A patient navigator works individually with each patient to coordinate care among our team of pain management, physical medicine and rehabilitation, neurosurgery, orthopaedics and physical therapy experts. We track and measure the success of patient care, and use that information to improve how we treat patients. Conservative approach to surgery. We work diligently with your patients and referring providers to identify other treatment options before suggesting surgery. If surgery is the best option, some of the foremost surgical experts are on staff. Multi-disciplinary team all under one roof. Rehabilitation specialists, pain management staff, orthopaedic physicians and neurosurgeons are available to help improve the lives of your patients. Measurable results. All patient results are tracked and reported through a sophisticated computer system so that we can monitor and improve clinical performance. Easy access. To help patients start feeling better as soon as possible, we provide speedy access to appointments, accept most health plans and welcome self-referrals. 2

Currently we are measuring functional outcomes at the OHSU Spine Center, to improve patient s physical functions and pain levels, leading a return to productive lifestyle. The Functional Outcomes Program uses a secure web-based survey tool to collect patient medical and social histories, and validated quality of life measurements at each appointment. The program allows us to not only track your patient s progress, but also to adjust treatment plans to optimize progress and identify best practices for conservative and surgical spine care.
