2 Basic Health Care OHIP Card ID Assistance Sandy Hill Community Health Care Centre Will assist with application for OHIP. May use Sandy Hill address for OHIP card. Will also assist in applying for other forms of ID, such as a SIN card. Call and book an appointment for ID. Emergency Drug Card City of Ottawa Employment and Financial Assistance Persons living in shelters are not eligible for Ontario Works but may receive essential health and social support services, which includes an emergency drug card. Patient self refers by calling from Mon Fri: Prefer SIN card, health card and banking info but may open file without this ID, based on name and birth date. Need the paper copy of a prescription or a prescription at a pharmacy to be eligible. 4 Addictions Detoxification Hope Recovery at Shepherds of Good Hope 256 King Edward Ave., Ottawa beds for male and female clients under the influence of alcohol and/or mood altering substances. Ottawa Withdrawal Management Centre 62 Bruyere St., Ottawa, ON Cornwall Withdrawal Management Centre 35 2nd St E, Cornwall ON Substance Use and Concurrent Disorders Program ROHCG 1145 Carling Avenue Ext Harm Reduction Withdrawal in a non-medical setting. An entry point for treatment, for relapse prevention and to support individuals who are in crisis related to drugs/alcohol. Same as above. Inpatient program with medical detox and outpatient program. 6-8 week waiting list. Office hours are Monday to Friday, & Call first to ensure that there is an available bed. Self- or clinician-referral. Call to ensure there is an available bed and for prescreen. No detox meds are provided, but staff will dispense meds. Ask about faxing prescription to pharmacy. 6 Emergency Shelters General Information Concerning Patients from Shelters The City of Ottawa has centralized information concerning shelter bed availability. Call 311. Almost all shelters have the requirement that clients vacate the premises during the day. Clients are only allowed bed rest if they have a doctor s note. Paper copies of prescriptions are at risk of becoming lost. Consider faxing the prescription to the patient s pharmacy.
