I have attached documents establishing paid proof of purchase for the items applied for on this rebate application. I agree verification by San Diego Gas & Electric of both the sales transactions and products installation, and I certify that the information on this rebat e application is true and correct, and that the tax ID provided is accurate. I understand that rebate payments are based on related energy benefits over the life of the product. I agree that if: 1. I do not provide San Diego Gas & Electric with 100% of the related energy benefits specified in the rebate application for the life of the product or for a period of five (5) years from receipt of rebate, whichever is less, or 2. On Bill Financing - In the event that I qualify and participate in On-Bill Financing offered by San Diego Gas & Electric Company obtain financing for the energy saving products listed in this rebate application, I understand that the terms and conditions of participation in On-Bill Financing will control (including, without limitation, the calculation of the rebate amount for the energy saving products listed in this rebate application) if there is any conflict between the terms and conditions applicable participation in Energy Efficiency Business Rebates and On-Bill Financing. In no case will the utility pay more than 100% of the actual purchase price of the items be rebated. Energy Efficiency Busine ss Rebates has a limited budget. If no reservation has been made, rebate applications will be accepted on a first-come, first-served basis, until allocated funds are spent, or by December 1, 2010, whichever comes first. I agree that I have not received rebates, incentives or services for the same measure(s) from another utility, state, or local program funded by the Public Purpose Program (PPP) or Public Goods Charge (PGC). Lamp Disposal: It is unlawful for California cusmers dispose of any lamps containing mercury in the regular weekly pick-up conducted for homes and businesses. 4 SECTION 8, 9, & 10 HOW TO APPLY, ELIGIBILITY, & REQUIREMENTS 2 Please read through these sections completely as they contain important information and requirements regarding your Energy Efficiency including information on the new Benchmarking requirement Energy-Efficiency SECTION 8 HOW TO APPLY Reservations are not required. However, ensure funding availability, you must reserve funds before you purchase and install equipment. Call (800) reserve your funds. Reservations are held for days. Please see the Energy Efficiency Business Rebates Cusmer Handbook for further details Benchmarking is is required for all all commercial business cusmers who participate in in Energy Efficiency programs. In In order meet this requirement visit or or call our Energy Information Center at at For non-lighting products, use the Rebate Product Information table calculate your rebate by by multiplying the quantity purchased (Column A) A) by by the rebate/unit (Column B) B) and record the amount in in Column C. NOTE: The following guidelines must be be adhered in in order participate in in the program: Applications with any sign of of liquid paper ( white out ) used change information will be be returned the cusmer or or contracr. Pre-made labels with contracr or or cusmer information (name, address, etc.) are not accepted on on rebate applications. However, a a contracr stamp is is acceptable, as as long as as there is is no no possibility of of covering up up existing information beneath the stamp. If If working with a a facring company, the contracr s name must appear as as the payee in in the Payment Information section of of the application. However, if if the contracr and cusmer agree that the rebate can be be mailed directly the facring company, the facring company address can be be listed under the contracr name. See example below. A rebate check for qualified measures is is generally mailed days after SDG&E receives and approves a a completed application including all all required documentation, and verifies installation. Incomplete or or incorrect applications cannot be be processed and notification will be be mailed out. SECTION 9 ELIGIBILITY Cusmers eligible for the SDG&E Energy Efficiency Business Rebates Program are defined as as nonresidential commercial, industrial and agricultural cusmers. The tal rebate limit is is $50,000 per service account, per fuel, per calendar year. Cusmers with multiple service accounts for multiple projects, qualify provided that each individual service accounts or or chain account cusmers rebates or or site limit $50,000 is is not exceeded. SECTION 10 REQUIREMENTS Rebate items must be be installed and operational at at the San Diego Gas & Electric service account location listed on on the rebate application. All All invoices or or receipts must be be dated between January 1, 1, 2010 and December 1, All All rebate items must be be purchased and installed prior submitting a a rebate form. Cusmers cannot apply other public goods charge rebate or or incentives for the same measure. Lamps purchased at at retail outlets do do not qualify for a a rebate if if the price has been reduced through a a utility buydown program. Equipment must meet the requirements and terms and conditions contained in in this rebate application. All All equipment must be be new. Used or or rebuilt equipment is is not eligible for rebate. Fuel switching and new construction do do not qualify. If If you have questions, call (800) confirm eligibility and reserve funds before purchasing and installing products. San Diego Gas & Electric reserves the right deny payment for products that do do not meet the requirements and terms and conditions and verify installation through onsite inspection.
