2 Cardiac Rhythm Disease Management Products Medicare C-Code List for Hospital Outpatient Device Reporting Updated January 1, 2012 Procedure to device code edits apply to a subset of device-dependent APCs and their corresponding C-Codes (see Table 1). Device (C-Codes) to procedure edits were implemented by the Centers for Medicare & Medicaid Services (CMS) to ensure that both the procedure and the device components were appropriately billed. These edits are shown in Table 2. For rate setting purposes of device-dependent APCs (Ambulatory Payment Classification), CMS eliminates claims that include a token charge, a device furnished without cost, with a full credit, or partial credit.
