1 Procedure Number : QP-DSS-001 Rev No: 0 Effective Date: 01/08/2008 Page No: 1 of 8 STANDARD OPERATING PROCEDURES 1.0 INTRODUCTION The objective of this fire and evacuation procedure manual is to familiarize all members of the Emergency Response & Evacuation Team (ERT) with the emergency procedures in place to facilitate safe, orderly and timely evacuation when necessary. Whilst fire is the principal reason for the implementation of an evacuation plan, other instances such as gas leak, bomb scare etc, may well necessitate evacuation of the premises. It is of vital importance that all members of the ERT are familiar with the procedure. In the event that any part of this procedure is unclear, please contact the ERT members. 2.0 FIRE DRILL AND EVACUATION TEAM / SET UP The size and configuration of the premises, together with the type of occupancy, will determine the interval between practice evacuations. The period between partial evacuations can vary between quarterly and yearly. They should be conducted even if they only involve the staff and ERT members. 2.1 Emergency Response Team members: Emergency Commander (EMC) Evacuation Controller (EC) Fire Fighting Team / Student committee Fire Floor Marshal (FFM) Search and Rescue Team Security Controller (SC) First Aid Team Observer, if any (Bomba)

4 Procedure Number : QP-DSS-001 Rev No: 0 Effective Date: 01/08/2008 Page No: 4 of All staff must be briefed by the ERT on the following: The identity of the marshals; Alarm system and the preferred method of reporting emergencies; Actions they are to take in response to the alarm signals; Assembly areas for their section; What is required at the completion of the exercise. This should not be a no-notice evacuation. Adequate warning should be given as the first evacuation can be difficult enough without the added confusion brought about by surprise. 2.4 Ensure the assembly areas are identified and to prepare evacuation routes before the fire drill (see attachments). 2.5 To prepare Chronological Plan of fire drill & evacuation exercise (Appendix A). 2.5 To prepare equipment(s) required for the fire drill & evacuation exercise. 2.6 To start fire drill & to prepare evacuation exercise report after the drill. A debriefing meeting conducted by the Emergency Commander after each practice evacuation is essential in order to identify any shortcomings in the organization or procedures, and then to amend the building response and evacuation plans where necessary. 3.0 PROCEDURES IN CASE OF FIRE 3.1 You discover a fire: Help people in immediate danger Warn others by shouting Fire, Fire, Fire, raise the alarm if not yet already sounding and telephone Decide if you can put the fire out, if you are not sure, do not attempt to Don t attempt to use a fire extinguisher if you have never been instructed on how to use one.

5 Procedure Number : QP-DSS-001 Rev No: 0 Effective Date: 01/08/2008 Page No: 5 of If you can put out the fire then do so, if not proceed to evacuate the building Inform the Security Department of the fire location and details 3.2 You hear the fire alarm ringing; you must prepare to evacuate the building: Switch off all computers, printers and electrical appliances Close all windows and doors Gather your personal belongings in preparation to immediately evacuate the building Organize/help other people in the room. 3.3 Evacuate the building and proceed to the assembly area: Move at a quick walk, do not run DO NOT PANIC If you have to move through a closed door that you cannot see through: Feel the door to see if it is hot; Look for smoke coming under the door; Open the door slowly and look around it to see if there is a fire behind it; If there is no fire on the other side, proceed through and close the door behind you; DO NOT USE THE LIFTS Walk down the fire escape staircase to the ground floor If the smoke is too thick, get under the smoke by crawling. You should be able to see where you are going

6 Procedure Number : QP-DSS-001 Rev No: 0 Effective Date: 01/08/2008 Page No: 6 of Move to the assembly areas quick as possible Report to your marshal that you/your group is there and if you know of anyone trapped in the building; Remain in the assembly area until you are informed that you may leave or move by either the marshal or a member of the ERT. 3.4 You notice someone on the verge of panic: Give them a task or responsibility; The person will still require constant monitoring; Do not hit them; Take hold of one of their hands and guide them out of the building to the assembly area; If they will not cooperate or start to grab hold of things. Leave them where they are; Evacuate yourself and inform your marshal, the Evacuation Control, one of the security personnel, or the police or Fire and Rescue Services immediately. 3.5 If you are trapped in a room: Exit through a window if you are on the ground floor; If you are not on the ground floor or cannot exit through a window: Close the door; Go to the window; If there is smoke in the room open the window a little so you can breathe fresh air. If not, do not open the window. This can assist in the spread of fire into your area from lower floors; Attract people's attention to your plight. This can be achieved by writing on a paper and sticking it to the window or

7 Procedure Number : QP-DSS-001 Rev No: 0 Effective Date: 01/08/2008 Page No: 7 of 8 calling out the window. If you open the window remember to close it again as this can be an entry point for fire. Do not open the window up fully; If the room is filling with smoke, stay close to the ground. The air is cooler and oxygen is more plentiful in this region; Wait for the Fire and Rescue Service to rescue you. 3.6 Do not procrastinate: REMEMBER Fires spread rapidly; Fires produce thick black smoke that is difficult to see through and causes suffocation; The freshest air will always be near the floor; Move quickly. Do not run; Be decisive; Think for 10 to 30 seconds; Make a decision and follow that decision. 3.7 Mobility impaired people If you encounter a person with some form of physical disability that restricts their mobility, you may be required to assist them from the building. If you are unable to remove them from the building many stairwells have respite areas for mobility impaired persons. They can be left here, where they are shielded from the fire, and retrieved by emergency personnel on arrival. It is important to inform the emergency personnel or Building Warden of their location so that they can be retrieved as soon as possible. Leave the person's mobility aids behind; they can always be claimed on insurance.

All enquires should be forwarded to the Chairman of OSH. This is to ensure that the media is furnished with accurate information and additional copycat incidents or crank calls are not precipitated by irresponsible statements from uniformed sources.
