Our Academy currently is offering Cosmetology Program, Esthetic Program, and Instructor Program. As one of the oldest beauty schools in the area, we have trained thousands of students to reach their dreams in the beauty industry successfully. We wish you become our next success story. It is the intention of Cosmetology and Spa Academy to afford a quality education to all those students who make the decision to enter the field of cosmetology and/or esthetics and qualify for enrollment. The purpose of this catalog is to advise prospective students and current students of the rules and regulations of the Academy, and their rights and responsibilities before, during, and after their training program. It will also guide you through your daily activities and define responsibilities, please read it carefully. We believe that each student is an individual, and it is our Mission to help students obtain their career goals and meet changing demands of the beauty industry. WHO WE ARE Each campus of Cosmetology and Spa Academy is an expression of our hands-on practical excellence educational image. The Cosmetology and Spa Academy is established in June 1998 when Mr. Halimi took over ownership. Over the years, the school has graduated thousands of cosmetologist who is now veterans of the industry. Each campus contains a professional modern salon environment- clinic floor where students perform services for guests and clients under their instructors supervision. In additional, each campus provides a professional welcoming reception area that is supervised by our students, salon look like classes, where students experience their first hands-on experience with mannequin or real models, service area for performing nail service and spa services, esthetic clinic spa area where esthetic students perform spa services for clients and guests, student lounge, staff offices, dispensaries and shampoo area. The student salon and spa clinic area provide students with valuable practical experience. 10 The average salary range for a salon professional in Illinois and America is $30,000 to $48,000 per year. This average is based on full-time Employment and excludes tips. Tips are often an important part of a person s earnings and may account for 10-30% of income, depending upon the salon and spa, service prices, and the city. Note: Tips are considered income and the salon professional is responsible for taxes related to that income. Approximately 63.4% of Illinois industry business owners classify their business as a full-service salon, 14.7% as a haircutting salon, 6.7 % as a barbershop, 4.1% as a day spa and 3.3% as a nail salon. Nationally, 58.4% of salons are classified as full service, while 16.9% are haircutting only salons, 7.3% are barbershops; 4.6% are day spa s, and 4.3% are nail salons. 49.1% of Illinois salon and spa professionals work full-time (35 or more hours per week), 31.4% are part-time (20 35 hours per- week) and 19.5% are small time (fewer than 20 hours per week). Keeping up with the latest techniques and experiences is a fundamental requirement for this industry. Continuing education is a state requirement, and it varies from state to state. State of Illinois requires 24 hours continuing education every two years in order for cosmetologist to maintain their license. We do offer advanced training hours at our location in Crystal Lake. For more information about these specialized training, please visit our web page INDUSTRY PHYSICAL AND SAFETY DEMANDS Exciting opportunities await you in the beauty industry. Professionals in the beauty industry provide hair, skin and nail services to enhance their client s appearance. The beauty industry is best suited for persons who are in good to excellent health. It is our goal to teach our students all skills necessary for success in the beauty industry but it is very important for applicant to understand that many products are used during the educational process and in the field that may contain chemicals to which a person may be sensitive. It is, however, a fundamental requirement of these programs to use and demonstrate the use of the products carried by the Academy, both for classroom, as well as clinical work. Physical Demands: Stylists and Estheticians by the very nature of what they do, are required to spend long hours standing, sitting, bending, reaching and performing repetitive motions. Hold the arm and hand in one position and, at other times, keep the hand steady while moving the arm, lift up to 30lbs. Perform and demonstrate the full range of esthetics-related services standard within the skin care industry ( e.g., facial and body skin care with massage, facial and body waxing, and makeup application). View details in objects within arm s length and discern the difference between colors, shade, and brightness within and between those objects. Perform and demonstrate the full range of hair-related services standard within the cosmetology industry ( e.g., shampoo with massage, comb, haircut, hairstyle, hair color and chemical texturing, braid, sculpt, hair dry, etc.) Perform and demonstrate the full range of nail related services (e.g., file, buff, clip, clean, and apply polish to nails and/or attach artificial nail products). An important part of our education is to teach students to understand that developing good habits at the beginning of training will help prevent injury and/or health issues in the future. Safety: In order to protect the general public, state laws, rules, regulations, and/or standards apply to this field. All persons working in the field are responsible for maintaining a level of understanding and are required to practice according to current law as well as adhere to changes in the law. The state is not required directly to notify any person or entity of changes in the law and/or changes to standards. LICENSURE REQUIREMENTS The State of Illinois requires cosmetologist, estheticians, nail technicians and barbers to have a license in order to practice their profession legally. Every State in U.S. has different licensing requirements. Licensure and Endorsement information may be obtained by contacting Student Service Officer during regular business hours. More information can be found at ADMISSIONS ADMISSIONS REQUIREMENTS FOR ALL PROGRAMS OFFERED The Academy does not discriminate in its employment, admission, instruction, or graduation policies on the basis of disability, sex, age, race, color, religion, creed, national origin, or ethnic origin, marital status, sexual orientation, or membership in other protected groups, as defined by local, state or federal law. The Academy makes admissions decisions based on merit, including each candidate s interest, aptitude, and ability. The Academy does not recruit students already attending or admitting to another school offering similar programs of study. 11 Have successfully completed high school or its equivalent as evidenced by any of the items on the following non-exhausted list: copy of diploma, copy of GED certificate, copy of a transcript showing high school completion, or a certificate of attainment (only applicable to non-title IV recipients, ), or Have evidence of completion of home schooling that state law treats as a home or private school. If the state issues a credential for home schooling, maintain this credential; * Sign a completed Enrollment Agreement * and pay the enrollment fee ($ 100) if applicable Be at least 16 years of age Have a valid photo ID or driver s license Instructor applicants must meet all of the above requirements and: have a current license as a cosmetologist or esthetician. *Verification of a foreign student s high school documentation must be conducted by an outside agency that is qualified to translate documents into English and confirm academic equivalence to U.S. highs school diploma. * In the event that you must postpone the original agreement start date prior to completing the first week of the program, the enrollment fee is transferable to a new agreement start date for up to one year from the original date. All applicants are encouraged to read the catalog prior to signing the binding enrollment agreement. Applicant is required to submit all admission documentation prior to the enrollment. Students, who are enrolling under a training contract with another entity, must meet the admission requirements set forth in the training agreement with the other entity. TRANSFER CREDIT The Academy will accept student transfers from other licensed schools. Students who transfer into the Cosmetology program must complete at least 300 hours at our Academy, and 350 hours for the Esthetics program unless different arrangements are made with the Financial Aid Director. Student must have an official transcript from the prior licensed school to receive credit for the transfer hours. The Academy reserves the right to test the applicant prior to admitting the credit hours in part or all practical skills if it founds it necessary. The Academy reserves the right to request student to buy additional kit supplies if it founds that necessary for the student to participate successfully in the program. The decision of the Academy is final. RE-ENTRY REQUIREMENTS Former students of the Academy who wish to re-enroll must require approval from the Student Service Officer at studentservices@cosmetologyandspaacademy.com. The application will be reviewed, and a decision will be made within 30 days of the request as the re-enrollment is not guaranteed and will be determined on a case by case basis. Students who re-enroll within 30 days of the original exit date will be charged the same tuition and fees as detailed in their original enrollment agreement. All previous payments will be credited to the student's account, less any refunds made as a result of the withdrawal. Students who re-enroll more than 30 days after the original exit date may be evaluated scholastically in the same manner as a transfer student to determine class level assignment. Any previous balance from prior enrollments with the Academy must be paid in full before the student will re-enroll unless a written approved agreement is made with the financial office. Current tuition and fees will apply at the time of re-enrollment. Students may only re-enroll once within a twelve month period. This stays true for all programs offered at the Academy at both locations. Students may only re-enter into a different location of the Academy only once within a twelve month period. HIGH SCHOOL VERIFICATION PROCEDURES. If there is a question about the validity of the diploma, GED certificate, or equivalent, the Academy has the right to ask for further proof. If you need help verifying a GED, you can contact the Illinois GED Administrator of the Illinois Community College Board in Springfield, IL ( ) or visit the website, There is no appeal process for students if the Academy determines the high school diploma to be invalid. ATB VALIDATION The definition of an ability-to-benefit student is a student who is beyond the age of compulsory education, lacks a high school diploma or its equivalent, and can benefit from the education or training offered at the institution. The Academy does not accept ATB students. DISABILITIES AND ACCOMMODATION Cosmetology and Spa Academy is committed that no qualified handicapped person, by reason of his or her handicap, will be excluded from enrolling in a course of instruction or be subjected to discrimination based upon disability. 12 persons with disabilities, as defined in paragraph 104.3(j) of the regulation under Section 504 of the Rehabilitation Act of 1973, may apply for admittance into the program. However, all prospective students must meet standard admissions requirements. Cosmetology and Spa Academy does not formally assess students under the standards of a traditional 504 plan (IEP/other). If an applicant for enrollment believes he or she may need an accommodation during his or her program, he or she should advise a Career Planer prior to class start and must submit a written request for specific accommodation(s). The written request must include documentation of the student s official assessment (for example, an Individual Education Program (IEP) from the student s high school) and outline the specific accommodation(s) requested. Reasonable accommodation plans determined after the program start will be effective from the date determined moving forward. All requests for accommodation will be reviewed by the Academy s Owner and a determination of reasonable accommodations (if applicable) will be provided in writing prior to the start of the student s program and/or within four weeks of the official request for currently enrolled students. Due to state licensing requirements and rigorous industry standards that are in place to protect the public, all students are required to meet the academic standards outlined in the most current student catalog. INSTRUCTIONAL LANGUAGE The Academy is an English-speaking facility, and all instruction is conducted in the English language. The Academy does not offer English-as-a-second-language instruction. There is no level of English language proficiency required to attend. PROGRAMS OFFERED AT BOTH LOCATIONS COSMETOLOGY PROGRAM 1500 CLOCK HOURS Students at the Academy participate in a unique curriculum that combines educational materials from a variety of resources. Every student can integrate theory and practice from the most famous and trusted names in the cosmetology industry. These techniques and concepts are from Milady's books. Milady understands that the change is good as well as constant in the industry. Milady uses input from educators, students, and school owners. Fresh text layouts, up-to-date photographs, and easy-to-follow formats give reliability to students. The Milady Program remains a top choice of cosmetology and esthetics programs throughout the United States, Canada, and the world. COSMETOLOGY PROGRAM GOALS AND OBJECTIVES The curriculum of the Academy is designed to meet the needs of students who are job and career-oriented. The men and women who make up our students may receive an education that prepares them for specific cosmetology-related professional careers. Our curriculum is mainly directed towards helping students develop desirable habits and attitudes with respect to health, sanitation, and safety. It also helps students develop advanced technical, business, and people skills and encourages self-reliance, readiness to assist others, and an ethical approach to the profession. COSMETOLOGY PROGRAM CURRICULUM The Cosmetology Program meets the state of Illinois 1500 hours curriculum requirements as follows: general theory / basic training (150 hours); practical chemical application (100 hours); hairstyling (100 hours); shop management / interpersonal relations (100 hours); esthetics (85 hours); hairdressing (375 hours); hair treatment (400 hours); sanitation / safety and shop management (100 hours); nail technology (55 hours); related electives (35 hours). COSMETOLOGY CURRICULUM IS DIVIDED INTO PHASES Classroom: The first hours are in a classroom setting with all necessary theory and practical skills. The theory is divided into blocks: spa/long hair design, cutting, color/chemical, and hair design. Each block has a core practical lesson and multiple theory chapters to accompany the practical skill being taught. As each lesson is taught, the students will be practicing skills learned and will have many opportunities to bring in live models to practice those skills. At the end of each block, students will be evaluated on their skills taught in order to allow them to move on to the student salon, commonly known as the clinic floor. Clinical Salon: This is the remaining hours of training. In the clinic, students will be practicing and perfecting all of the skills learned in the classroom and applying them to the salon environment. Students will be accepting clients and gaining experience with client relations and consultations under the supervision of our trained educators. Students will also be encouraged to market themselves and begin building their clientele. Students may practice cuts, color applications and foil techniques using gel, cholesterol or equivalent of on their mannequins. Services must be School Catalog Previous Version Revise Page 12

13 completed only on clients. Our source materials are: ABC Colouring the Sassoon Way; ABC Cutting Hair the Sassoon Way, along with the appropriate DVDs; Regents / Prentice Hall textbooks of Cosmetology; Milady s Textbooks; Martin Parson's long hair design textbook with DVDs. Periodicals include American Salon, American Spa Video Program, and Vogue Magazine., Tigi color line, Sam Villa. Students will be taught the application of fusion Extension technique in hair design training, Shellac technique on nail and Air Brush technique during their spa training. STATE LICENSING REQUIREMENTS In the State of Illinois, a cosmetology student must complete the 1500 hours of cosmetology training. A person is qualified to receive a license as a cosmetologist will have to file an application on forms provided by State of Illinois, pays the required fees and 1. is at least 16 years of age; and 2. is beyond the age of compulsory school attendance or has received a certificate of graduation from a school providing secondary education or the recognized equivalent of that certificate; and 3. PAYMENT PLANS FOR ALL PROGRAMS Method of payment includes financial assistance to those that qualify, cash, check, credit card or via payment from an outside agency or scholarship.payment plans and are free of interest and available in the office.there is a 3% fee on each payment processed with credit card. KIT PAYMENT OPTIONS FOR ALL PROGRAMS Students must pay the cost of the kit for books and supplies one week prior to the starting date unless the Financial Aid covers it. Student will receive a kit in parts based on their hours earned and the level of education they are at. Kit is property of the school and must stay in the school facility until it is paid in full. If it covered with financial aid kit will become the property of the student at 1500/750 hours only. Lost or damage on kit supplies will be replaced by students on their costs. The kit fee is nonrefundable in any case. Options for covering the cost of the kit are discussed in a financial meeting before the student begins training. GUEST SPEAKERS The Academy regular schedules guest speakers and educators. They include instructors, professional stylists, salon owners, and major manufacturer representatives. Students gain insight into the real world of the cosmetology industry. ESTHETICS PROGRAM- 750 CLOCK HOURS The Esthetics Program requires 750 hours of training in Illinois. Students will learn how to be professional in Esthetics and successful in their careers by going through theory and practical procedures that make them comfortable and confident in a professional environment. This program is currently offered in both our Crystal Lake and Schaumburg locations. ESTHETICS PROGRAM GOALS AND OBJECTIVES Students will learn the conditions of the skin, determine treatment service, use correct techniques, and can successfully perform services on real clients. They will also learn the proper operation of the facial machinery, chemical peels, microdermabrasion, facials, body treatments, hair removal, and makeup procedures. ESTHETICS PROGRAM CURRICULUM The Esthetics Program meets the State of Illinois curriculum requirements as follows: basic training in the general theory (75 hours); instruction in scientific concepts (150 hours); instruction in practices and procedures (500); education in Illinois law and business practices (25 hours). ESTHETICS PROGRAM CURRICULUM DIVIDED INTO PHASES Phase One: The student will be introduced to the field of Esthetics and will learn the basic practices and procedures including facials, hair removal, sanitation, machine facials, microdermabrasion, and chemical peels. The student will be fully immersed in the student spa fine tuning the procedures learned in the previous phases. STATE-LICENSING REQUIREMENTS 750 CLOCK HOURS Students must complete the 750 hours of Esthetics training, have a high school diploma / official graduating transcript / GED certificate, be at least 16 years of age, and pass the State exam with a 75% or above. The Academy follows proper procedures to evaluate the validity of the student's high school diploma. The Esthetics license is renewed every two years. INSTRUCTOR PROGRAM 500 AND 1000 CLOCK HOURS The applicant must have an active cosmetology/esthetic license to enter this level of training, complete an application for enrollment, and complete an instructor in training application to be forwarded to the State Board of Cosmetology/Esthetics. Graduates from this program must pass the State written exam prior to being issued Instructor's license. Cosmetology instructors find employment in schools where they can teach cosmetology, manicuring, and esthetics. Esthetic instructors find a job in schools where they can teach esthetics. Employment may occur in proprietary, or vocational and technical schools on either a full- or part-time basis. Earnings vary with experience and motivations as well as from one region of the country to another. Cosmetology instructors stand for most of their working hours and work with their hands demonstrating practical techniques. They are exposed to chemicals in hair colors, permanent waving lotions, manicure, pedicure, and facial products. Esthetics Instructors stand for the majority of their working hours and will need to sit for long periods also with individual demonstrations of practical techniques. They must be knowledgeable with a majority of machines and modalities in skin care. Lesson planning, evaluating, and counseling students are all parts of both programs instructors job description. They should also be able to communicate well and enjoy working with students. 16 INSTRUCTOR PROGRAM GOALS AND OBJECTIVES The curriculum of the Instructor Program requires students to learn classroom management, teaching methodologies, goal planning / setting, curriculum and lesson planning, and teaching styles. INSTRUCTOR PROGRAM CURRICULUM 1000 HOURS The Instructor Program meets the State of Illinois requirements as follows: educational psychology (20 hours); teaching methods (20 hours); application of teaching methods (150 hours); business methods (50 hours); student teaching (260 hours); graduate training (500 hours). The 500 hours program will complete half of each hour required at 1000 hour's program. STATE REQUIREMENTS Must be a currently licensed cosmetologist or esthetician and must have a high school diploma or GED. Applicants are required to have at least two years of practical licensed salon and spa experience for the 500 Clock Hour Teacher Training courses. Proof of licensure is required. In the state of Illinois, cosmetology teachers may also teach esthetics and/or nail technology provided they have demonstrated skills in these areas. To be become Instructor in the State of Illinois candidate must have: 1. graduated from high school or its equivalent; and 2. has a current license as a cosmetologist; and 3. has either: completed a program of 500 hours of teacher training in a licensed school of cosmetology and had 2 years of practical experience as a licensed cosmetologist within 5 years examination; or completed a program of 1000 hours of teaching training in a licensed school of cosmetology; and 4. has passed an examination authorized by the Department to determine fitness to receive a license as a cosmetology teacher. INSTRUCTIONAL METHOD AND MATERIALS USED TO TEACH: The Academy uses the Milady Master Educator textbook series. We host sessions of instruction where students will do teachbacks to other staff members for constructive criticism. The student instructor will also create lesson plans and take master educator exams throughout the course of training. 17 The barbering program curriculum of the Cosmetology and Spa Academy is designed to meet the needs of students who are job and career oriented. The men and woman who make up our students receive training and practical experience which qualifies them as outstanding candidates for many barbering related jobs. Our curriculum is mainly directed towards helping students develop desirable habits and attitudes with respect to helping, sanitation, and safety. It also helps students develop advanced technical, business, and people skills and encourages self-reliance, readiness to assist others, and an ethical approach to this profession. BARBER PROGRAM CURRICULUM 1500 HOURS The Barbering Program meets the State of Illinois requirements as follows: Classroom instruction in general theory/ basic training (150 hours) : haircutting (70 hours) students will implementations, tools and equipment, properties and disorders of the hair and scalp, shaving and facial design, men s haircutting and styling, woman s haircutting services, treatment of the hair scalp, principals of prevention control, solution and strengths, disinfection and procedures, public sanitation and rules of sanitation, chemical services (35 hours): student will learn the safety of working with chemicals and chemical procedures as well as: nature of chemical texture services, permanent waving, hair relaxing, high coloring and lighting, color theory, product applications, procedure terminology, corrective hair coloring, coloring mustaches and beards, safety and precautions, basic perm and relaxer, touch up, foil, Spa (35 hours) in this block student will take time and focus on grooming of nails, scalp treatments and massage, facials, hair replacement and sanitation, subdermal system and facial massage, theory massage; safety and shop management ( 35 hours ), in this phase students, will learn preparing for state board exams, rules, and regulations, industry trends then and now and preparing for employment techniques, employment review, self-employment and business ownership, how to operate a successful barbershop; clinic hands on practical (1350 hours) when students will learn to practice all the skills they will need in a real job environment. STATE REQUIREMENTS In the State of Illinois, a barber student must complete the 1500 hours of barber training. Each applicant shall file an application for examination, on forms provided by the Division, at least 45 days prior to an examination date. Individuals who do not obtain a license within five years of graduation from barber school will be required to complete a 250-hour refresher course before they may obtain a license. A person is qualified to receive a license as a barber if that person has applied in writing on forms prescribed by the Department, has paid the required fees and a. Is at least 16 years of age; and b. has a certificate of graduation from a school providing secondary education, or the recognized equivalent of such a certificate, or persons who are beyond the age of compulsory school attendance; and c. has graduated from a school of barbering or school of cosmetology approved by the Department, having completed a total of 1500 hours in the study of barbering extending over a period of not less than 9 months nor more than 3 years. A school of barbering may, at its discretion, consistent with the rules of the Department, accept up to 500 hours of cosmetology school training at a recognized cosmetology school training at a recognized cosmetology school toward the 1500 hour course requirement of barbering. Time spent in such study under the laws of another state or territory of the United States or of a foreign country or province shall be credited toward the period of study required by the provisions of this paragraph; and d. has passed an examination caused to be conducted by the Department or its designated testing service to determine fitness to receive a license as a barber. Passing all written and practical exams with a score of 75% or higher. Passing all parts of the Final Exam with a score of 75% or higher. Complete at least 80% of clinic requirements. Please note: In order to take the State Board Exam, student will need an Official Transcript. Student will receive an Official Transcript, after all, financial obligations with the Academy are paid in full. The Academy will charge $ 2.00 /page fee for any additional official transcript, enrollment agreement, school catalog, or any other formal copies of student s file. The request to receive an official transcript must be in writing and must be sent to Student Service Department. If all the requirements are satisfied, the official transcript will be ready within 20 business days from the date the request has been received. Payments after 1400 hours for Cosmetology and 600 hours for Esthetics will be accepted only in the form of cash or money order. Upon graduation, a Graduate Survey will be distributed. All students should complete the Graduate Survey, prior to filing for the state board exam. It is the student s responsibility to ensure that all of his or her exams, curriculum, and other assignments are completed and to ensure that his or her financial obligations have been met prior to the completion of hours and exams. Failure to comply with the payment agreement will deem the student in default and subject to all reasonable collection charges and/or attorney fees. All student financial aid recipients are also required to attend an exit interview at STATE BOARD EXAMINATION In order to legally practice cosmetology or esthetics in Illinois, a person must complete and pass all portions of the Illinois state board examination and must be at least 16 years old. The state charges a separate fee for the examination and licensing. Failure of the exam or subsequent exams (retakes) will result in additional costs. All licensing and testing fees are the responsibility of the person filing for the exam or license. Licensing and testing costs are not included in the tuition and fees of the Academy. In order to file for the state board examination, students must satisfy the requirements of both The Cosmetology and Spa Academy (see Graduation Requirements) and the Illinois Department of Professional Regulation, The Illinois Department of Professional Regulation requires the submission of all pertinent forms and payment of all monies prior to taking the state exam. All graduates are encouraged to file immediately upon graduation. A delay in filing may be costly to the graduate not only in increased fees but also in loss of time employed. Note: The State Board may refuse to issue a license to any student that has been convicted of any crime under the laws of the United States or any state of territory thereof that is : a felony, a misdemeanor, an essential element of which is dishonesty, or a crime which is related to the practice of cosmetology. 19 SCHOOL FINAL/RETAKES Schools finals will be taken each month. Each student is required to take and pass a final exam prior to graduation. The final exam consists of a written and practical test. Student must request in writing to take the final exams at least one full week prior to the testing date to ensure appropriate time for the approval. To be able to sign up for the final exam, the student must meet the following requirements: Complete 1450 hours for a cosmetology program and 720 hours for esthetic program; ( exceptions will be considered by the Academy to rare situation like due date on pregnancy, death in the family, or moving out of state) See Student Service Officer for more information. Complete 80 % and over syllabus requirements; Complete all tests in the course of study; If a student fails to attend or pass the Academy finals (practical or written), there is a charge of $50.00 that must be paid prior to retaking the exam at the next scheduled test date. Financial Aid will not cover these fees. Final exams are only given while the daytime, evening students must make arrangements to take all final exams during the day. If a student has to make the final after 30 days of the graduation date, the student has to pay $ prior to taking the final test. In the case of unforeseen circumstances, student must provide documentation, and the fee may be avoided upon approval by the administration. All financial obligations must be fulfilled and in good standing prior to taking the final exams. Cosmetology students are required to take and pass a pre-final test at 900 hours of training. Information in this prefinal exam will be provided to students prior to the exam. LEAVE OF ABSENCE The Academy will grant a leave of absence for medical/ health related or emergency reasons only. The reason must be documented and must be for the student or immediate family members, (kids, husband/wife, parents or sibling). The leave of absence must be requested in writing and must be approved by the Financial Aid Director at least five days in advance unless there is an emergency situation. Deadline for Filing a Request: Leaves should be required in advance of the requested leave date. In emergencies, this may not be possible; for this reason, the student is to contact the Student Services immediately. All requests for leave are to be submitted in writing. Faxed applications may be considered; however, by federal regulation ed requests are not sufficient. The requests must state the related reason the LOA is required and who is the person involved in the situation, start date of the LOA and end date of LOA. The request must be dated and signed by the student. Student who fails to follow the LOA request procedure will be considered withdrawn from the program. Any student approved for a temporary leave of absence will not have this interruption in training counted against his / her compliance with satisfactory progress guidelines. However, a leave of absence will extend the student's contract period and maximum time frame by the same number of days taken in the leave of absence. The minimum time frame for a leave of absence is two weeks; a leave of absence is limited to 60 days. In the case of a health emergency with documentation, the Academy may grant a leave of absence for up to 180 days. A student not making satisfactory progress prior to a leave of absence will resume training, not making satisfactory progress. During an approved leave of absence, the student may not receive federal loans. HOLIDAYS AND VACATIONS Official Academy holidays and vacations for 2015: January 1 -New Years Day, Memorial Day; Week of July 4; (Summer Break); Labor Day; Thanksgiving Day; December 23 December 28 (Winter Break). Additional dates may be observed as determined by the Academy. WEATHER-RELATED ACADEMY CLOSING The Academy does not close due to weather-related conditions. When there is a serious weather emergency that causes retail, business and government offices to close, the Academy will typically cancel or delay classes. For both locations, an announcement will be posted on the official Cosmetology and Spa Academy Facebook page. Students who are required to follow the Academy on Facebook for weather updates and school closings. If a student does not have access to social media, they must contact student services with alternative contact information prior to the first day of class, preferably during the Orientation session. It is imperative that each student keep the Academy updated with any changes in their address and cell phone number to ensure proper delivery of these messages and like our Facebook Page. 20 weather emergency the graduation date of the student will not be updated and students may use their personal hours or make up the day based on the current makeup hours policy. SATISFACTORY PROGRESS POLICY SAP Satisfactory Academic Progress Policy is consistently applied to all students enrolled in a specific program and scheduled for a particular category or attendance ( par time/full time). Satisfactory progress in attendance and academic work is a requirement for all students attending the Academy, whether participating in the Title IV financial aid program or not and is explained and provided to the student prior to enrollment as a crucial part of the Academy catalog. Students receiving funds under any Federal Title IV financial aid program MUST maintain satisfactory progress in order to continue eligibility for such funds. The policy complies with the guidelines established by the National Accrediting Commission of Career Arts and Sciences (NACCAS) and the federal regulations established by the United States Department of Education. DETERMINATION OF PROGRESS STATUS Student is meeting both minimum requirements: 67% attendance standard and the 75% academic standard, at the evaluation point is considered to be making satisfactory progress until the next scheduled evaluation. Student who does not achieve the minimum standards is no longer eligible for Title IV, HEA program funds, unless the student is on warning or has prevailed upon appeal of the determination that has resulted in the status of probation. Student will receive a hard-copy of their Satisfactory Academic Progress Determination at the time of each of the evaluation. EVALUATION PERIODS Students are evaluated for Satisfactory Academic Progress as follows: Cosmetology Program at 450, 900, 1200, 1500 clocked actual hours ; Esthetics Program at 375, 750 clocked actual hours; Instructor's Program 1000 hours at 450 and 900 clocked actual hours; Barber Program- at 450, 900, 1200, 1500 clocked actual hours. Students that are meeting SAP at the time of the evaluation will be continuing financial aid. Students that are not in SAP will meet with student service department to discuss their progress status. ATTENDANCE PROGRESS EVALUATIONS Students are required to attend a minimum of 67% of the hour's possible based on the applicable attendance schedule in order to be considered maintaining satisfactory attendance progress. Evaluations are conducted at the end of each evaluation period to determine if the student has met the minimum requirements. The attendance percentage is determined by dividing the total hours accrued by the total number of hours scheduled. At the end of each evaluation period, the school will determine if the student has maintained at least 67% cumulative attendance since the beginning of the course that indicates that, given the same attendance rate, the student will graduate within the maximum time frame allowed. ACADEMIC PROGRESS EVALUATION The qualitative element used to determine academic progress is a reasonable system of grades as determined by assigned academic learning. Students are assigned academic learning and a minimum number of practical experiences. Academic learning is evaluated after each unit of study. Practical assignments are evaluated as completed and counted toward course completion only when rated as satisfactory or better. If the performance does not meet satisfactory requirements, it is not counted, and the performance must be repeated. At least two practical skills evaluation will be conducted during the course of study. Students must make up failed or missed tests and incomplete assignments before graduation. Students must maintain a written grade average of 75% and pass a FINAL written and practical exam prior to graduation. Numerical grades are considered according to the following scale: Theory / Practical quality: Excellent, 100%-93% (A); Good, 92%-87% (B); Average, 86%-80% (C); Below Average, 79%-75% (D); Unsatisfactory, 74%-0% (F). MAXIMUM TIME FRAME The maximum time ( which does not exceed 150% of the course length) allowed for students to complete each course at satisfactory academic progress is stated below: Course Maximum time allowed Weeks Scheduled Hours 1.
