Period of Performance: June-November Targeted Physician Practices: 509 in 3 Phases 6. Weekly Target/Trainer = 12 Practices/Week 7. Completed Phase I = 11 practices/31 users 1. Results: Accessing the CHR and Referrals Management Network provides value in supporting improved patient care with the ability to now view community health data and have a closed-loop referral process.
