SENATE ENROLLED AT N. 414 AN AT t amend the Indiana de cncerning insurance. Be it enacted b the General Assembl f the State f Indiana: SETION 1. I IS ADDED TO THE INDIANA ODE AS A NEW SETION TO READ AS FOLLOWS [EFFETIVE JULY 1, 2013]: Sec. 16. (a) As used in this sectin, "cvered individual" means an individual wh is entitled t cverage under a state emlee health lan. (b) As used in this sectin, "high breast densit" means a cnditin in which there is a greater amunt f breast and cnnective tissue in cmarisn t fat in the breast. (c) A state emlee health lan must rvide cverage fr an arriate medical screening, test, r examinatin fr a female cvered individual wh is at least frt (40) ears f age and wh has been determined t have high breast densit. SETION 2. I , AS AMENDED BY P.L , SETION 3, IS AMENDED TO READ AS FOLLOWS [EFFETIVE JULY 1, 2013]: Sec. 7. (a) The bard shall d the fllwing: (1) Adt rules and frms necessar t imlement this article that cncern, but are nt limited t, the fllwing areas: (A) Qualificatin b educatin, residence, citizenshi, training, and character fr admissin t an examinatin fr licensure r b endrsement fr licensure. (2) Administer aths in matters relating t the discharge f its fficial duties. (3) Enfrce this article and assign t the ersnnel f the agenc duties as ma be necessar in the discharge f the bard's dut. (4) Maintain, thrugh the agenc, full and cmlete recrds f all alicants fr licensure r ermit and f all licenses and ermits issued. (5) Make available, un request, the cmlete schedule f minimum requirements fr licensure r ermit. (6) Issue, at the bard's discretin, a temrar ermit t an alicant fr the interim frm the date f alicatin until the next regular meeting f the bard. (7) Issue an unlimited license, a limited license, r a temrar medical ermit, deending un the qualificatins f the alicant, t an alicant wh successfull fulfills all f the requirements f this article. (8) Adt rules establishing standards fr the cmetent ractice f medicine, steathic medicine, r an ther frm f ractice regulated b a limited license r ermit issued under this article. (9) Adt rules regarding the arriate rescribing f Schedule III r Schedule IV cntrlled substances fr the urse f weight reductin r t cntrl besit. (10) Adt rules establishing standards fr ffice based rcedures that require mderate sedatin, dee sedatin, r general anesthesia. (11) Adt rules r rtcl establishing the fllwing: (A) An educatin rgram t be used t educate wmen with high breast densit. (B) Standards fr rviding an annual screening r diagnstic test fr a wman wh is at least frt (40) ears f age and wh has been determined t have high breast densit. As used in this subdivisin, "high breast densit" means a cnditin in which there is a greater amunt f breast and cnnective tissue in cmarisn t fat in the breast. (b) The bard ma adt rules that establish: (1) certificatin requirements fr child death athlgists; (2) an annual training rgram fr child death athlgists under SEA 414 ncur+

I IS ADDED TO THE INDIANA ODE AS A NEW HAPTER TO READ AS FOLLOWS [EFFETIVE JULY 1, 2013]: hater Ntificatin ncerning Dense Breast Determinatin Sec. 1. A facilit that erfrms a mammgrah examinatin shall, if the atient is determined b the facilit t have an amunt f breast and cnnective tissue in cmarisn t fat in the breast that wuld require fllw u care r testing, ntif the atient f the determinatin. The ntice required under this sectin must be included with a summar f the written mammgrah rert. SETION 4. I IS ADDED TO THE INDIANA ODE AS A NEW HAPTER TO READ AS FOLLOWS [EFFETIVE JULY 1, 2013]: hater verage fr Medical Services fr Wmen with High Breast Densit Sec. 1. This chater alies t a lic f accident and sickness insurance that is issued, delivered, amended, r renewed after June 30, Sec. 2. As used in this chater, "high breast densit" means a cnditin in which there is a greater amunt f breast and cnnective tissue in cmarisn t fat in the breast. Sec. 3. As used in this chater, "insured" means an individual wh is entitled t cverage under a lic f accident and sickness insurance. Sec. 4. As used in this chater, "lic f accident and sickness insurance" has the meaning set frth in I The term des nt include the fllwing: (1) Accident nl, credit, dental, visin, Medicare sulement, lng term care, r disabilit incme insurance. (2) verage issued as a sulement t liabilit insurance. (3) Autmbile medical ament insurance. (4) A secified disease lic. (5) A shrt term insurance lan that: (A) ma nt be renewed; and (B) has a duratin f nt mre than six (6) mnths. (6) A lic that rvides indemnit benefits nt based n an exense incurred requirement, including a lan that rvides cverage fr: (A) hsital cnfinement, critical illness, r intensive care; SEA 414 ncur+

4 4 r (B) gas fr deductibles r caments. (7) Wrker's cmensatin r similar insurance. (8) A student health lan. (9) A sulemental lan that alwas as in additin t ther cverage. (10) An emler snsred health benefit lan that is: (A) rvided t individuals wh are eligible fr Medicare; and (B) nt marketed as, r held ut t be, a Medicare sulement lic. Sec. 5.
