9 CERTIFICATION IN A HEALTHCARE PROFESSION OR HEALTHCARE WORK/HEALTHCARE VOLUNTEER EXPERIENCE: Continued Proof of Certification Options (Choose one option to meet this requirement) Certification Details Option 1: Department of Health license Option 2: Official college/university transcript Option 3: DT214 or Military Transcript Option 4: Physician in other country Option 5: Health care Work/Health care Volunteer Experience Log Form (p. 11, Application) NEW FOR Provide a photocopy of a Department of Health issued health care license. Examples of licenses: LPN, NAC, MA, PTA, EMT, LMP A sealed transcript from a regionally accredited college/university showing graduation/completion of health care related certificate or degree; an assessed international transcript showing graduation/completion of a health care related certificate or degree Provide photocopy of DT214 or official copy of Military Transcript. Documents military medical service and/or training. No time limit. A sealed, agency assessed international transcript showing graduation/completion of an M.D. or a sealed, agency assessed credential evaluation showing equivalency to a U.S. M.D. Documents at least 250 hours of paid work and/or volunteer experience with direct human patient care interaction. Log form is filled out by applicant with detail about tasks and duties performed and then signed by supervisor. Attaching a job description will not be accepted. There are no time limits on volunteering or work performed.

11 REQUIRED APPLICATION ELEMENT: SHORT ESSAY RESPONSE Review details regarding essay on page 7 of application. This is a points bearing element. Question: Describe a circumstance, obstacle, or conflict in your life and the skills and resources you used to resolve it. What did this teach you and how will that knowledge support your success in the nursing program at LWTech? NEW FOR
12 REQUIRED APPLICATION ELEMENT: SHORT ESSAY RESPONSE: continued Essay considerations:» Have your essay read by somebody else to edit for grammar and spelling» Consider using campus resources for editing and writing style guidance such as: Writing Center; Learning Lab and/or advice from previous ENGL course instructor if available» Answer all parts of the question» Your essay will be scrutinized for content, clarity NEW FOR of message, grammar and spelling» If evidence of plagiarism or academic dishonesty is discovered, your application will be rendered unacceptable and materials will be forwarded to the conduct officer for review and possible adjudication
13 TEAS Version 5 Details Testing Policy: Take test first time, then wait at least 30 calendar days before taking test again. After that retake, you must wait 365 calendar days before the testing cycle can begin over again. Scores are valid for two years The LWTECH testing center has chosen not to offer testing in their office in the week leading up to a deadline. This pertains only to their office. You may find that other assessment centers in the area can accommodate your need for a test date up to or on the application deadline.
14 REQUIRED APPLICATION ELEMENT: TEAS (VERSION 5) = TEST OF ESSENTIAL ACADEMIC SKILLS Reading paragraph comprehension, passage comprehension, and inferences/conclusions (48items, 58 minutes) Math whole numbers, metric conversion, fractions, decimals, algebraic equations, percentages, and ratio/proportion, stats (34 items, 51 minutes) Science science reasoning, science knowledge, biology, chemistry, anatomy, physiology, basic physical principles, and general science (54 items, 66 minutes) English and Language Usage punctuation, grammar, sentence structure, contextual words, and spelling (34 items, 34 minutes) GRAND TOTAL=170 questions in 3 hours & 29 min. for TEAS questions
