2 REQUEST FOR QUALIFICATIONS PART 1 - PROJECT OVERVIEW 1.1 The City of Centennial is interested in pre-qualifying Construction Contractors for the City s 2013 Concrete Rehabilitation Program, which will include work generally consisting of removal and replacement of concrete curb, gutter, sidewalk, curb ramps, crosspans, inlet tops and gutter pans, sidewalk chases, removal of driveway fillets, and installation of new handicap ramps, all of which shall be constructed in accordance with current ADA standards (collectively, the Work ). Following the date on which the City has compiled its short list, as explained in additional detail below, the City will request bids from the selected qualified firms. All Work in the program will be within existing City of Centennial right-of-way, unless strictly approved by the engineer. 1.2 Budget. The construction budget for the 2013 Concrete Rehabilitation Program (the Program ) is estimated to be approximately Eight Hundred Thousand Dollars ($800,000.00), but the Program budget may be adjusted up or down by the City in its discretion. 1.3 Qualification Submission A. The City of Centennial, Colorado will accept responses to this RFQ via written submissions returned to the City on or before the following closing date and time: 2:00 p.m. on Friday, January 11, 2013 to: City of Centennial, Attn: Elizabeth Dunaway, Contract Administrator, E. Arapahoe Road, Centennial, CO B. Written responses to this RFQ shall be marked as follows: 1. City of Centennial 2013 Concrete Rehabilitation Program, Project No RFQ C. Complete construction bid documents will be available at a later date for the firms selected pre-qualified through this RFQ process. D. Respond to each question and do not attach additional marketing documents to the qualification submission. E. Any questions regarding the Program should be directed to Elizabeth Dunaway, Contract Administrator, at or edunaway@centennialcolorado.com. Any and all costs incurred by the respondent in the preparation and delivery of the written response to this RFQ and subsequent bid are the sole responsibility of the respondent and will not be reimbursed by the City of Centennial. G. No consultant, subconsultant, contractor, or subcontractor will be accepted as a member of a Construction Team if any of the consultants, subconsultants, contractors or subcontractors is in arrears to the City of Centennial, or has any claims, arbitration proceedings, or lawsuits pending, outstanding, or threatening to the City. PART 2 - PROJECT SCOPE 2.1 Project Description A. The goal of this Request for Qualifications (RFQ) is to prequalify potential Construction Contractors for a subsequent construction bid to be issued by the City of Centennial for the Program. Included in the Program shall be work including but not limited to: Providing construction zone traffic control on City streets including residential, collectors, and arterials. Removal and replacement of concrete structures such as: o Sidewalks o Curb and Gutter o Crosspans o Combination curb/ gutter/ sidewalks o ADA accessible ramps; Concrete slab replacement; Concrete drainage structures (as directed); Concrete joint and spall repair; Concrete patching; and Restoration of property affected by construction activities associated with Work. B. The Construction Contractor and the Contractor s sub-contractors (if applicable) (collectively, the Construction Team ) selected through the subsequent bid process will be the Contractor for the Program in accordance with the Contract Documents applicable to the Work, and as directed by the City. Currently, the target date for the Work to be substantially complete is August 15,

4 1. General Comments Regarding the Work: a. The Contractor shall be required to provide and maintain a secure Work area. b. The Contractor shall be responsible for coordination with applicable agencies to ensure that underground utilities and services are not unreasonably impacted by the Work. c. All Work must be completed in a timely manner. d. Following final acceptance by the City of Centennial, the Contractor shall warrant all Work in the Program for a minimum of one (1) year. 2.2 Details and Elements of the Request for Qualifications A. Contractors preparing written responses to this RFQ should demonstrate a high level of competence in the construction of similar roadway construction projects together with a demonstrated ability to work effectively with: public agencies, utility companies, and other stakeholders. The selected Contractor should have demonstrated ability to provide high quality services on time and within budget. The City of Centennial requires certain minimum qualifications in insurance coverage, indemnification, and financial standing. B. The Construction Team preparing responses to this RFQ shall submit four (4) copies of the responses on or prior to the closing date and time identified in this RFQ. Proposers are required to provide responses to the following list of questions and requirements. Responses should be in the form of 8-1/2 x 11 reports. The total number of pages shall not exceed fifteen (15) pages. The written responses to this RFQ shall include, at a minimum, the following: 1. Include information about each firm listed as a member of the proposed Construction Team including size, history, office locations, expertise, bonding, insurance and banking references; and specific experience relative to the Work anticipated to be completed as part of the Program. a. Include number of years the firm(s) has/have been in business as a Roadway Construction Firm/Contractor. b. Include number of years the firm(s) has/have been in business under its present business name. c. List the point of contact and contact information for each member of the Construction Team. d. Have any of the members of the Construction Team ever had any past or pending litigation against an Owner of a project with whom the firm(s) contracted work? If so, explain. e. Have any of the members of the Construction Team ever paid Liquidated Damages or had Liquidated Damages deducted from any contract to which the member(s) of the Construction Team was/were a party? If so, explain. 4

Have any of the members of the Construction Team ever had an Owner of a project with whom the firm(s) contracted work submit a claim to the surety under a payment bond, performance bond or warranty bond? If so, explain. g. Include a current balance sheet and profit/loss statement for each member of the Construction Team. 2. Project Construction a. As an attachment, provide the following information on three (3) projects similar to the Work to be completed as part of the Program, that have been completed within the last five (5) calendar years. 1) Name, location and brief description of each project. 2) Name, address, and telephone number of the Owner on each project (indicate contact person). 3) Original budgeted cost and final construction cost of each project. b. A brief statement of your communications strategy for the Work to be completed as part of the Program. Provide a statement of understanding in regards to working in various environments. (i.e.: residential, commercial, school zones, etc.) Provide a statement on Work scheduling (planned prosecution and progress) within the communications strategy. c. Submit a comprehensive strategy for traffic control for the Work. Identify traffic control personnel and qualifications or traffic control subcontractors qualifications, if applicable. d. Submit a summary of the specific resources that the Construction Team is willing to commit to the Work should the Construction Team be awarded the contract following the bidding process, from the date of the Notice to Proceed issued by the City through the date of substantial completion of the Work (the Resource Summary ). The Resource Summary should include a listing of specific manpower resources and equipment that the Construction Team is willing to dedicate exclusively to the Work, as well as a listing of other resources and equipment that the Construction Team has available to commit to the Work on an as-needed basis. e. A summary of how any disputes and claims for damages will be handled. Provide examples of how disputes have been handled on similar past projects. f. The structure of the Proposer s Construction Team, including the proposed construction superintendent and other key personnel that would be assigned to the Work, identified by name and title. The construction superintendent and other key personnel may also be listed and identified in the Resource Summary. 5

6 g. The Proposer will be responsible for quality control (QC) testing and inspection, and the City of Centennial will be responsible for quality assurance (QA) testing and inspection. Please submit a strategy for conducting required QC testing and executing and reporting all QC information to the City s Project Manager. C. Provide any additional information that the Proposer feels is necessary to substantiate qualifications to perform the Work and services required for the Program. D. The four (4) copies of the written response to this RFQ are to be submitted to: 2.3 Selection Process City of Centennial Elizabeth Dunaway, Contract Administrator E. Arapahoe Road Centennial, CO A. The City anticipates selecting a list of most qualified firms (the prequalified list ) after receiving and reviewing the written responses received as part of this RFQ. The City will review and evaluate the proposals submitted by all responsive Proposers. The City will select the most highly qualified Proposers to receive bid documents for the Project, which will outline the remainder of the selection process requirements. B. Criteria used to select the prequalified list will be based on, but not limited to, any or all of the following: a. Demonstrated quality of experience in the Denver Metropolitan region and demonstrated ability to successfully complete projects of comparable scope and complexity; b. Completeness and content of Construction Team information; c. Completeness and content of written responses to this RFQ; d. Responses from references; and e. Review of other information submitted. The City of Centennial reserves the right to select the prequalified list following the review of the written responses submitted by the Proposers to this RFQ. The City further reserves the right to select the prequalified list that it deems to be in the best interests of the City. C. The City reserves the right to reject any Proposer submitting a response to this RFQ and to select only those Proposers that are determined to be the most qualified to complete the Work, as determined by the City in its sole discretion. The City will not be responsible to defend, document or demonstrate its selection or rejection of any firm. 6

7 2.4 Schedule of Events. It is the intention of the City of Centennial to have the Work completed on or before the August 15, 2013 target substantial completion date. The table below sets forth the preliminary schedule of events. The City reserves the right to modify the dates set forth in the table below as necessary to accommodate the Work.
