It is designed and required to be constructed to meet LEED Silver Standards. Tenders anticipated It is the intention of the owner to pre-qualify a list of Sub-Contractors specifically who will be invited to submit bids on this project. 3 DIVISION SCOLAIRE FRANCO-MANITOBAINE PRE-QUALIFICATION OF MASONRY SUB-CONTRACTORS April 15, 2013 The Division Scolaire Franco-Manitobaine invites Masonry Sub-Contractors to pre-qualify for bidding on the erection of a 775 sq. m. wood frame daycare and 1,340 sq. m. concrete, concrete block and steel classroom addition at Ecole Tache. It is designed and required to be constructed to meet LEED Silver Standards. Tenders anticipated It is the intention of the Division to pre-qualify a list of Masonry Sub-Contractors specifically who will be invited to submit bids on this project. The following documentation is mandatory: CCDC 11 Contractor s Qualification Statement appropriately completed (Contractors shall be responsible to purchase and apply the CCDC 11 Copyright Seal), including the division s Contractor s Qualification Statement Appendix D. A minimum of three (3) references from recent similar projects, including location of project, description and value of work, name and telephone number of person to contact for verification of reference. Indication of local office. Evidence of ability to provide Bonding, Insurance and Workers Compensation Board coverage. Curriculum Vitae of proposed project manager and foreman. Indication of current workload for Copy of certificate of recognition in the National COR Standard Safety accreditation program. Letter of confirmation of engagement in the Apprenticeship Manitoba System, Province of Manitoba. Letter of Declaration identifying: - no dispute or pending dispute involving the Government of Manitoba (MB) or any School Division in MB - no owing of any monies to the Government of Manitoba or any other School Division in MB that is overdue or otherwise delinquent - no conflict of interest, possible conflict of interest or perceived conflict of interest with the Government of MB or any School Division in MB pursuant to this prequalification process. Basis of Selection: It is understood by the contractor submitting a pre-qualification that to be considered for the construction contract (s), Sub- Contractors must achieve a minimum overall score of eighty (80) points. The rating is performed on a scale of a hundred (100) points; all contractors who achieve a minimum overall score of eighty (80) points will be pre-qualified for the bidding phase of these projects.
