Original copies of the following: 3.1 NSO Birth Certificate or any valid substitute 3.2 Secondary Scholastic Records (previously uploaded in the OAF) 3.3 Certificate of Good Moral Character (previously uploaded in the OAF) Document To Be Received Stamped Letter of Acceptance from DLSU Please see next page for preliminary PROCEDURES and REQUIREMENTS for International Students Students with Dual Citizenship Filipinos Born Abroad Notes High School graduates before AY must submit a Declaration as a New Student form (use link below) All graduates from International Schools abroad must present an authenticated Transcript of Records and the Certificate of Eligibility from the Department of Education. Admission status of high school graduates of schools not included in DepEd Order No. 42, s. September 8, 2015 Version 2 is conditional and subject to the school s submission of necessary certificate Checks are not accepted 2 Payment of Confirmation Fee (PhP 10,000.00) Payment Referral Slip DLSU Official Receipt (for confirmation) The Confirmation Fee will be credited to the tuition and fees if enrolled on Term 1 of AY -17. Otherwise, it is non-refundable and will cover administrative costs. 3 Claiming of Confirmation Card DLSU Official Receipt (for confirmation) 1. Confirmation Card 2. Enrollment Schedule and Procedures The required attire for male is plain polo with tie while the required attire for female is any blouse or dress with collar and sleeves. 4 ID Picture Taking Confirmation Card All students are required to wear clothing that will contrast against a beige background The ID In-charge will refuse to take the picture of students who will not comply with the required attire The IDs will be distributed on enrollment day

International Students Who will Apply for Student Visa and Special Study Permit DLSU, through International Center, will process this at the Bureau of Immigration Reference: Brochure Visa Conversion Process The following documents must be submitted: o o Application forms for visa conversion or SSP (available at the International Center) DLSU Official Receipt of Payment for visa processing PhP 14,700 for student visa conversion PhP 9,000 for Special Study Permit 2. Students with Dual Citizenship and without substantial proof of Filipino citizenship must apply for Student Visa or Special Study Permit. Please refer to item Number 3 (Documents to be Submitted by Students with Dual Citizenship) for the list of documents that show substantial proof of Filipino Citizenship.
