1 City of Balch Springs, Texas Request for Proposal (RFP ) Pipe Bursting Sewer Line Replacement Balch Springs is a city in Dallas County, Texas, United States. It is a suburb of Dallas and part of the Dallas Fort Worth Metroplex. The population was 23,728 at the 2010 census. Balch Springs is located at N W. The city is bordered by Mesquite to the north and east, and Dallas to the south and west. The City of Balch Springs, Texas is seeking proposals from qualified firms for the replacement of sewer lines as part of the wastewater collection system upgrade for The replacement of the sewer lines are to be performed for four (4) separate lines in two areas of the City of Balch Springs: 1.0 POJECT DESCRIPTIONS Project A - Balch Springs Road 6 upsized to 8 HDPE Sewer Line replacement (1283 lf.) Project B - Quail Drive 6 upsized to 8 HDPE Sewer Line Replacement (944 lf.) Project C - Timothy Lane 6 upsized to 8 HDPE Sewer Line Replacement (390 lf.) Project D - Russell Lane 6 upsized to 8 HDPE Sewer Line Replacement (633 lf.) 2.0 GENERAL PROJECT REQUIRMENTS 2.1 All materials and construction shall conform to Public Works Construction Standards Standard Specifications & Standard Drawings published by the North Central Texas Council of Governments. 2.2 Bidders shall prepare price proposals for all four projects. Projects will not be awarded on an individual basis. 2.3 Pricing for each project will be accepted, however, evaluation will be based on combined pricing. 2.4 CONTRACTOR shall be responsible for the location and protection of all underground utilities while performing the work. 2.5 CONTRACTOR shall utilize the services of a professional underground utility locating service for each project. 2.6 Ground surface pavement, utility poles, fencing, structures, landscaping and obstructions shall be protected while performing the work. 2.7 Repair, replacement or improvement of surface pavement, utility poles, fencing, structures, landscaping and obstructions shall be the responsibility of the CONTRACTOR and his cost 2.8 Access to private property shall be the responsibility of the CONTRACTOR for notification and permission required to complete the work. 2.9 Plans, Specifications and details of the work are as shown in this RFP 2.10 Additional detailed plans and drawings as required shall be provided by CONTRACTOR CONTRACTOR shall inspect each project site thoroughly prior to submitting a proposal Previous point repairs may have been performed on each project and it is the responsibility of the contractor to determine the locations and number if required for each project. 3.0 PROPOSED SCOPE OF WORK

2 3.1 Project A Balch Springs Road 8 HDPE Sewer Line replacement Project begins at manhole located on west side of Balch Springs Road identified as MH-1 in this RFP. Project ends at MH-3 as identified in this RFP. Line replacement is along the west side of Balch Springs Road between MH-1 and MH-3. CONTRACTOR shall add 1 new manhole half way between MH-1 and MH-2 identified as NMH-1. Sewer line is clay tile pipe with approximately 3-4 PVC pipe point repairs made during sewer lifetime. (See attached map Project A) 3.2 Project B - Quail Drive 8 HDPE Sewer Line Replacement Project begins at manhole located south side of Quail Drive identified as MH-1 in this RFP. Project ends at MH-2 as identified in this RFP. Line replacement is along the south side of Quail Drive between MH-1 and MH-2. The CONTRACTOR will install a new manhole at Deerfield Trail identified as NMH-1. Sewer line is clay tile pipe with unknown number of point repairs made during sewer lifetime. (See attached map Project B) 3.3 Project C Timothy Lane 8 HDPE Sewer Line Replacement Project begins at manhole located on south side of Quail Drive identified as MH-1 in this RFP. Project end at MH-2 as identified in this RFP. Line replacement is along the east side of Timothy Lane between MH-1 and MH-2. Sewer line is clay tile pipe with an unknown number of PVC pipe point repairs made during sewer lifetime. (See attached map Project C) 3.4 Project D Russell Lane 8 HDPE Sewer Line Replacement Project begins at the manhole located on the south end of Brown Drive identified as MH-1 in this RFP. Project end at MH-2 as identified in this RFP. Line replacement is along the south side of Russell Lane and extending past the dead end of Russell Lane through an easement between MH-1 and NMH-1. Sewer line is clay tile pipe with approximately 4-5 PVC pipe point repairs made during sewer lifetime. The CONTRACTOR will install a new manhole at west end of Russell Lane identified as NMH-1. The CONTRACTOR will remove an existing access chamber approximately 150 feet west of the dead end on Russell Lane identified as AC-1. (See attached map Project D) 4.0 PROJECT SPECIFCATIONS 4.1 CONTRACTOR shall prepare combined work plan for Projects A, B, C & D i l l u s t r a t i n g and describing each component of the work to be performed including but not limited to the following: Locate and call out all surface and underground utilities and mark in field Describe pipe bursting methods for each project Pipe shall be HDPE 8 diameter with maximum SDR of Locations and details of insertion and receiving pits for each project Detailed equipment, rigging, anchors, winching, shoring and associated safety systems Detail methods of connection/sealing to lateral services and manholes Describe in detail preconstruction CCTV inspection of pipe joining, leakage, connections and alignment Show staging locations and traffic control plan for each project Describe in detail temporary bypass plan for each project

3 Describe unscheduled dig procedures for pipe burst head recovery due to obstructions or site condition CONTRACTOR shall notify t h e F i e l d O p e r a t i o n s M a n a g e r a n d r e c e i v e a p p r o v a l if an unscheduled dig is proposed prior to performing the work Submit supplemental shop drawings of manhole installations where installed for each project including proposed elevations, depths, flow lines and dimensions as required for each project CONTRACTOR will be required to furnish post-cctv inspection records to OWNER for review prior to project completion CONTRACTOR shall submit final records of construction to Field Operations Manager within 30 days of construction completion. 5.0 PROPOSAL SUBMITTAL INSTRUCTIONS 5.1 Plans & specifications as described in this RFP can be obtained from the City of Balch Springs Public Works Department, Alexander Road, Balch Springs, Texas or by registering with the Texas Purchasing Group: The cost proposal must be submitted in a separate sealed envelope with (2016 Pipe Bursting Project Sealed Bid) on the face of the envelope by 12:00 p.m., April 15, 2016 to William Freeman, Field Operations Manager, Alexander Rd, and City of Balch Springs, Texas The Cost proposal bids will be publicly opened and read aloud in the City of Balch Springs, City Hall Council Chambers at 2:30 p.m., April 15, Any questions regarding this RFP should be directed to William Freeman, Field Operations Manager, City of Balch Springs. He may be contacted by phone number X207 or by at [email protected]. 6.0 PROPOSAL SUBMITTAL REQUIREMENTS 6.1 Late proposals will not be considered. 6.2 Cost of proposal preparation shall be borne by the proposer. 6.3 This request does not constitute an offer of employment or to contract for services. 6.4 The City reserves the option to reject all proposals, wholly or in part, received by reason of this Request. 6.5 The City reserves the option to retain all proposals, whether selected or rejected. All proposals shall remain firm for ninety (90) days following closing date for receipt of proposals. The fee schedule and cost proposal shall be submitted in a separate sealed envelope. 6.6 The person signing the proposal must be a legal representative of the firm authorized to bind the firm to an agreement in the event of the award. 6.7 The City reserves the right to award the contract to the firm that presents a responsive and responsible proposal, which, in the judgment of the City, best accomplishes the desired results, and shall include, but not be limited to, a consideration of the professional service as described in the bid documents. 6.8 A cashier s check or acceptable bidder s bond payable to the City of Balch Springs, Texas in the amount of not less than 5% of the largest possible total for the bid submitted, must accompany the bid. 6.9 In the case of ambiguity or lack of clearness in the prices of the proposal, the City reserves the right to accept the most advantageous contract thereof to the City, or to reject the proposal.

4 7.0 PROPOSAL FORMAT 7.1 A responsive, qualifying proposal must address at a minimum all of the following items: Cover Letter with following information: Title of this RFP Applicant or Firm name and address Contact person and information Individual authorized to execute the agreement shall sign the cover letter/ Proposal Completed Bid Sheet with proposal pricing 8.0 FIRM QUALIFICATIONS 9.0 SCHEDULE 8.1 Description of firm 8.2 Provide age, capacity, and range of services. 8.3 List the names and qualifications of personnel to be assigned to provide professional services to the City. This shall include any sub-contractors & vendors that may be needed to fulfill the required tasks. 8.4 Provide a list of project history and/or brief description for which similar work has been performed. 8.5 Include letters or statements of reference for projects and work similar to tasks within this advertisement. 8.6 CONTRACTORs license to perform work. 9.1 Schedule is based on a 90 calendar days to complete the work described herein from the date of notice of award INSURANCE REQUIREMENTS 10.1 The selected consultant/proposer will be required to provide insurance coverage in the amount shown in the attached agreement for professional services. This amount of insurance coverage shall be reflected in the estimated professional fee The selected consultant/proposer shall provide within five (5) calendar days after the Notice of Award is issued by the City a certificate of liability insurance naming the City of Balch Springs and its employees and officers as additionally named insured. This shall be maintained in full force and in effect for the duration of the contract and must be in an amount and format satisfactory to the City The selected consultant/proposer will be required to defend, indemnify and save harmless the City of Balch Springs, its employees, officers, and agents, from any and all claims, demands, damages, costs, expenses, judgments or liability of any nature whatsoever which may result from the services.
