1 DODGE CARAVAN KIDS TEAM SPONSORSHIP PROGRAM IMPORTANT INFORMATION PACKAGE HOCKEY SEASON DODGE CARAVAN KIDS IS CELEBRATING ITS 10TH ANNIVERSARY!! Thank you for joining Dodge Caravan Kids for our 10th anniversary season! Our first nine years have been a success because of you and we are excited to share this milestone and another year of great hockey memories with all of the hard working association staff, coaches and volunteers that form the backbone of minor hockey in Canada. NEW ONLINE REGISTRATION PROCESS EFFECTIVE FOR THE SEASON HOW TO REGISTER? The new Dodge Caravan Kids Online Registration Process will be accessible between September 14, 2015 and December 1, 2015 at DodgeCaravanKids.ca. IMPORTANT NOTES The NEW Dodge Caravan Kids Registration Process is a TWO STEP online process. Please review the steps below and ensure you COMPLETE BOTH STEPS to fully complete your registration. The number of team sponsorships in your region are set at the Member Partner level and are available on a FIRST COME, FIRST SERVED basis. To reserve your sponsorship, we encourage you to complete Step 1 of the online registration process as soon as possible. You will have 45 days OR until December 1, 2015 (whichever comes first) to complete Step 2 of the process. After 45 days or December 1, 2015, your application will expire. Dodge Caravan Kids will make every effort to re-allocate any available sponsorships within the program to meet demand. If there are no available sponsorships in your region, visit the Dodge Caravan Kids Online Registration website (DodgeCaravanKids.ca) between November 17 and 27, 2015 to see if any new sponsorships have opened up in your region.

2 STEP 1: APPLY FOR TEAM SPONSORSHIP Sign your teams up for Team Sponsorship by selecting your Member Partner and Association from the dropdown menu and completing the online application form (applications are First Come, First Served ) START HERE COMPLETE TEAM APPLICATION HERE Upon submitting your application form and completing Step 1 of the process, you will be asked to download your Team Photo Release Form and order your Dodge Caravan Kids jersey patches You will also have the chance to enter the Dodge Caravan Kids Score & Win Contest to win $5,000 for your Minor Hockey Association and a 2016 Dodge Grand Caravan for yourself share this contest with the coaches and parents on your sponsored teams to give your Association more chances to win Your application will be processed by us within 3-5 days Upon being processed, an will be sent to the address you provided in your registration application with your unique PIN and URL to complete Step 2 of the registration process (see below)

3 Example of Successful Step 1 IF THERE ARE NO AVAILABLE SPONSORSHIPS IN YOUR REGION, PLEASE CHECK BACK BETWEEN NOVEMBER 17 TO 27 If there are no available sponsorships in your region, and you received a Check Back message, please re-visit the Dodge Caravan Kids Online Registration Website (Dodge CaravanKids.ca) between November 17 and 27, Sponsorships may be re-allocated to meet demand.

4 STEP 2: COMPLETE REGISTRATION BY SUBMITTING TEAM PHOTO & PHOTO RELEASE FORM Click the Step 2 URL in the you received (after completing Step 1) OR visit the Dodge Caravan Kids Online Registration Website (DodgeCaravanKids.ca) and click Step 2 Input your unique PIN (your PIN can be found in the you received after completing Step 1) Can t find your with your unique URL & PIN? info@dodgecaravankids.ca with your full name and full team name to have your PIN re-generated and sent to you Upload your team photo (patches must be clearly visible) Upload your team photo release form (ALL parent/guardian signatures for the children shown in the team photo must be included). If your photo release form is incomplete, your sponsorship application will not be approved. Example of Successful Step 2

5 ERRORS WITH UPLOADING TEAM PHOTO OR TEAM PHOTO RELEASE FORM Any errors will be clearly displayed on the photo registration page. Potential errors include: Incorrect PIN (please check and re-enter PIN) Incorrect (please check and re-enter ) Team Photo File Size or Format (either too large or incorrect file type - please ensure you have attached in JPG or PNG format) Team Photo Release File Size or Format (either too large or incorrect file type - please ensure you have attached in JPG, PDF, TIFF or PNG format) EXPIRATION DATE & INCOMPLETE SPONSORSHIP APPLICATIONS Incomplete applications for sponsorship will expire in 45 days or on December 1, 2015, whichever comes first. If your application expires, you will need to return to the Online Registration Website (DodgeCaravanKids.ca), check for available sponsorships in your region and re-register your team. 14

6 ABOUT DODGE CARAVAN KIDS The Dodge Caravan Kids Program is a joint effort between participating minor hockey associations and Chrysler, Dodge, Jeep, Ram retailers that provides up to $500 in funding and access to exclusive hockey benefits to novicelevel hockey teams across Canada. The goal of the Dodge Caravan Kids Program is to help enrich the game of hockey for participating coaches, players and their families and represents one of FCA Canada s key community efforts. WHO IS ELIGIBLE? Canadian Novice hockey teams (NOTE: Atom Division teams are ONLY eligible in Atlantic Canada) and associations located within 150 kilometres of a Chrysler, Jeep, Dodge, Ram retailer. Visit to locate your closest retailer. o Exemptions can be made upon request (i.e. rural areas where car dealerships are not local) Teams and associations are eligible to participate regardless of other corporate sponsorships they may have, unless those partnerships are with a competing automaker or their associated retailer. PROGRAM BENEFITS Each team enrolled in the Dodge Caravan Kids Program will receive: Up to $500 in funding. Please contact your Member Partner/Minor Hockey Association for further information on funding policies. The opportunity to enter the Dodge Caravan Kids Photo Contest to win up to $500 in extra funding. Team kits including an exclusive Dodge Caravan Kids gift for all players and an important information booklet for all parents/guardians The opportunity to enter our Score & Win contest for a chance to win a 2016 Dodge Grand Caravan and $5,000 for your local Minor Hockey Association. TEAM SPONSORSHIP BRANDING REQUIREMENTS Any team that joins the Dodge Caravan Kids Program must acknowledge their participation through the following: Adopt the Dodge or Dodge Caravan Kids name in their team name. (E.g., Sunnyside Dodge Caravan Kids, Dodge Chargers, Dodge Caravan Kids Team Red, Dodge Caravan Kids Green Chargers, etc. Team jerseys can be any colour but MUST feature the Dodge Caravan Kids sponsor patch or screened logo on either: o Back of jersey OR Front of jersey Name bars and patches are available through Artwork files are available by contacting daniel.clement@omha.net FRONT OR BACK OF JERSEY LARGE PATCH
