standard for receiving medical images from a hospital, and is quickly becoming the standard for all types of medical communication. Devices, from MRIs and CT Scans to Printers and Software, are DICOM-compliant only if they strictly follow DICOM conformance. Conformance envelopes both a file format as well as a protocol for communication that DICOM-compliant devices must use. The advantage of keeping a clinic or hospital DICOM-compliant is that DICOM is an open, non-proprietary standard. A result of this is that any DICOM-compliant device can communicate with any DICOM server or printer, regardless of brand, type of device, or operating system. 1
4 Overview of Server Features Technical, Administrative, and Security Features Network Load Balanced/Failover Capability The MiPACS Storage Server can be configured in a Network Load Balancing Cluster to provide even distribution of traffic to more than one MiPACS Storage Server. Such a configuration, achieves failover capability as well. Upon failure of the Server service or physical failure of a server, other servers in the cluster pick up the slack. Runs as a service The MiPACS Storage Server runs as a service automatically in the background and requires no user interaction to start. If a server is restarted, there is no need to login. Logging of all DICOM, Server, and Error messages An extensive log can be kept of every event taking place on the MiPACS Storage Server. Every DICOM transaction, with clients identified, can be recorded. Database runs on SQL Server The MiPACS Storage Server database runs on a trusted and proven database server the Microsoft SQL Server. Secure Communications Supported The MiPACS Storage Server can be configured to communicate with other devices over a secure port, with the option to verify server and client certificates. DICOM Features Full Compliance with the DICOM 3.0 Standard The MiPACS Storage Server is fully compliant with the DICOM 3.0 medical standard, and a conformance statement is provided with the Server. IP and Subnet Mask or Hostname Verification Client AE (Application Entity) titles can be bound to an IP address and subnet mask or to a hostname. With verification, if another computer on your network tries to connect as the AE title of an authorized computer, it will not be permitted unless the exact IP address or hostname are verified on the server. Access Level Security per Client The MiPACS Storage Server controls which AE Titles can query, retrieve, and/or store to the server. Capable of Anonymous Connections Anonymous connections can query and store to the server without being specified in the clients table. Receives Images The MiPACS Storage Server stores any received image, in its entirety, in its internal data store. Also, the MiPACS Storage Server extracts the query information and stores the patient, study, series, and image data in its internal database. 2
5 Accepts Queries from Other Devices The MiPACS Storage Server responds to any query request, basing its decisions on the records stored in the database. Accepts Retrieve Requests from Other Devices The MiPACS Storage Server responds to C-MOVE commands in order to process retrieve requests of images for the requester by obtaining a reference from the database. The object is then obtained directly from the data store. Transmits Images The MiPACS Storage Server transmits images to other compatible devices as a Service Class User of C-Store service. TCP/IP Network Communication Support The MiPACS Storage Server supports the TCP/IP network Communication Support stack protocol, adhering to the DICOM 3.0 standard. Multiple Simultaneous Connections The MiPACS Storage Server spawns a new process for each connection request it receives. Asynchronous Nature The MiPACS Storage Server allows only a single outstanding operation on an association. Therefore, the MiPACS Storage Server does not perform asynchronous operations, other than the default specified by the DICOM 3.0 standard. Image Forwarding Series can be forwarded manually or on a recurring schedule to other DICOM Servers. For example, a satellite clinic can forward exams to a central server at a different location on a daily or weekly basis for archiving. Image Cleanup Images can be cleaned up from a server manually or on a recurring schedule based upon the age of the exam, if they have been forwarded to a central DICOM server for archiving. For example, a satellite clinic can conserve storage space by deleting all images over 2 months old if they have already been forwarded to a central server for archiving. Autocopy The auto copy feature allows the storage server to be configured to automatically route moved DICOM image data to any number of PACS servers. This feature can be set up so that when the server receives a C-MOVE request to a destination AE it will automatically store it to some other server as well. When the storage server to connects to the remote PACS servers, it can be configured to optionally use a custom AE title. For performance, the auto copy processing can be performed in a user-specified number of different threads Gateway The gateway feature allows DICOM servers to be created that act as a proxy to communicate with other DICOM servers on the client s behalf. This allows clients to send query/retrieve requests only to the gateway server. Those requests can then be automatically relayed to any number of other PACS servers. The Gateway can forward C-FIND and C-MOVE DICOM messages. Requests are forwarded to the specified remote DICOM servers and the responses forwarded back from the gateway to the requested clients. Images moved from remote servers are stored in the local main storage server database. Multiple remote servers can specified for each gateway. If one remote server fails to respond, then the next remote server in the list is used, continuing until a server responds before sending a failure message to the requested client. 3
6 System Requirements The requirements of the MiPACS Storage Server depend on the number of clients connecting to it and the amount of traffic those clients generate. In addition, the amount of storage required depends on the number of patients seen. Examples of recommended configurations are shown below. The MiPACS Storage Server is designed for 32-bit or 64-bit* operating systems running Microsoft Microsoft Windows 2000 Server, Microsoft Windows Server 2003 or Microsoft Windows Server To utilize Network Load Balancing, Microsoft Windows Server 2003 or 2008 is required. 12 MiPACS Storage Server Operating Procedures The Server Directory Layout and Notes about the Application: Note that the MiPACS Storage Server saves the DICOM images separately from the database. Images are stored to a physical drive and the references to the DICOM images are stored in a SQL database. By default the MiPACS Storage Server creates a database (MiPACS_DB.mdf ) and a transaction log file (MiPACS_DB_LOG.LDF) in the SQL server s default database directory (C:\Program Files\Microsoft SQL Server\MSSQL\Data). You can copy the database and map the image folder in the MiPACS Storage Server to a more suitable location on the network where more disk space is available for storing DICOM images. This also makes it easier for a database administrator to manage and back up the data. For example, you can do the following: Create a Directory for storing the images and database (e.g. S:\Medicor Imaging ). Create two sub-directories, named Images and Database. (e.g. S:\Medicor Imaging\Images, S:\Medicor Imaging\DataBase ). For more information, see the section on Configuring the MiPACS Storage Server using the Control Panel. 10
