Emergency Drug Card City of Ottawa Employment and Financial Assistance Persons living in shelters are not eligible for Ontario Works but may receive essential health and social support services, which includes an emergency drug card. Patient self refers by calling from Mon Fri: Prefer SIN card, health card and banking info but may open file without this ID, based on name and birth date. Need the paper copy of a prescription or a prescription at a pharmacy to be eligible. An entry point for treatment, for relapse prevention and to support individuals who are in crisis related to drugs/alcohol. Same as above. 6 Emergency Shelters General Information Concerning Patients from Shelters The City of Ottawa has centralized information concerning shelter bed availability. Call 311. Almost all shelters have the requirement that clients vacate the premises during the day. Clients are only allowed bed rest if they have a doctor s note. Paper copies of prescriptions are at risk of becoming lost. Consider faxing the prescription to the patient s pharmacy.
