PACE an affiliate of PSI Program for Accessible Health Communication and Education (PACE) from 2009 Continues to implement all PSI Uganda programs Government of Uganda/Ministry of Health mandate Focus on people in need Working through partnerships page 2
Diarrhea and safe water system PHAs in Uganda have 6 times higher rate of diarrhea Safe Water system involves chlorination and safe storage of water in home RCT among 509 PHA 25% fewer diarrhea episodes and less dysentery Benefits all family members Costs $4 per year per family page 3
Roll out of BCP among PHAs Support from PEPFAR/CDC-Uganda Collaboration with MoH & 198 PHA organizations, PACE led implementation for 5 years ( ) Goal: Contribute to the reduction of morbidity and mortality caused by OIs among PHAs and prevention with positives As of 2008, over one million Ugandans were living with HIV (1,101,317 MoH 2009) page 4
Components of the Preventive Care Package Family-based HCT PWP Cotrimoxazole prophylaxis Safe water system Insecticide treated mosquito nets INH prophylaxis Multivitamins ARV and TB care 5
Safe Water System (SWS) Water Treatment Filtration with cloth Disinfection with chlorine Waterguard and PuR Safe storage of water 20-liter water vessel Maintains disinfection Prevents recontamination Behavior change Hand washing Improved sanitation 6
Household Water Treatment Options Safe Water Solution (Sodium Hypochlorite Solution) - Water Guard Liquid Calcium Hypochlorite tabs(water Guard Tabs) Flocculant Disinfectant - PuR (Purifier of Water) page 7
Achievements Distributed 314,626 basic care package kits reaching 28% of PHAs in Uganda Distributed 1,258,504 bottles of safe water solution At least treating 830,612,640 liters of water. All water treatment options are available to the public through SM efforts (sustainability) Integrated into HIV care & support partners activities page 8
Results baseline and follow-up Indicators Baseline (2005) Follow up (2010) PHA boiled water 60.4% 39% Ever heard of water treatment with Chlorination technologies 7% 95% PHAs who used Chlorination to treat water 1.8% 65% Doing nothing 35.9% 5.1% page 9
Reasons for non use of WG among PHAs who received BCP kit (n=682 Follow up study 2010) Reasons for non use % Never heard about WaterGuard 5.1 My current water source is safe 8.8 Bad taste/smell 27.8 Don't need 2.2 Too difficult to use 0.6 I prefer to boil my water 16.7 Run out of stock 36.1 page 10
Next steps ( ) Distribute 500,000, 15 million, 5 million Waterguard solutions, tablets and PuR sachets respectively Increase % of PHA who consistently use SWS to treat drinking water from 65.9% to 95% Increase % of PHA who consistently store drinking water properly from 66.9% to 95% Reach 250,000 PHA with product refills and replacements and 150,000 PHAs with the starter kit. page 11
Partnership with Alive Medical Services (AMS) supported by P&G Integrated Nutrition/HIV Program in Central region for PHAs 2,000 families reached in 1 yr (30 sachets per family per month -10 liters daily) Procured 414,000 sachets of PuR (4,140,000 litres of safe water) from PACE PACE conducts staff TOT and IEC on proper product knowledge and use page 12
Partnership with FXB supported by P&G Community WASH project in Kampala and Kyenjojo districts Targets 800 households and 97 schools Reached 595 PHAs with PUR sachets. 573,623 PUR sachets supplied by PACE to FXB page 13
Lessons learned Integrating HWTS into HIV programs reduces diarrhea related morbidity in PHA and their families. Establishing local partnerships minimizes service duplication and increases reach to hard to reach communities. Interpersonal communication by PHA peer educators increases BCP uptake page 14
Lessons learned cont d It is possible to support local manufacturers to produce BCP commodities (Safe water system) sustainable supply Social marketing of BCP commodities to the general public reduces stigma of product ownership page 15
