4 2 About Us Surrey Place Centre provides specialized clinical services that are responsive to individual need and promote health and well-being in the Toronto region. We help children and adults living with developmental disabilities, autism spectrum disorder and visual impairments reach their full potential. We offer a variety of groups and workshops for clients, families and caregivers, as well as extensive education and consultation services to community agencies. Our comprehensive programs and services range from assessment, diagnosis, and one-on-one treatment, to family counselling and group support, and it is all provided by a broad network of clinicians and professionals. 5 Community Programs Developmental Services Ontario Toronto Region Community Network of Specialized Care Toronto Network of Specialized Care MMW-Videconferencing Program Access and Referrals At Surrey Place Centre, we want to make it easy for people to access our services. Anyone may refer an individual to Surrey Place Centre for assessment or treatment services. This includes: An individual who has or may have a developmental disability Family Members Caregivers or Guardians Physicians and other Health Care Providers School Board Members Workers at other Community Agencies You must call us to be assessed for a developmental disability. If you have been previously diagnosed with a developmental disability, a call will be required to gain access to our specialized groups and services. Please call Surrey Place Centre is affiliated with the University of Toronto and other academic institutions, and is a teaching site for students in a variety of health care professions. Surrey Place is accredited with Accreditation Canada with Exemplary Standing. About Us 3
6 4 Infancy and Early Childhood Program Surrey Place Centre s Infancy and Early Childhood Program provides services for infants and children from birth to age six who have or are suspected of having a developmental disability. The program also supports their families and caregivers. We use a family-centered, interdisciplinary team approach. Focus is placed on working with parents and caregivers to develop a service plan that addresses their child and family needs. The child s progress is monitored regularly by a primary contact person for the family. We support children and their families through various transitions including from home to childcare and to school. Our clinicians work closely with parents and other professionals in establishing a transition plan that will support the ongoing development of the child. Description of Services Provided: A comprehensive family-centred approach to service delivery Specific education, assessment, consultation and treatment/ intervention services through an interdisciplinary team approach Information on community resources, assistance in service planning and referral Consultation and training to community agencies and service providers
7 Supervised training for students in several disciplines through practicum, internship and residency placements Program evaluation and research projects Services Provided May Include: Audiology Behaviour Therapy Counselling Developmental Therapy Medicine Nursing Occupational Therapy Psychology Service Coordination Speech-Language Pathology Specialized Programs and Services: In addition to our interdisciplinary services, Surrey Place Centre s Infancy and Early Childhood Program includes city-wide services for three designated provincial programs: Infant Development Program Surrey Place Centre is one of five agencies that operate an Infant Development Program in Toronto. Toronto Preschool Speech and Language Services Surrey Place Centre is one of the citywide agencies contracted to provide speech and language services to children with specific needs. Infant Hearing Program Surrey Place Centre is one of the designated sites for the Infant Hearing Program, which provides audiology services for infants referred for assessment and intervention (treatment) as needed. 8 6 Blind-Low Vision Program Surrey Place Centre s Blind Low Vision Program provides early intervention support to children birth to age 6 with a diagnosed visual impairment. This family-centred service provides supports to children and their families in natural environments including the home and early learning settings. The goal of the service is to mitigate the risks of the visual impairment on the child s development. Intervention provided through the Blind Low Vision Program will assist families and caregivers in providing opportunities that support the development of all the senses for optimized learning.
9 Surrey Place Centre s Blind Low Vision Program serves children and their families living in Toronto, Halton, Peel, York and Durham. Description of Service Provided: Assessment of sensory functioning to determine the child s sensory functioning to identify modifications for access to learning Evaluation of the child s environments to develop recommendations for optimizing the child s learning opportunities Support to families and caregivers to promote healthy attachment and social interactions Guide parents in understanding their child s sensory functioning and required modifications to the environment to support learning Collaboration with the child s professional team to promote strategies to support sensory functioning Consultation and training to community agencies and service providers Promotion of emergent literacy skills that include access of materials through print, braille and adapted auditory materials Facilitate transitions to early learning settings and school Referrals for Service Contact: Referrals for service must be received through Regional Blind Low Vision Lead Agencies: Families living in Toronto - Toronto Public Health Families living in Halton or Peel - ErinoakKids Families in York and Durham - Markham Stouffville Hospital Lead agencies also provide access to a Family Support Workers funded under the Blind Low Vision Programs. 11 Surrey Place Centre s Children and Youth Program provides interdisciplinary assessment, treatment and consultation to children and youth aged 6 to 18 who have or are suspected of having a developmental disability. The program also supports their families, caregivers and other service providers. The Children and Youth Program offers assessment, treatment and support to help school-age children cope with the challenges of daily living, promote their independence and enhance their overall health and well being. The Children and Youth Program develops treatment plans in consultation with clients, caregivers and community partners, involving them in the treatment plans and sessions. 13 The Adult Program at Surrey Place Centre provides assessment, treatment, consultation and support to people 18 years of age and over who have a developmental disability. The program also supports their families, caregivers and other service providers. Our goals are to help clients to manage the challenges of daily living, promote their independence and enhance the overall quality of their lives. The Adult Program offers a unique set of programs to adult clients, including interdisciplinary assessment, treatment, primary health care, mental health services and parenting support. 14 12 Fetal Alcohol Spectrum Disorders Adult Diagnostic Clinic Fetal Alcohol Spectrum Disorder Adult Diagnostic Clinic provides a diagnostic clinic specifically for adults with suspected Fetal Alcohol Spectrum Disorder (FASD) and developmental disability. This is the first FASD Clinic specifically for adults in Ontario. FASD describes the type of deficits that can occur when an individual is exposed to alcohol before birth. Effects can include physical, behavioural, and intellectual problems. In the FASD Adult Diagnostic Clinic, we provide assessment and diagnostic services to individuals 18 years of age and older living in the city of Toronto. Recognition and diagnosis of FASD is important because it can lead to: A better understanding of the individual and his or her needs Access to additional supports in the community More effective interventions and services tailored specifically to the individual s strengths and weaknesses. This service is made up of an interdisciplinary team of clinicians who can also offer counseling, behavior therapy and service coordination when appropriate. 15 13 Parenting Enhancement Program The Parenting Enhancement Program (PEP) at Surrey Place Centre provides parent skill training and support to parents with developmentally disabilities. We strive to reduce the risk of child neglect and abuse and to promote the social, emotional, cognitive and physical development of the children we work with. Our purpose is to improve parenting abilities through a multifaceted teaching method which emphasizes individualized, homebased child care instruction: education in child development, and advocating in support of the family. Training may be provided in basic infant and child care skills, home safety, first aid, nutrition and positive parent child relationships. We recognize that the needs of parents and families extend beyond parenting skills. Therefore, we often provide case management by making appropriate referrals and acting as a liaison between the families we service and community agencies. Program staff are available for workshops, presentations, consultations and one to one staff training. 16 14 Augmentative Communication and Writing Aids Program The Augmentative Communication and Writing Aids Program (ACWA) at Surrey Place Centre is a specialized Augmentative and Alternative Communication (AAC) Clinic providing service to people with a developmental disability. ACWA was designated as a General Level Clinic by the Assistive Devices Program of the Ministry of Health and Long-term Care in March The ACWA Team is comprised of part-time Speech-Language Pathologists, Occupational Therapists and Communicative Disorders Assistants who work together to assess and train clients and caregivers to use Speech-Generating Devices and written communication aids. Information on funding for the purchase or lease of the devices is also provided. Services at Surrey Place Centre: Assessment of communication needs and ability to use AAC Assessment of ability to access a faceto-face or written communication aid Development of an AAC system with the client and caregivers Training for clients and caregivers regarding how to use AAC systems Training for communication partners to support communication systems Providing information on costs for purchasing or leasing of communication aids Providing information on funding resources for communication aids
17 Acting as a resource to Surrey Place Centre and community Speech Language Pathologist and Occupational Therapist Providing education about AAC as needed to the greater Surrey Place Centre community Becoming an ACWA Client To receive services at ACWA, all individuals must go through the Surrey Place Centre Intake Process to become a client at Surrey Place Centre. The individual s AAC needs will be screened to determine whether service should be through an Individual Authorizer or a General Level Clinic. Clients and caregivers will be notified regarding the level of AAC service that is appropriate. All clients will be placed on a wait list and will be notified when services are ready to commence. How to Refer for AAC Services at Surrey Place Centre All new referrals must go through Surrey Place Centre s regular intake process. Clients who are new to Surrey Place Centre or who have not had services from Surrey Place Centre in over a year should call and you will be directed to a service coordinator to discuss your needs. If you are currently a Surrey Place Centre client you should contact your primary clinician to discuss your communication concerns and needs. 18 16 Toronto Partnership for Autism Services (TPAS) TPAS is a community based service that helps children with autism, and their families, through Intensive Behavioural Intervention (IBI) and related services. Families may choose either the Direct Service Option delivered in partnership with Surrey Place Centre, Adventure Place, Aisling Discoveries Child and Family Centre, The Etobicoke Children s Centre, and Geneva Centre for Autism, or the Direct Funding Option, delivered by an approved private provider. IBI is: a scientifically proven treatment provided for a minimum of 20 hours/ week delivered through one-to-one and
19 small group instruction requires frequent, direct measurement of child performance/ progress designed to target a broad range of developmental areas and improve key learning skills e.g. cognitive, language, social The goal of IBI is to increase the rate of learning. Children have different outcomes with IBI however; treatment research suggests that up to 40-50% of young children may make substantial progress over a 1 to 2 year period. In order to do so, intervention should be provided intensively during children s best learning periods. Applying for TPAS Parents with children who: live in Toronto and have a confirmed diagnosis of Autistic Disorder or fall toward the severe end of the autism spectrum. Getting a Diagnosis for My Child A physician or a registered psychologist can diagnose autism. Eligibility A screening appointment is conducted to determine eligibility and suitability for IBI. Parent Education and Support Services Parent Education and Support Services begin immediately for all eligible children. Services were designed to build a foundation of knowledge to support child development and to link families with resources in their community. Transition Services IBI is not intended to be a long term replacement for school. TPAS provides supports to children and families that help to make the transition from IBI to new environments as smooth as possible. This includes linking to Connections for Students, a collaborative program with Surrey Place Centre and the public school boards. Toronto Partnership for Autism Services (TPAS) 17
20 18 Toronto Autism ABA Services Toronto Autism ABA Services are funded by the Ministry of Children and Youth Services and are free to children and youth with a confirmed Autism Spectrum diagnosis including Autism/Autistic Disorder, Pervasive Developmental Disorder-Not Otherwise Specified (PDD-NOS), and Asperger s Disorder up to their 18th birthday. Treatment services are delivered in partnership with Surrey Place Centre, Kerry s Place Autism Services, Adventure Place, Aisling Discoveries Child and Family Centre and The Etobicoke Children s Centre. Our goals are to assist children and youth to learn important skills through the use of Applied Behaviour Analysis (ABA) strategies and promote success at home and in the community. Parent involvement in treatment is important to achieving these goals. Toronto Autism ABA Services Will: Help children and youth with ASD develop skills in the areas of:»» Communication: understanding and using language for social purposes»» Social/Interpersonal: establishing and maintaining relationships with other people»» Daily Living Skills: including personal care activities, personal safety etc.»» Emotional/Behaviour Regulation: including preparation for group, organizational and self-calming
21 Use positive and systematic approaches to teaching practical skills within natural routines, building upon what motivates and interests the child or youth Be offered in treatment blocks up to three months in length with weekly sessions lasting two to four hours Typically be delivered through group sessions with individual coaching as appropriate Provide education to parents/ caregivers on how to apply ABA strategies which will help their children learn, maintain and generalize skills Eligibility Criteria: Children and youth up to their 18th birthday Must have a confirmed Autism Spectrum diagnosis including Autism/Autistic Disorder, Pervasive Developmental Disorder-Not Otherwise Specified (PDD-NOS), and Asperger s Disorder Must be living in the Toronto Region Not concurrently receiving any other ABA based services through this initiative Treatment Programs: Children are placed in groups following an assessment of strengths and needs and are clustered by age and gender as appropriate. Treatment groups may change over time and will be offered on varying schedules in each quadrant of Toronto based on the needs of the children and youth who are waiting for service. 22 20 School Support Program The School Support Program (SSP) at Surrey Place Centre offers services to enhance existing supports in publicly funded school boards and authorities. Our client is the school board and its personnel. The program does not provide direct services to students with an ASD or their families (with the exception of the Connections for Students Service). Our objectives are to: Enhance the knowledge and skills of school board staff with respect to: The characteristics of students with an autism spectrum disorder (ASD) The principles and applications of applied behaviour analysis (ABA) Other evidence based teaching strategies
23 Promote linkages between schools, school boards and community services. Our core services, as mandated, are consultation, training, resource development and distribution as identified by school boards and authorities. Consultation: This may include interaction with educators, school board level planning and linkages to other community supports. Training: The SSP may provide training opportunities for school board personnel. Presentations are driven by school requests and may include topics such as Introduction to ASD, Visual Strategies and Introduction to ABA. Resource Development and Distribution: The SSP may assist school board personnel in identifying, developing and sharing resource materials relevant to supporting students with an ASD. Connections for Students:» The Ministry of Children and Youth Services and the Ministry of Education have developed Connections for Students (CFS), a collaborative approach to implementing a seamless transition for children exiting their publicly funded Intensive Behavioural Intervention (IBI) to full time school entry. This service begins approximately 6 months before IBI ends and a child enters school on a full time basis with the formation of a school based transition team, and will continue for up to six months after. 24 22 Developmental Services Ontario (DSO) Developmental Services Ontario (DSO) helps adults with developmental disabilities connect to services and supports in their communities. There are nine agencies across Ontario who will help adults with developmental disabilities and their families and/or caregivers to navigate the services that are available. These nine agencies are funded by the Ontario Ministry of Community and Social Services. Goal of Developmental Services Ontario (DSO): We help people with developmental disabilities to become more independent, to make their own choices and to get involved in their communities. Wherever you live in Ontario, we can help you or someone you care for connect to the following services and supports: residential supports caregiver respite community participation supports (like recreation, volunteering, employment, or in-home supports) professional and specialized services person-directed planning and other supports to help people with developmental disabilities become more involved in their communities!

26 24 Central Region Community Network of Specialized Care (CNSC) The Ministry of Community and Social Services has established four Community Networks of Specialized Care (CNSC) across Ontario Central, East, North, and South. These networks enhance service for adults with a developmental disability who also require specialized care for co-existing mental health and/ or behavioural issues. The Central Region CNSC is comprised of three regions with an associated lead agency: Central East (Community Living Huronia); Central West (Central West Specialized Developmental Services), and Toronto (Surrey Place Centre). Each region has a network Coordinator and Health Care Facilitator. Activities across Central Region are facilitated by Coordinators for videoconferencing and education/research. Aside from being the lead for the Central Network, Surrey Place Centre hosts the Central Region education and research functions. CNSC education events help to increase capacity and enhance the skill level of caregivers. The CNSC also facilitates the development and maintenance of mutually beneficial relationships within the academic research community and assists researchers with effectively communicating the outcomes of their work to the practitioners in the field. 27 25 Toronto Network Of Specialized Care (TNSC) The Toronto Network of Specialized Care (TNSC) is part of the Central Region Network of Specialized Care. There are three other regional Community Networks (CNSC s) in Ontario, where adults who have a developmental/ intellectual disability, mental health issues and/or challenging behaviours are the focus. Support is provided through cross-sector collaboration and linkages and increasing community capacity through education, training and research. The Toronto Network offers the following specialized services in partnerships with member organizations: Case management Crisis response Specialized clinical supports Day and residential treatment Respite Inpatient/outpatient hospital care The TNSC offers a monthly Clinical Conference to assist caregivers who are experiencing challenges in service provision to adults 18+ who have a developmental disability, mental health issues and/or challenging behaviours. Through this process system barriers and service gaps are identified. The TNSC is connected to Developmental Services Ontario Toronto Region and fulfills the Ministry of Community and Social Services mandated Service Resolution function for Toronto. 28 26 Mashkikiiwininiwag Mazinaatesijigan Wichiiwewin MMW-Clinical Videoconferencing Program The program provides specialized clinical services and resources for adults living with developmental disabilities. It also provides a unique opportunity for individuals, families, caregivers, and professionals to share resources and information over significant geographic distances. Videoconferencing technology allows participants to meet via high speed, secure telecommunication links, in real time, face to face. Surrey Place Centre, in collaboration with its partners, provides this service to communities situated in Northwestern Ontario. Local agencies, professionals, and communities have access to specialized (therapeutic) services and
29 educational opportunities, that may otherwise not be available due to cost and other factors associated with the distances from major centers. Services Provided May Include: Behaviour Consultation Counselling Diagnosis and Assessment Educational Opportunities Psychiatric Consultation Psychological Assessment and Consultation The service team includes Psychiatrists, Psychologists, Intake- Service Coordinators, and Behaviour Therapists. Who We Serve: This program serves adults 18 years of age and older and their families living with developmental disabilities who: may have mental health issues, may have challenging behaviours, require assessment for eligibility to services are difficult to diagnose, and / or need help with their medication. By providing services where and when people need them, the program enables adults with developmental disabilities to continue to live in their homes in the community. Education and Training Benefits: The MMW-Clinical Videoconferencing Program offers valuable education and training opportunities, including: A recognized, College-affiliated, certificate program in Dual Diagnosis; Training seminars and learning opportunities on topics related to caring for and working with people living with developmental disabilities and mental health issues. 31 The primary objective of the Research and Evaluation Unit at Surrey Place Centre is to promote best practice in the field of developmental disabilities. Our activities are driven by key clinical and service issues. We provide guidance and support to staff and students and engage with community partners to carry out and build capacity for research and evaluation. Research We participate in large collaborative projects with community partners and smaller locally developed projects involving Surrey Place Centre staff and clients. Our Research Ethics Board reviews research proposals and monitors all studies involving Surrey Place Centre clients to ensure their compliance with the Tri-Council Policy Statement: Ethical Conduct for Research Involving Humans. Evaluation Our evaluation work focuses on internal services and programs. We work with clinicians to identify and/ or develop evaluation tools that are clinically relevant. We also support evaluation activities for communitybased partners.
