1 HOW TO FIND US From the M4. Leave at Junction 10 and take the A329(M) towards Bracknell. At Twin Bridges roundabout take the third exit (A3095 Mill Lane), immediately take the slip road to the left. At the T-Junction turn left, follow the road round, the home is just past Aldworth Close on the left. From the M3. Leave at Junction 3 and take the A322 towards Bracknell. At the second roundabout turn left into South Hill Road, signposted Sandhurst. At a mini roundabout turn right into Reeds Hill and at the next roundabout turn right into Crowthorne Road. The home is just past Saffron Road on the right. 2 Welcome to Bracknell Care Home Bracknell Care Home is a purpose built care home providing a high standard of nursing care and care for physically disabled adults. We also provide short term respite care. The home is located in a residential area of Easthampstead within easy access of Bracknell centre, close to local shops and amenities and transport links. Bracknell Care Home has a happy, strong, motivated team of staff which is committed to providing a high standard of care to its residents, remembering that we work in their home and not that they live in our work place. 3 First Class Care Our person centred approach means that each resident is an individual treated with respect and dignity. Each resident has a named key worker and an individual care plan. This will cover their personal and medical history, as well as a record of particular activities they enjoy and any cultural or religious aspects of their care. Residents, and where appropriate their families, are involved as far as practical in the decisions regarding their care. Emphasis on all aspects of care is the Manager s primary concern and, together with the experienced team of qualified staff, all adhere to rigorous standards with internal audits and checks. At Bracknell Care Home, we have an excellent working relationship with the partners from the local GP practice. We very much recognise the importance of personal, individual and resident focused care. To achieve this, good communication between all members of the care team, be they nurses, carers, relatives or doctors, is essential.

4 Facilities The home is tastefully decorated and benefits from a bright, spacious lounge area with a flat screen television and a quiet lounge area to relax in. The home has large bedrooms each with en suite, television and personalised to each resident s taste. The residents are encouraged to personalise their rooms with pictures, photos and mementoes. At Bracknell Care Home there is an inviting dining room where our Chef provides nutritionally balanced menus that are carefully prepared to ensure the best combination of nutrition and flavour for our residents. We have a personal activities leader who ensures our residents can enjoy an active social life if they wish and residents can get involved in hobbies of their choice and take part in activities that interest them. The varied and interesting programme includes handicrafts, quizzes to regular trips out to local places of interest such as local museums. At Bracknell Care Home there are on-site hairdressing facilities available that are visited regularly by an experienced stylist. Other complementary therapies can be tailored to individual needs and the home is attended frequently by a qualified chiropodist. The home is well equipped with specialist equipment in bathrooms and has a lift and passenger stairway to access both floors. The home has pleasant gardens with a patio area where residents and visitors can enjoy relaxing outdoors in the warmer months.

5 Types of Stay Bracknell Care Home provides a high standard of general nursing care. We also cater for those who need short term or long term respite care. The home has a team of well trained dedicated care assistants and qualified nurses that provide an excellent standard of 24 hour care. We fully appreciate the stress associated with leaving home to live in a new location and are happy to accept new residents for an agreed trial period at the end of which there would be no further obligation.
