4 Introduction Healthy Families Florida (HFF) provides the HFF Performance Management System to projects to assess their progress in achieving goals related to process measures and participant outcomes. Users may access the HFF Performance Management System through the Healthy Families Florida Systems Access Page at HUhttp:// The HFF Performance Management System uses data from the HFF Tracking System to generate a wide array of reports. Users can define reports by various sorting and grouping options such as date, staff and satellite office. In addition, each report is designed to allow the use of the Find (Ctrl + F) function to search. The reports are divided into seven major categories: 1) The Screens/Assessments category includes reports to assist the project in monitoring screening and assessment of participants and families. These include: Aggregate Report for Assessment Results Aggregate Report for Monthly Assessments Assess/Agree/1st Visit Intervals Duplicate Assessments Check Report Participant Assessments by Result Screening Analysis Report Screening Zip Code Report Aggregate Report for HFF Referral Sources Aggregate Assessment Referrals Report 2) The Capacity/Closures category includes reports to assist the project in monitoring site capacity and participant and family closures. 5 3) The Outcomes/Process Measures category includes reports that provide an overview of the project s progress with meeting all of HFF participant outcomes and process measures and reports to assist the project in monitoring data that directly affects participant outcomes and process measures. They provide participant lists, information on due dates, referrals that have been made and the status of those referrals, information about home visits and home visit attempts, and information collected on individual families. 6 5) The Demographics reports provide general descriptive information about the participants served by the project. These reports may be useful for preparing the Annual Service Review. These reports include: Participant Demographic Report (All) Participant Demographic Report (Closed Cases Only) Participant Race/Ethnicity Report (By Race) Target Child Demographic Report (All) Target Child Demographic Report (Closed Cases Only) Target Child Race/Ethnicity Report (By Race) Assessment Demographics Report Staff Demographic Report 6) The Data Checks reports are useful for identifying erroneous data or reviewing specific types of data. These reports include: Child Insurance Clinical Positive Education Levels at Intake/Close Kempe Scores TANF-WAGES Eligibility Target Children w/ Gestational Age 37 and Under Target Children w/ Low Birth Weight (Less than 5 pounds, 8 ounces) Target Children w/ Incorrect DEIP Target Children Marked as Severely Delayed Unusual or High Assessment Scores 7) The Missing Data reports are useful for identifying specific types of missing data. The following document provides samples of each of the reports listed above, including a description of the report and the report s organization options. The best course of action for the user to familiarize themselves with the reports is to review the various reports on-line for their project. 8 Systems Access The Healthy Families Florida Systems Access Web page located on the Healthy Families Florida website, HUhttp:// provides users with access to the following components: The Ounce Learning Management System link provides access to the Ounce Learning Management System (OLMS) for HFF training. The HFF Tracking System is the data entry application. The Staff Management System is used to maintain program staff and supervision information. The Staff Management System also offers reports on training received by program staff. The Staff Management System User s Manual provides detailed information for each section of the Staff Management System. Effective 01/02/2013 8
9 The Performance Management System provides access to data reports. The Performance Management System User Manual (this document) provides a detailed description of each report in the HFF Performance Management System. The FTP Site provides access to requested ad hoc reports, install programs, specialized reports developed by HFF and other files. The FTP site is password protected. Program managers must contact the MIS specialist for the username and password. The Request System Enhancements/Modification link allows program site staff to request additional reports or changes to current reports. The Request Data Entry/Service Record Modifications form allows program site staff to request modifications to data or archived service information. This document must be password protected. Program managers must contact the MIS specialist for the password. System Login After selecting the Performance Management System link, the user is directed to the Login Screen. 11 Aggregate Report for Assessment Results The Aggregate Report for Assessment Results report provides aggregate data for assessment results during the specified time period. This report may be organized in the following manner: By a select period of time By a select satellite site By a select FAW or staff conducting assessments By a select zip code Suggested Uses: To determine the results of assessments conducted during a specific time frame To view the program acceptance rate (by project, satellite or by FAW) To determine the number of assessments still on outreach indicated by a None Selected assessment result To identify areas for improvement among families who scored out or lost contact before enrollment A sample of this report follows on the next page. 13 Aggregate Report for Monthly Assessments The Aggregate Report for Monthly Assessments report provides aggregate data on the number of assessments completed during a fiscal year. This report may be organized in the following manner: By a select fiscal year. The previous fiscal year s data may also be included By a select FAW or staff conducting assessments Suggested Uses: To monitor the rate of assessments completed for the project or per staff member. The expectation is that a full-time FAW can conduct 20 assessments per month A sample of this report appears on the following page. Effective 01/02/

15 Assess/Agree/1st Visit Intervals The Assess/Agree/1 st Visit Interval report lists participants and their assessment, agreement and first home visit dates. The report tallies the number of days between the assessment date and agreement date, the days between the assessment date and the first home visit and the number of days between the agreement date and the closure date. The report may be organized in the following manner: By open, closed or both open and closed children By a select satellite site By a select FSW Suggested Uses: To identify families who have not received their first home visit within 30 days of assessment To identify lag time between assessment and agreement or between agreement and first home visit A sample report follows on the next page. 17 Duplicate Assessments Check Report The Duplicate Assessments Check report lists all participants with multiple assessments in the HFF Tracking System. Participants should receive only one assessment per pregnancy, so assessments listed in this report may represent duplicate assessments. The report may be organized in the following manner: All duplicate assessments, or only duplicate assessments within 10 months of each other By the date of the most recent assessment Suggested Uses: To ensure no participant has duplicate assessments More information on duplicate assessments is available in the HFF Policies & Procedures Manual and the Data Collection Forms and Guidelines. 19 Participant Assessments by Result The Participant Assessments by Result report lists individuals assessed using the Healthy Families Florida Assessment Tool. The report may be organized in the following manner: By the date the assessment was conducted By assessment result By a select satellite site By a select staff person (Assessment worker or Creative Outreach worker) Suggested Uses: To ensure all assessments have been entered To ensure all assessments have a final assessment result identified To identify families refusing at assessment or during outreach. 21 Screening Analysis Report The Screening Analysis report lists all monthly screen data. The report may be organized in the following manner: By date, selecting a specified time period for reviewing screening data By zip code or multiple zip codes Suggested Uses To determine the number of families screened during a specified period of time. This information may be useful when developing the Annual Service Review To observe outcome patterns of screens processed by the project and assist in reviewing changes that may be needed to increase the number of families assessed To check data accuracy, compare the Total # that should be assessed (the number of expected assessments) with the Total # assessed (the number of assessments completed and entered into the HFF Tracking System) for the same time period A sample report follows on the next page. 34 Report Definitions A. This number includes all participants that were open on the first day of the quarter, including those that opened on that date. B. This number includes all families enrolled from the first day of the quarter through the last day of the quarter. C. This number includes all families closed during the quarter, from the first day of the quarter though the last day of the quarter. D. This number includes all families served during the quarter. E. This number includes all families that were open on the last day of the quarter, including any families that closed on the last day of the quarter. F. This number includes all families that were served from the beginning of the fiscal year to the end of the quarter or the end of the selected time period. Effective 01/02/
35 Attachment A (With Date Parameters) The Attachment A report (with Date Parameters) provides data on the number of families served and enrolled for a selected period of time. 63 Child Medical Provider The Child Medical Provider report lists target children and their medical provider. The report may be organized in the following manner: By open, closed or both open and closed target children By a select satellite site By FSW Suggested Uses: To identify target children without a medical provider listed. Hospital Outpatient is not considered a medical provider. FSWs with children who use the hospital as their primary care should continue to work with the family to identify an actual medical provider. 65 Initial Home Visit The Initial Home Visit report lists participants and the date of their first home visit. The report automatically tallies the number of days between the target child s date of birth and the first home visit.
