For many eligible professionals, 2014 will be the year that they will be attesting for Meaningful Use Stage Two. In Stage Two, providers will build on their Stage One experience and meet expanded core measure requirements involving clinical decision support processes, clinical information exchanges between providers and health information access by patients. In this session, attendees will be presented with an overview of Stage Two requirements including: Review of Stage Two Core, Menu and Clinical Quality Measures (CQM) Top Challenges of Stage 2 including ecqm submittal, Patient engagement, TOC & Public health reporting. Payment Adjustments & Hardship exemption applications Notice of Proposed Rule Making updates Meaningful Use Challenges patient electronic access, secure messaging and electronic transmission of summary of care records Interoperability is an important goal of meaningful use, particularly in Stage 2. While stage 1 of meaningful use emphasized the digitalization of medical records, stage 2 is emphasizing the sharing and exchange of these records with patients, other providers of care and health data registries. This session focuses on electronic record sharing with patients and providers and how meaningful use is encouraging this to occur. Invited speakers will discuss how they are using patient portals and EHR generated transition of care documentation to satisfy the patient electronic access, summary of care and secure electronic messaging measures of meaningful use. Tips will be offered on how to overcome some of the most common challenges practices face as they implement patient portals and electronically exchange medical records with other providers HIT Summit Page 1 Updated 7/15/2014

In this presentation, attendees will be provided an overview of the general and distinctive security issues that the Hawai'i Health must deal with, its framework of privacy/security policies for managing access to patient information and patient opt-out alternatives. Discussion topics will include what patient information may be shared, patient/physician disclosure consents/opt-out, and who may be authorized users. The Health enet: Making the HHIE Connection Since being named the designated State entity for Hawai i in 2010, the Hawai i Health Information Exchange (Hawai i HIE) has made significant progress in developing trust and support from the Hawai i healthcare community. Building on the successful launch of its secure messaging service in 2012, Hawai i HIE initiated Health enet in 2013 which comprises the secure messaging platform and its formative health information exchange service. In this session, attendees will be presented with information on the roll out plan for deploying Health enet Community Health Record including what major provider groups will be participating, planned EHR system interfaces, public health reporting features and capabilities for accessing patient health information. With the enhanced utilization of EHR systems however, there has also been an accompanying increased risk exposure for information security breaches. This risk exposure is especially acute in small provider practices that typically lack significant experience and technical resources for managing their EHR systems at a level that meets standards and guidelines stipulated in the HIPAA Security Rule, HITECH Act and the Omnibus Privacy & Security Rule. In this session, attendees will be presented with an overview of regulations impacting deployment of their EHR systems including required policies/procedures, security risks assessments, employee training, and security breach notification requirements Pharm2Pharm: Utilizing Health enet to Transform Healthcare for Patients at Risk According to the Hawaii Health Information Corporation, medication-related hospitalizations cost Hawaii over $100,000,000 in 2010 alone. Recognizing the opportunity to improve medication management, the University of Hawai i at Hilo College of Pharmacy Center for Rural Health Science received a CMS Innovation Award to launch a new service called Pharm2Pharm. The Pharm2Pharm team, a hospital-based consulting pharmacist collaborating with Pharm2Pharm s community consulting pharmacists, utilizes health information technology to assist in identifying drug therapy problems and help physicians resolve them for patients most at risk when they move from hospital to home. This session will provide attendees an overview of the Pharm2Pharm program, preliminary results, and the innovative use of Health enet technology to support transformation of the healthcare team. The Hawai'i Medicaid EHR Incentive Program and MU-Stage 1: It s Not Too Late to Get Paid 2013 was an eventful year for many eligible professionals (EPs) who successfully attested for Adopt, Implement, or Upgrade (AIU) in the Hawai'i Medicaid EHR Incentive program and qualified for the $21, year one payment. Coming into 2014, these EPs will be attesting for Meaningful Use for the first time in the Hawai'i Medicaid program. In this session, attendees will be presented with information on the Hawai'i Medicaid EHR Incentive Program and how to register, eligibility requirements, and how to attest for Meaningful Use HIT Summit Page 3 Updated 7/15/2014

You Have A Good Chance of Being Selected Eligible Professionals attesting to receive an incentive payment for either the Medicare or Medicaid EHR Incentive Program may be subject to an audit. CMS program guidance suggests that up to ten (10%) percent of EPs receiving incentive payments may be audited; and 10% may be audited prior to receiving a payment. This session will provide attendees with an overview of the audit process and provide valuable information on relevant supporting documentation that EPs will likely be requested to produce in the audit. Brian Ahier, Director of Standards and Government Affairs, Medicity Matthew Narbut, Demonstrator, OneMedNet Natalie Pagoria MD, Business Analyst, Hawai'i Health Making Meaningful Use Meaningful An often cited value of health information exchanges is the capability to extract concurrent and retrospective patient information from disparate electronic health record (EHR) systems across the healthcare continuum and to aggregate the information for the purpose of managing chronic disease and high healthcare utilization patients. In this session, attendees will be presented with a case study that illustrates the current state of population management tools and techniques, critical issues, the efficacy of these methods. Imaging Results and Medications History: Thinking Outside of the EHR Box The goals for health information exchanges are lofty and the challenges for aligning them to Meaningful Use (MU) goals are extremely complicated. In this session, attendees will be introduced to two Hawai i HIE programs aligned with MU and initiated to improve patient care and improve efficiencies within a clinical practice. The first addresses Medication Reconciliation (MU Measure 14: The process of identifying the most accurate list of all medications that the patient is taking, including name, dosage, frequency, and route, by comparing the medical record to an external list of medications obtained from a patient, hospital, or other provider). The session will showcase how medication history and reconciliation can be done for patients transitioned into the care of the provider. The second addresses Imaging Results (MU Menu 3: More than 10 percent of all tests whose result is one or more images ordered by the EP during the EHR reporting period are accessible through CEHRT). Although an image can currently be viewed by a physician through the existing CEHRT, integration into Health enet will streamline the physician s workflow when searching for images from separate imaging technologies or when the EP needs to search for an imaging study HIT Summit Page 4 Updated 7/15/2014

As health care providers accountable for entire communities, we must redefine ourselves, and our role in the community. What does it mean to focus on the health of individuals in the community instead of focusing on treating patients who present themselves to our facilities? Seniors make up a growing proportion of the population; they are disproportionately represented in our patient population. What would it take to help them maintain their independence and their health, not just treat them when they are sick?
