2 Physical Therapist Assistant Overview Program length: 6 quarters Day, full-time (varying schedule by quarter). Look at a current LWIT schedule of classes Off-site clinicals are 40 hrs/wk Own transportation required First 3 weeks of quarter IV (summer) Last 6 weeks of quarter V (fall) Last 7 weeks of quarter VI (winter) Program Cost: Tuition rates for Estimated Total: $13,329

3 PTA Program Admission Application Deadline: June 24, 2016 All prerequisite courses must be complete prior to application. There is no longer an in-progress provision. Up to 15 total applicants admitted per cohort PTA Dept. can create an alternate list for a program year-this is not a guaranteed practice

5 Prerequisite Grade Considerations Cumulative GPA threshold is 3.0 and is taken from the six prerequisite courses Minimum grade in each class must be 2.0 Cumulative GPA is initially a non-points bearing application requirement GPA could be used to rank candidates later in the review process in the event of otherwise equally qualified applications All prerequisite courses must be complete prior to applying.

7 References-Requirements (Used in Stage I) One reference form must be from a faculty/instructor Second reference form can be from another faculty/instructor or an employer, co-worker, advisor or counselor References can send completed form to you in a sealed envelope OR give to you in a sealed envelope, signed across flap Include both documents in your application packet Application element is worth up to 25 points

8 Physical Therapy Observation and/or Work Experience-Requirements (Used in Stage I) 50 total hours of observation of licensed PT or PTA in clinical settings one from Category 1 and one from Category 2 and spend a minimum of 4 hours in any location Use the forms provided in the application, returned to you sealed, signed across flap Required component to ensure that applicants gain first-hand knowledge of the field Applicant is responsible for finding and setting up own observation/work experience Application element is worth up to 25 points

9 Prepared Essay-Requirements (Used in Stage I) Two page essay Essay question in current application-page 6 of the supporting informion document Answer each part fully Careful editing Check grammar, punctuation and spelling Application element is worth up to 25 points

10 On-Campus Mandatory Essay- Requirement of top 30 candidates (Used in Stage II) Top 30 qualified applicants invited to write mandatory essay Essay is hand-written and proctored Essay questions provided at time of event Pen and paper provided On campus essay will be on July 19, 2016 Application element is worth up to 25 points

15 Two Options for Advising OPTION 1: For quick advising needs (10 to 15 minutes) drop-in General Advising in West 207. Use this option if: You want to immediately discuss your COMPASS results You want to register for classes for the upcoming quarter (that s one quarter of classes only-not a full class plan) You are checking in on progress with a plan that was already created Drop-in schedule varies by quarter call ahead to check schedule: (425)

Option 2: For comprehensive, advising for future Dental Hygiene, Nursing & PTA applicants Register for Pre-Allied Health Advising from the PTA, Nursing or DHYG Department webpages.
