1 VENTYX esoms SHIFT OPERATIONS MANAGEMENT SYSTEM Introduction Managing complex plant operations demands a comprehensive, integrated software system that not only optimizes performance, but can be implemented quickly and adapted to plant-specific procedures and processes. For over two decades, Ventyx has been at the forefront of the development and successful implementation of operations management information systems for the chemical, electric power, marine, military, petro-chemical, and pulp & paper industries. Drawing on its experience in the operations management automation area, Ventyx developed the Shift Operations Management System (esoms). A mission-critical application, esoms users rely on the software to ensure the safe, efficient, and reliable operation and maintenance of their facility assets. esoms Modular Advantage esoms is a modular software application suite that is designed to automate and integrate the major processes involved in plant operations management. 2 Benefits of Using esoms esoms is designed to provide a consistent, organized, and integrated approach to those activities which affect equipment configuration, system status, and hence plant operation. The esoms Narrative Logs module has become a fixture on the desktop of many plant managers as a tool to track the current operating status of the plant and to disseminate directives such as standing orders. esoms reports are Crystal Reports compatible, allowing you to customize the standard reports or create your own. The esoms AutoPrint module generates Adobe Acrobat files of critical operations documents (procedures, tagouts, rounds, logs) and automatically transmits them to third-party electronic document management systems eliminating much of the document control administrative burden. Program Modules At a Glance esoms program modules encompass the basic software tools that assist in the management of plant operations. A brief description of each module is provided below. Equipment - provides quick access to equipment data of critical importance to plant operators. In addition to nameplate data and related documents (drawings, procedures, regulatory limits, etc.), current as well as historical equipment operating status and configuration data are provided. Users see not only the current configuration of equipment, but are able to trace its path through the many operational activities (tagouts, operating procedures, rounds, etc.) that lead to its current status. Configuration Control - designed to automate and track configuration activities associated with plant procedures (alignment checklists and normal operating instructions) and equipment deviation records (temporary mods, operator aids, equipment out-of-service logs, etc.). esoms incorporates a "living" model for all phases of the configuration control process that provides operators with information related to equipment configuration control and status based on actual plant conditions, as opposed to static procedures

4 The system also provides the flexibility to model different equipment and system configuration requirements based on a plant s operational status. Clearance - designed to assist in implementing, controlling, and executing a plant s tagout/lockout procedure. The system facilitates access and linkage to equipment isolation, work status, and schedule information through interfaces to third-party work control, maintenance management, and scheduling software systems. Operator Rounds a data acquisition and analysis module designed to automate the tasks of collecting, storing, and analyzing equipment operating data from operator rounds. The system provides personnel responsible for plant operation with a powerful yet easy to use tool for collecting and analyzing massive amounts of equipment operating data quickly and efficiently. Operator Rounds puts the power and flexibility of complex calculations, trending, and analysis at the fingertips of operators both at the desktop and in the field where the likelihood of detecting and preventing equipment problems is greatly enhanced, thus maximizing plant reliability. LCO Tracking - designed for those industries where regulatory compliance requirements may directly affect plant operations. The system provides personnel responsible for plant operation and compliance with an easy to use tool to quickly schedule and track required regulatory and/or administrative actions with respect to plant specific limiting conditions for operation. LCO Tracking provides the flexibility to integrate a diverse spectrum of compliance requirements such as plant technical specifications, technical requirements manuals, fire protection guidelines, off-site dose calculation manuals, and administrative procedures. The likelihood of missing or incorrectly scheduling required actions with LCO Tracking is greatly reduced, thereby preventing costly regulatory fines. Narrative Logs - an electronic journal for recording and qualifying events which occur during an individual's shift watch. The system tracks shift staffing, plant operating parameters, log entries, associated qualifying data, and web/document links on an individual plant unit and/or staff position basis. In addition to on-demand log entries, Narrative Logs has the capability to accept automatic log entries from other esoms modules to automatically document the statusing of procedures, tagouts, operator rounds readings, regulatory action compliance, etc. Personnel Qualifications & Scheduling - a scheduling tool designed to ensure the adequacy and compliance of plant staffing. The system tracks personnel for man-hours, shift positions, qualifications, and shift details over the entire range

5 of plant operation and shutdown. A natural complement to Narrative Logs, PQ&S has been designed to integrate shift staffing requirements, position qualifications, regulatory work constraints, crew makeup, man-hours on a per person or per shift basis, shift rotation changes (into and out of outages, changes in shift duration, etc.), overtime callouts, vacation scheduling, and a host of other information critical to maintaining a qualified plant staff. Notice of Change a change management and messaging tool designed to manage the complete life-cycle of an organization s Notice of Change (NoC) process. NoC entities can be external to esoms (Required Readings, SOP s...), or created automatically by modules within the esoms Suite (Clearance signon/off actions, Operator Rounds alerts). Administrative functions allow the preparation, distribution, maintenance, and acknowledgement status reporting of NoC records. An internal messaging service handles the delivery, access, and acknowledgement of NoC records by end-users. Mobile esoms - realizing the data entry burden that could be placed on plant operators if the processes were to remain paper-based, Mobile esoms has been designed to work with state-of-the-art mobile devices. Information captured in the field with mobile devices during equipment configuration activities (for example procedures, tagouts, operator rounds, narrative logs, etc.) is synchronized with the plant status database, eliminating the need to collect information on paper, and then enter it back at the desktop. AutoPrint - to eliminate much of the administrative burden associated with document management issues, the esoms AutoPrint module automates the process of creating, transmitting, and tracking critical operations documents from esoms to third party electronic document management systems. System Interfaces to enable integration of esoms with other enterprise data systems, modules are available to manage the exchange of data between esoms and industry-leading project scheduling, maintenance management, asset tracking, and data historian software systems

6 esoms MODULES: EQUIPMENT MODULE A critical aspect of the esoms operations management model is the consistent identification of plant equipment within the database. With a consistent equipment identification system both internal (within esoms) and external (esoms with third-party applications and databases) interfaces are enabled to provide an enhanced level of information to the esoms user. Without it, many of the benefits of data sharing are lost. The esoms Equipment module is used to assist in the definition and implementation of a consistent, organized equipment identification system. The software has the capability to employ multiple types of equipment identification schemes, from rigorously coded IDs to noun descriptions. This affords front-line users of the software the freedom to use the identification system that they are most familiar with, while maintaining back-end compatibility with other systems and databases. 7 In support of equipment identification, the esoms Equipment module provides users with a flexible system of both fixed and user-definable equipment related attributes, which are also important in the performance and tracking of operations tasks. These include: Plant Names Verification Requirements Systems Buildings Columns Plant Units Equipment Types Functional Groups Elevations Notes Plant Operating Modes Safety Trains Components Types Rooms Configurations/Positions Integration and synchronization of the esoms equipment database with a plant s master equipment list (MEL) as maintained by third-party asset tracking and maintenance management databases has also been shown to be highly beneficial. Such integration can eliminate data inconsistencies and reduce data maintenance, while providing an enhanced level of interoperability between systems. Mobile esoms for Equipment provides operators in the plant the capability to both access data normally available only at the desktop or collect data and update the enterprise database as part of a plant walk-down. CONFIGURATION CONTROL MODULE Maintaining configuration control of plant equipment and systems is often the most difficult aspect of plant operations management. Without a system to assist in placing the plant in a desired configuration, and a system for tracking deviations from that configuration, configuration control of a plant can be lost leading to possible plant shutdowns or delayed restarts. To ensure that configuration control of a plant is maintained, the esoms Configuration Control module automates and tracks configuration activities associated with plant procedures (alignment checklists and normal operating instructions) and equipment deviation records (temporary modifications, equipment out-of-service logs, etc.)

8 Through its integration with other esoms modules, Configuration Control incorporates a "living" model for all phases of the configuration control process. This model presents users with information related to equipment configuration control and status based on actual plant conditions as opposed to static procedures. For example, an equipment lineup procedure is generated with equipment tagged or locked on a clearance indicated as such on the lineup. The system also allows the plant to take credit for equipment configuration activities that have occurred in the past, eliminating repetitive work activities and helping to reduce personnel exposure and wasted man-hours. Features: The Configuration Control module features two configuration control activity sub-modules: Procedure Management - Procedures are plant operating instructions, generally associated with a particular plant system or component, which are performed prior to full power operation, or as a part of normal plant operation or shutdown. Procedures are created, maintained, generated, and performed within the Procedure Managers of the Configuration Control module. Procedures are automatically updated with the current status of plant equipment to alert operators to possible conflicts with other procedures, deviation records, or tagouts already in progress. Time-saving features such as take credit allow procedures to be completed using equipment configuration verifications recently performed on other procedures, deviation records, or tagouts. Configuration Control procedures also can be used to automatically set the normal configuration of equipment when releasing deviation records or tagouts. Deviation Record (DR) Management DRs are electronic forms used to organize and track a range of possible items that affect the configuration status of plant equipment. DR s may be used to track temporary equipment modifications, equipment out-of-service, abnormal equipment configuration, etc. DR s can be used to restrict plant mode changes to prevent entering operating conditions that require deviated equipment to be in service. 9 CLEARANCE MODULE The tagout/lockout process is perhaps the most important and time-consuming function that a plant s operation and maintenance staff performs because it is critical to both personnel and plant safety. Errors in the tagout/lockout process can result in work delays, safety stand-downs, regulatory fines, and personnel injury or death. By automating this process, significant gains can be realized in both efficiency and safety. The esoms Clearance module is designed to assist in implementing, controlling, and executing a plant s tagout/lockout procedure. The system enforces the logic of plantspecific procedures to ensure personnel compliance. Clearances facilitates access and linkage to equipment isolation, work status, and schedule information through interfaces to third-party work control, maintenance management, and scheduling software systems. OPERATOR ROUNDS MODULE Even in the most modern plants, routine personnel inspections of equipment and facilities is critical to detecting and preventing costly maintenance problems that might go undetected by automated monitoring systems alone. To ensure that you are getting the most out of plant inspections, the esoms Operator Rounds module is designed to automate the task of collecting, computing, and storing data from operator rounds. The system also provides personnel responsible for plant operation and performance with a powerful yet easy to use tool for analyzing the data quickly and efficiently. 12 Features: Reference Document Management Tools are provided to identify and categorize those plant design documents which require actions to be performed as a result of abnormal environmental, plant, system, or equipment conditions. Once categorized, reference documents can be broken down into discreet sections, then sections broken down into action statements or conditions, which form the basis for limiting conditions of operation (LCOs). LCOs can be related to plant equipment, linked to source documents, and individual actions with associated timings. Tools are also provided to automate the processing and linkage of Word -based reference documents to the esoms data structures. Setup and Configuration Management Each reference document type may have unique LCO Tracking requirements. LCO Tracking incorporates the tools to manage the setup and configuration of: Tracking Records type, numbering, operational characteristics, verification requirements, attributes General terminology, system preferences Tracking Record Management LCO Tracking contains sophisticated tools to manage the day-to-day preparation, storage, retrieval, execution and revision of tracking records, including: Mutli-class database master, active, and archive class tracking records Data tracking details, action statement entry/exit, action scheduling and performance, attributes, verifications, equipment, related documents (tagouts, work orders, condition reports, PMTs) Modular integration built-in interfaces to Equipment, Configuration Control, Clearances, Operator Rounds, LCO Tracking, Narrative Logs, Personnel Quals & Scheduling NARRATIVE LOGS MODULE As many shift workers can attest, maintaining manual log books can be a tedious and timeconsuming task. Providing a tool to assist in taking logs, and to automatically capture and document events in a log can significantly reduce the time required to maintain and increase the accuracy and usefulness of these important plant records. The esoms Narrative Logs module provides plant operators, supervisors, and managers with an electronic journal for recording, capturing, and qualifying events which occur during an individual's shift watch. The system tracks shift staffing, plant operating parameters, log entries, associated qualifying data, and web/document links on an individual plant unit and/or staff position basis. In addition to on-demand or template log entries, Narrative Logs has the capability to automatically capture events from other esoms modules as log entries. These automatic log entries are used to document the performance of procedures, tagouts, operator rounds, regulatory action compliance, etc

13 Features: Setup and Configuration Management Flexible and adaptable setup tools allow the system to be adapted to a variety of journaling needs: Log setup - any number or type of log (operations logs, maintenance logs, test logs, etc.) can be created and tracked within the system. Watchbill definitions each log can have associated with it a watchbill template defining a shift staffing complement which can be linked to a watchbill in the esoms Personnel Qualifications & Scheduling (PQ&S) module for enhanced scheduling and compliance evaluation capabilities. Parameter definitions each log can also have associated with it key operating parameters which are to be tracked as part of the log. Templates log entry and shift turnover report templates can be defined to speed the on-shift log entry and turnover processes. Log Management Tools to manage routine logging tasks include: Watchbill staffing manual personnel staffing assignments or automatic personnel staffing through links to the PQ&S module Parameter tracking entry and historical tracking of key operating parameters Log entry on-demand, template, or automatic log entries captured from other esoms modules all log entries include author identification, log text, and a range of related data Turnover tracking - functions to track and manage the turnover of both personnel and logs Mobile esoms for Narrative Logs automates the performance and tracking of logs in the field. PERSONNEL QUALIFICATIONS & SCHEDULING MODULE What once was a rather simple task determining a workers qualifications to hold a particular position and scheduling personnel in a shift rotation has, through the introduction of increasingly complex regulatory and labor work restraints become a timeconsuming and often times frustrating process. Many organizations use a collection of loosely-coupled spreadsheets, databases, and manual forms to manage the personnel qualification and scheduling process. This approach leaves wide-open the possibility for mistakes in the scheduling of personnel who are not qualified for the positions to which they are assigned, or overworking personnel beyond the regulatory and/or labor agreement work limits. These oversights can result in costly regulatory fines or labor grievances being filed

14 To better manage this process, the esoms Personnel Qualifications & Scheduling (PQ&S) module has the tools to: Develop and maintain user requirements and associated position qualifications Track user requirement status via direct input or links to external sources (e.g. training databases) Generate, staff, and validate shift schedules from pre-defined watchbill rotations, incorporating time-off Evaluate schedules for compliance with position qualification requirements and work-hour limits Features: Qualifications Management Tools are provided to create and maintain user requirements and position qualifications. User requirements encompass a range of entities that affect a users qualification to hold a particular watchbill position. Examples of user requirements include certifications, licenses, medical examinations, training classes, watch-hours stood, etc. User requirements can be combined into complex logical expressions using Boolean operators (e.g. AND, OR, NOT), and internal functions (e.g. to calculate hours worked, rounds taken, etc.) to form a position qualification. Watchbill Management - Watchbills are the required shift complements created for departments and their schedule routines (e.g. 8-hour weekday, 12-hour weekend, etc.). Watchbill rotations (6 crew weekly rotation, 4 crew rotation, short rotations, etc.) are created and extended into the future based upon planned schedule requirements and adjusted to accommodate interruptions (e.g. planned outages). Schedule Management The Scheduler performs evaluations of personnel compliance with position qualifications and work-hour limits over a chosen timeframe. Qualification non-compliance, work limit violations, time-off (vacation, leave, etc.), and other scheduling conflicts are identified before-the-fact to allow adjustments to be made to schedules in advance if necessary. The Scheduler allows for fine-tuning (replacement of on-shift personnel, addition of temporary shift positions, etc.) and adjustments to standard shift staffing requirements. This entry-by-exception method significantly reduces the amount of data input required to maintain the system. Modular Integration PQ&S integrates with Configuration Control, Clearances, Operator Rounds, and Narrative Logs to ensure personnel compliance at the point of performance of operations related tasks

15 NOTICE OF CHANGE MODULE The latest in the esoms program suite, the Notice of Change (NoC) module provides personnel with a software tool to manage the complete life-cycle of the NoC process. Implementing an automated NoC process helps to ensure that personnel are notified of changes to procedures and practices in a timely manner and facilitates access to NoC related information. Reporting tools allow management personnel to quickly determine the status of end-user NoC acknowledgement. Part of the NoC process refers to the preparation, distribution, delivery, access, acknowledgement and maintenance of changes to a variety of entities external to esoms such as: Required Readings Management of Change Operational Guidelines Notice of Routine Duty Change Standard Operating Procedures Also included in the NoC process is the generation, delivery and tracking of automatic notifications from modules within the esoms suite such as: Clearance holder sign-on/off Operator Rounds reading alerts Features: NoC Administration this portion of the process covers the preparation, distribution, maintenance and reporting of NoC. Each NoC requirement is defined by a NoC record. A NoC record may contain details, attachments, and notification assignments. Details include general descriptive information about the NoC. Attachments may include embedded or linked documents (e.g..doc,.pdf files or hyperlinks). Notification assignments can be made by esoms security group, department, organization or individual. Once approved, NoC records are delivered to assigned users. The acknowledgement status of NoC records can be tracked and NoC records eventually closed and archived. NoC Delivery & Acknowledgement the delivery, access, and acknowledgement of NoC records by end-users is managed by the esoms Messenger. The messenger is a component of all esoms modules and is automatically activated each time a user logs into a module - making it an integral part of routine duties. Once delivered, users can access information associated with a NoC record or request additional information. 16 MOBILE esoms MODULE Mobile esoms is the solution for plant operations management data collection and analysis needs in a mobile environment. Mobile esoms provides the software tools which not only eliminate the need for paper-based procedures, tagout, rounds, and log sheets in the field, but bring about a whole new level of power and flexibility to plant operators. Mobile esoms can significantly streamline business processes by: Improving Mobile Worker Productivity Capturing data at the point-of-performance, the time-consuming paperwork and data entry cycle is eliminated. Expanding the range and type of data available to be captured in the field to include text, audio, and graphics (still or motion pictures) Delivering critical information in the field not possible in a paper-based system, the mobile worker can make better informed decisions. Ensuring Compliance Application logic enforces internal procedures and external regulatory requirements while maintaining flexibility for the mobile worker. Leveraging bar coding and other auto-identification technologies to ensure mobile worker accuracy and compliance. Providing More Timely Information to the Enterprise Allowing managers to track field activities more closely and to make better informed decisions. 17 AUTOPRINT MODULE Many industries are required by law to maintain a permanent record of operations related activities. To eliminate much of the administrative burden associated with document management issues, the esoms AutoPrint module automates the process of creating, transmitting, and tracking critical operations documents from esoms to third party electronic document management systems. AutoPrint interfaces with the following esoms modules for the transfer of documents: Configuration Control procedures and deviation records Clearance tagouts, sections and temporary lifts Operator Rounds round sheets LCO Tracking tracking records Narrative Logs log sheets Personnel Qualifications & Scheduling watchbills AutoPrint is triggered by a change in status of a document (e.g. clearance release, log turnover/approval, ). AutoPrint in conjunction with Adobe s Acrobat Distiller software is used to generate the electronic forms for each module s critical documents in PDF file format. An XML-based interface is used to provide detailed document tracking and content information to the electronic document management system software. Fault-tolerant processing and auditing features are incorporated into AutoPrint to ensure that documents are transmitted and processed successfully by third-party systems. System Interfaces The successful management of plant operations involves the coordinated efforts of many groups and disciplines throughout the enterprise. Ensuring that the information systems used by these different entities can share information and resources is critical to that success.
