3 Introduction PE 4910 Internship in Exercise Science is designed to be off-campus supervised educational work experience at an approved worksite offering programs and experiences in fitness development and/or health promotion. The internship allows the student to develop health promotion-related job skills under the guidance of trained and established professionals. The internship experience affords the students an opportunity to apply theoretical knowledge and skills in a practical situation while gaining valuable pre-professional in-service training. The students participating in this internship will spend a minimum of 20 hours per week for 15 weeks at the Worksite. Exceptions due to Worksite schedules must be approved by the internship coordinator. Activities that would be most appropriate for the student to experience would include, but not be limited to, fitness testing, exercise prescription, exercise supervision, program planning, daily scheduling, fiscal management, public relations, advertising, development of workshops, participation in workshops, smoking cessation programs and nutrition management programs. The student will meet formally a minimum of four times with the Worksite Supervisor and a minimum of six times with the University Supervisor for local placements. In addition to submitting weekly activity reports, the students will be evaluated periodically by both the University Supervisor and the Worksite Supervisor. The student's grade will be determined by a composite of all evaluations. This manual has been developed in order to provide the student, Worksite Supervisor and University Supervisor with the policies, procedures, roles and responsibilities of those participating in the internship. For placements outside of the Omaha Metropolitan area the student must meet formally with the University Supervisor prior to and following the internship. During the internship, the student and University Supervisor will maintain communication through blackboard, , and/or phone.
4 PE 4910 INTERNSHIP IN EXERCISE SCIENCE 6 Credit Hours UNDERGRADUATE INTERNSHIP MANUAL COURSE SYLLABUS 1.0 Course Description 1.1 This course is an off-campus, supervised, educational work experience of at least 300 clock hours at an approved Worksite offering programs and experiences in fitness development or health promotion. 1.2 This course is intended for Exercise Science graduate students. 1.3 The student must be in his or her last year of course work and receive permission of the internship Coordinator. 1.4 None 2.0 Objectives The objectives of the internship are to: 2.1 Provide the student with an opportunity to work in an off-campus program in order to experience actual work conditions in a fitness or health promotion program under supervision of professionals outside the university environment as well as a university faculty member. 2.2 Refine and gain confidence in the skills developed in the Exercise Science program. 2.3 Provide the opportunity to learn to work effectively with professional colleagues. 2.4 Allow the student to assess his or her personal strengths and limitations as an exercise leader in the work environment. 2.5 Gain insights regarding all aspects of managing a fitness development or health promotion program operated outside the university system. 3.0 Content and Organization 3.1 Topics vary with internship sites. The intern will meet six times with the University Supervisor and a minimum of four times with the Worksite Supervisor. 5 12 Worksite Worksite Supervisor 12 UNO (seminar) University Supervisor 15 Worksite Worksite Supervisor 16 UNO (Exit Interview) University Supervisor 4.0 Teaching Methodology 4.1 This course is designed to give the student practical experience while under the supervision of a worksite professional and a university faculty member. 4.2 The student will spend at least 20 hours per week in one semester at an agency/business which sponsors a fitness development or health promotion program. Activities may include, but are not limited to, fitness testing, exercise prescription, exercise supervision, program planning, personnel management, fiscal management, communication, participation in and conducting workshops, and other health promotion programs. 4.3 The following statement will be included in the course outline provided to each student registered for this course: Accommodations are provided for students who are registered with UNO Disability Services and make their requests sufficiently in advance. The Worksite Supervisor must submit a post 1st week, post 7th week and post 15th week evaluation of the intern's performance to the University Supervisor. The intern must submit weekly activity reports to the University Supervisor. The intern must complete an exit interview with the University Supervisor. 5.2 The University Supervisor assigns all grades with information and recommendation of the Worksite Supervisor. The final grade is determined from a composite of evaluations by the University Supervisor and the Worksite supervisor as follows: Points Possible: Attendance and participation at seminars 30 Supervisor's worksite evaluation 20 Weekly activity reports evaluated by the: University Supervisor 30 University Supervisor's exit interview 20
6 Post 1st week evaluation by the Worksite: Supervisor 10 Post 7th week evaluation by the Worksite: Supervisor 20 Post 15th week evaluation by the Worksite: Supervisor 70 TOTAL: 200 The final letter grade will be determined on the basis of total points for the semester. 7 PROCEDURES TO RECEIVE APPROVAL AS AN INTERNSHIP SITE To receive approval as an internship site for PE 4910 the Agency/Business must: 1. Compose and submit a letter (addressed to the Exercise Science internship coordinator) indicating interest in and a commitment to interns in the Exercise Science emphasis of the Physical Education Program at the University of Nebraska at Omaha. 2. Prepare and submit a statement of policy, goals and objectives of the Agency/Business. Describe briefly the personnel, including their length of experience, education, training and job description. In addition, give a brief description of the facilities, programs and budget of the Agency/Business. 3. Develop and submit a statement indicating why the Agency/Business is a viable and dynamic educational setting for training professionals. In essence, to present the unique educational opportunities afforded a student completing his or her internship with the Agency/Business. 4. Provide a statement regarding liability to inform the student of his or her status concerning Workman's Compensation, liability and health insurance. 5. Provide a vita of the individual supervising the intern. 6. Provide a description of the intern's role and responsibility. After receipt of all the above information, a contract will be prepared and sent to the Agency/Business requesting an intern. After the signed contract is returned to the Exercise Science internship coordinator, the Agency/Business is approved to receive an intern. An Agency/Business must generally request an intern at least one full semester prior to placement since fall placements are determined in March and spring placements are determined in October.
8 INTERNSHIP ELIGIBILITY REQUIREMENTS Basic Requirements to be Eligible for PE 4910 Internship 1. Prerequisite courses that must be completed prior to enrolling in PE 4910 include: a. HED 3030 First Aid b. PE 2400 and PE 2500 Human Physiology and Anatomy I & II or BIOL 2740 and BIOL 2840 Human Physiology and Anatomy I & II c. PE 4630 Biomechanics d. PE 4940 Exercise Physiology e. PE 4010 Lab Methods in Exercise Science f. PE 4800 Exercise Leader Practicum g. PE 4900 Practicum University Fitness Program (Only required if in a catalog prior to ) 2. An overall GPA of Completion of the Internship Application and Student Information Form by the stated deadline. Spring Semester deadline is October 15. Fall Semester Deadline is March Current CPR certification. 5. Approval of the Exercise Science Internship Coordinator.
9 PROCEDURES FOR INTERNSHIP PLACEMENT 1. Student must research places of possible internship opportunities. The internship coordinator will provide a master list of internship sites to assist the student for options. 2. Contact possible internship sites to ask about types of internships available, application procedures, learn of intern s duties and responsibilities and obtain internship site supervisors contact information. 3. Student must establish a list of top three preferred internship sites. A meeting can be scheduled with the internship coordinator if the student needs guidance in internship site options. 4. The student must complete a PE 4910 Internship Application. 5. Prior to the deadline for filing the PE 4910 Internship Application (October 15th for Spring Semester, March 15th for Fall Semester), the student must meet with the Exercise Science Internship Coordinator to verify the requirements for PE 4910 have been met and to check the completeness of all forms. 6. The student will be notified by mail of the tentative internship. 7. The Student must make an appointment for an interview with the Worksite Supervisor at the tentative internship site. 8. Both student and Worksite Supervisor must notify the Exercise Science internship coordinator of acceptance of placement by December 1st for Spring Semester, May 1st for Fall Semester. 9. The Exercise Science internship coordinator will notify both student and Worksite Supervisor of final approval for internship placement.
10 STUDENT RESPONSIBILITIES To the Agency/Business: 1. To conform to the rules and regulations of the Agency/Business. 2. To complete assignments and responsibilities as outlined in the Intern-Agency Agreement. 3. To provide 20 hours of service per week for 15 weeks in the area of' fitness development or health promotion. Exceptions must be approved by the Exercise Science Internship Coordinator. To the University: 1. To submit an Intern-Agency Agreement after consulting with the Agency/Business Supervisor during the first week of service. The agreement must detail the responsibilities of the student and Agency/ Business. 2. To submit an Intern-University Agreement. 3. To submit an Intern Assignment and Class Schedule Form. 4. To submit, thereafter, to the University Supervisor the Weekly Activity reports. 5. To submit the Self Evaluation and Final Report during the 15th week of service. 6. To submit the Student Evaluation of Practicum Site during the final week of service. 7. To submit the Student Evaluation of' Site Supervisor during the final week of service. 8. To represent the Physical Education Program, and School of' HPER, as a University student in adherence to the current Student Code of Ethics and Appeal Procedures. 9. To complete assignments and responsibilities as outlined in the Intern-University Agreement.
11 To the Student: EXERCISE SCIENCE INTERNSHIP COORDINATOR AND UNIVERSITY SUPERVISOR RESPONSIBILITIES 1. To hold an individual conference with each student during the semester prior to his or her affiliation. 2. To represent the University in all official arrangements with cooperating Agencies/Businesses. 3. To become familiar with all Worksite Supervisors, 4. To supervise arrangements for and give final approval of all internship assignments. 5. To review the Intern-Agency Agreement with the student and to sign the form indicating approval. 6. To review and evaluate Weekly Activity reports and discuss them with the intern. 7. To make at least one on-site visit during the semester to observe the student (local placement only). 8. To assign a final grade to the student by combining the University and Agency/Business Supervisors' evaluations and by considering all other pertinent information. 9. To conduct on-campus seminars for all Local interns. To the Agency/Business: 1. To approve the internship. 2. To make the final decision on placement of the intern with an Agency/Business. 3. To provide evaluation forms for all evaluations. 4. To maintain open communication at all times with the Agency/Business concerning its cooperation and supervision of the intern. 5. To make periodic on-site visits for conferences about the University program in general and the work with the student in particular.
12 AGENCY/BUSINESS RESPONSIBILITIES To the University: 1. To receive approval from the Exercise Science Internship Coordinator as a PE 4910 Internship in Exercise Science Worksite. 2. To cooperate in the exchange of information on the Intern. 3. To meet with the University Supervisor periodically to discuss the student's progress. 4. To provide the University Supervisor with a formal evaluation of the student's performance post 1st week, Mid-Session and one week prior to the final conference. 5. To provide the University Supervisor with an informal evaluation of the student's performance between the Mid-Session and final formal evaluations. 6. To discuss curriculum offerings as related to the student's weaknesses and suggest changes. 7. To notify the University Supervisor in the event of a serious problem with the Intern. To the Student: 1. To confer with the student to determine responsibilities during the experience and to present an overview of the Agency's/ Business's purposes, policies, administration, program and plant. 2. To inform the student of all rules and regulations to which he or she must conform. 3. To schedule periodic conferences (e.g., weekly or biweekly) to aid the student and give him or her an idea of his or her progress. 4. To keep continuous records of the student's work and progress. 5. To develop a working agreement with the Intern (Intern-Agency Agreement) which clearly delineates his or her role and responsibilities in the Agency/Business (to be developed in the first week of the Internship and placed in writing with copies to the student and University). 6. To review and evaluate Weekly Activity Reports and discuss them with the Intern Student when necessary. 7. To complete the Preliminary Student Performance Evaluation (to be signed by the Agency/Business Supervisor and then forwarded to the University Supervisor). 8. To complete the Mid-Session Student Performance Assessment, discuss it with the Intern, (this form must be signed by the Agency/Business Supervisor and the intern) and then forward it to the University Supervisor. 9. To complete the Final Student Performance Appraisal, discuss it with the intern (this form must be signed by the Agency/Business Supervisor and the intern, and then forward it to the University Supervisor. 10. To complete the recommendation form for the intern and submit it to the Internship Coordinator.
