A total of 55 locals, school districts, or colleges submitted forms in varying states of completion. For districts for which there are multiple insurance plans or providers, or from which there is more than one submitted survey with differing information, multiple lines of data are shown. For each major category of the questionnaire on benefits, the responses are listed alphabetically by school district name. te page 4 listing an explanation of column headings and abbreviations.
