The Designer s Approach to education brings together the best ideas of science and art within a sound educational foundation. The operator s course prepares students to pass state boards and begin a lucrative career in cosmetology. The teacher s course reinforces the basics as well as the necessary teaching fundamentals. They learn how to pass their knowledge and skills on to others. Esthetician students will be taught advanced skin care techniques so they can join the ranks of licensed estheticians, in this rapidly growing industry. Our goal is to provide every student with the tools needed to grow as individuals and become leaders in the field of cosmetology. WHAT CAN YOU EXPECT You can expect an education far beyond the 'how" of cosmetology and into the "why". At the Pennsylvania Academy of Cosmetology, we offer more than the education necessary to pass your state board examination and become a practicing cosmetologist. We offer a 1250-hour Cosmetology Operator's course, which stems from fundamental principles and incorporates classroom instruction with practical client application. We also offer a 600-hour Cosmetology Teacher course, which gives the fundamental principles and incorporates classroom instruction with practical application in a classroom. We assign Student Teachers to the clinic or to specific classes to teach theory under the supervision of a licensed instructor or for observation in the teaching principles depending on the student s ability to assist instructors in scheduled activities. They must perform mock teaching activities for various units of instruction along with practical application of cosmetology practice as applied to teaching. 02/14/2013 4

You Will Look At Hair A Whole New Way Textural Variety The Arts Nature The Hair Medium Hair is an artist's medium much like textile fibers. One important aspect of any artistic medium is its ability to take on various textures and forms. It allows the hair-designer full range of expression from serene and flowing to exciting, highly activated texture. 6 process. Second, the principles to develop effective teaching techniques, which combine the design and decision-making process with the development of hand control. LEARN THE EASY WAY STEP-BY-STEP You will learn the principles of science and art through Programmed Learning. The renowned psychologist, B. B. Skinner, introduced the concept of Programmed Learning. His idea was simple but effective learning is easier when complicated procedures are broken into short easy steps. On this concept many educational tools are used to support our curriculum. Conceptual tools help you clearly picture the design image in your mind before you begin working with hair. The range of conceptual tools offered is unique to the Pivot Point system. You will learn through sculptures, which demonstrate the elements of form and shape, models, which present varieties in texture. Hair goods progress from a simple hair weft, all the way to full head forms. Programmed exercises with a full range of tools help you to develop manual control. You will learn the how and the why of hair. MODULAR TRAINING SYSTEM The complete modular training system facilitates our Programmed Learning Approach. You will achieve scientific and artistic understanding through the Modular Training System. This system allows you to progress from the simplest concept to the most complex. This involves head-form drawings to enhance your comprehension, and progress to hair wefts for line experimentation, rectangle sections for movement, and full mannequins for total design concepts. After mastering these, you will progress to live models-- your real test! This system of learning helps you analyze, breakdown and understand the components for creating cuts, designs, color, and texture services for your clients. OUR EDUCATIONAL FACILITIES Our workshop classrooms are spacious, well lit and ventilated. Each one adapts easily to either a theater or a workshop style room and contains plenty of seating, a platform area, and storage cabinets, sanitized floors and work area, water facilities and drying cabinets. All of our classrooms are equipped with large screen TV s and laptop computers with internet access. Our curriculum has power point presentations and DVD s that support each chapter in our Cosmetology Fundamental Textbook giving students a visual reinforcement to support the lessons. The Student clinic is divided into sections similar to a salon, with each section designated for students in a particular cycle. The large, well-ventilated and properly lighted area for students assigned to the student salon, hydraulic chairs, a shampoo area, a lab dispensary, reception seating and a reception desk. There is also a special area for you to practice skin care, manicures, and make-up on your clients. Our school includes a student locker area with individual locker sections. Here you can secure you personal property. The Student lunchroom is equipped with vending machines for your convenience. It is a nice place to eat your lunch or just to meet with your fellow classmates. You will also be able to purchase professional supplies and products in our conveniently located fully stocked Educational Service Store. As a student, you will receive special discounted prices on all purchases. INTERNET ACCESS Pivot Point added another valuable educational tool for our students by giving them access online for 24 months to all of their Pivot Point course books, textbooks including practice exams. 7 REFERENCE LIBRARY The Pennsylvania Academy of Cosmetology maintains an extensive library of materials. It includes a large section of audio/visual programs and equipment, which are available for student research. You may refer to these programs to extend your knowledge or brush up on a past lesson. If you wish to view any educational material just, inform your instructor she/he will inform you of the policy and procedure for signing out materials. PENNSYLVANIA ACADEMY OF COSMETOLOGY The National Accrediting Commission of Career Arts and Sciences is our accrediting agency and; licensed by the Pennsylvania State Board of Cosmetology. We are also a member of the following professional groups: National Association of Accredited Career Schools (NACCAS) National Cosmetology Association (NCA) American Association of Cosmetology Schools (AACS) Cosmetology Educators of America (CEA) THE COSMETOLOGY OPERATOR COURSE OBJECTIVE The objective of our cosmetology operator's course is to acquaint you with all aspects of cosmetology so that you will have the well-rounded background you need to become a successful cosmetologist. We strive to teach you principles, which you will be able to rely on throughout your career. You will learn the physics, biology and chemistry of hair, as well as artistic approaches to hair sculpture, design, color and texture. You will be able to work with hair of all lengths, styles and textures. Skin care and manicuring is also part of you training. In addition, you will receive special preparation for the State Board Examination, which you must pass in order to practice your profession. To prepare you for that successful career; the Academy has a Salon Success Program. As you work with Salon Success, you will see that it is preparing you to succeed, not only in the classroom and during clinic training, but also in your ongoing career. Salon Success deals with essential life skills that will create your personal foundation for career success. You will develop skills for character development, interpersonal relationships, professional communication, career planning and self-management. These skills will help you in all facets of your life. PROGRESSIVE GRADING SYSTEM A progressive grading system will be part of your curriculum. To graduate from any of the courses the Pennsylvania Academy of Cosmetology offers you will take written and practical examination throughout your entire education. This system of grading allows the teacher to see that all students' progress and complete the requirements necessary to graduate. It also tests the ability of the student to execute the techniques on clients. We can evaluate your practical knowledge for the various levels of training and be able to convey this grading system to potential salon owners. Students need accumulative grade of 80% of combined academic and practical evaluations to graduate from any one of the three courses that the Academy offers you must complete all necessary requirements in order to receive a diploma or certificate from the school. Competitions are additional opportunities not just for student participation, but also as a means of evaluation. The Mock State Board Written Examination helps prepare you to obtain your license. The reviews and the actual "Mock" examination spotlight cosmetology laws, client safety and sanitary precautions. Each student must have a grade of 80% or higher on the Mock examination in order to pass. The grading system for the Pennsylvania Academy of Cosmetology is base percentage. All students must maintain satisfactory progress a grade average of 80%. These competitions give you the opportunity to receive your first professional award. We grade on a 20 to 30 point scale, as dictated by the international competition system. You will have three categories of competition to work in. We award first, second and third place Academy pins per division. ADMISSIONS The first step in registering for the Pennsylvania Academy of Cosmetology is to contact our Admissions Office. All prospective students are required to take the verbal portion of the Wonderlic Basic Skills Test. This test assesses the prospective student s basic reading comprehension level this assessment is part of the admissions process taken before a prospective student can enroll. You can select a starting date and fill out an application form that very same day. Upon receipt of your application form, we will finalize the necessary papers and assist you in all the help you need in completing your financial aid forms; we have all the forms available at the Admissions Office. The Friday prior to your start date, you will go through our Orientation Program. This will give us the opportunity to provide information about the course, the goals of the course, review policies and procedures and meet staff 02/14/2013 8
9 and students. A $ enrollment fee will be due on or before your first day of class. When you completed your enrollment requirements, you are on your way to an exciting and rewarding career. ENROLLMENT REQUIREMENTS To enroll in our cosmetology operator s course or esthetician course you must possess a high school diploma, GED Certificate, letter or transcript from high school, GED test score of 40 or more per category. TUITION COSTS AND PAYMENT TERMS For specific information on tuition and payment terms, refer to the course catalog page entitled "Starting Dates, Registration Fee, Tuition and Kit Costs" see catalog page 21. SCHOLARSHIP POLICY Each graduating senior is eligible for a $150.00, $ or $ scholarship. They must start in the June, August or September classes. Due to certain circumstances, scholarships may extend beyond the September class deadline. Students that attend home schooling may run into a situation where additional time to meet requirements to graduate would take them passed our September class deadline. The President and supervisor will review the situation and may grant a scholarship after the September deadline. The Academy reserves the right to review certain circumstances that may extend the deadline. A student beginning our Operator (1250 clock hours) course and meeting the above requirements may receive a $ Scholarship. A student beginning our Teacher (600 clock hours) course and meeting the above requirements may receive a $ Scholarship. A student beginning our Esthetician (300 clock hours) course and meeting the above requirements may receive a $ Scholarship. As per our transfer policy, any high school senior that is required to finish hours to complete 1250 will be eligible for a $ scholarship. All operator graduates who received 900 or more hours at the Pennsylvania Academy of Cosmetology are eligible to apply for a $ Scholarship towards their Teachers Course. To qualify you must begin the teacher s course within one month after graduating from the operator s or esthetician course. The evaluation and awarding of these scholarships are on an individual basis. The Pennsylvania Academy of Cosmetology has the right to eliminate or adjust this offer at any time. TRANSFER STUDENTS POLICY AND PROCEDURE We at the Pennsylvania Academy of Cosmetology Arts & Sciences accept all students who request transfer from another cosmetology school or Vocational Technical school, to our institution. This includes transfers from all other states. (When contacting the State Board of Cosmetology they said as long as we would accept hours from other states, they would also accept them.) For us to accept transfer hours the hours they must be on an official school transfer of hours form. The transfer hours must also meet the State Board Policy requiring a transfer student to complete all 1250 hours within a four consecutive year window. The only stipulation, the Academy has, is that the student must complete a minimum 600 clock hours at our institution this is due to the Pivot Point Training offered. Due to the Vocational Technical Schools recently offering the Teacher Course, we will except transfer hours, however if a student needs less than the minimum of 450 clock hours which is needed for our Pivot Point Training we cannot certify them as a Pivot Point Graduate. THE COSMETOLOGY OPERATORS COURSE CURRICULUM Your complete cosmetology operator's course consists of 1250 hours of instruction and practical experience divided into 6 cycles of learning. The course extends 45 weeks. Students cannot exceed 30 hours a week. 02/14/2013 9
10 Enrollment Requirements are as follows: Be at least 16 years old 10 th grade education or equivalent; or waive the 10 th grade education requirement by providing proof of being 35 years of age or older. (Proof of age: copy of your Driver s License, Birth Certificate, Visa or Passport) Throughout your education, you will be exposed to theory, audiovisual presentations, lectures, demonstrations and practical hands-on experiences, in many cases, the Academy's program exceeds state board minimum requirements giving you the advantage over graduates from "minimum requirement" schools. The cycles of instruction cover the following areas: Long Hair Graphics - 60 hours - Practical 55 hours - Sanitation 5 hours. Basic Facials, Manicures/Pedicures & ph and the Professional - 30 hours - Theory 10 hours - Practical 17.5 hours - Sanitation 2.5 hours Hair Sculpting - 90 hours - Theory 40 hours - Practical 44.5 hours - Sanitation 5.5 hours Hair Design - 90 hours - Theory 40 hours - Practical 44.5 hours - Sanitation 5.5 hours Chemical Cycle hours - Trichology/Texture - Theory 40 hours - Practical 44.5 hours - Sanitation 5.5 hours Hair Coloring - Theory 45 hours - Practical 40 hours - Sanitation 5 hours Junior Cycle hours - Theory 30 hours - Practical hours - Sanitation 27.5 hours Senior Cycle hours - Theory 24 hours - Practical 311 hours - Sanitation 15 hours During your Junior and Senior Cycle you will be in the student salon, performing work on clients and demonstrating your ability to execute what you have learned. During your Junior Cycle, you will go through Black Hair Design, Color Design, and Men's Sculpting. Senior Cycle you will enjoy lectures and discussion sessions focused on resume writing, interview tips and various career opportunities and Sculptured Nails. In addition to the specified classroom hours in each subject, you are required to have a minimum number of clinic units in each subject. Daily we record each service or assignment you perform in the student salon ensuring that you complete the minimum number of units per subject. During your Junior Senior cycle, you will practice the Mock State Board examination. The Mock Examination closely simulates the actual state examination you will be required to take in order to receive your license and practice your profession. A holder of a cosmetologist license is qualified, without further licensure to perform the functions of a cosmetologist, esthetician, nail technician and natural hair braider. COSMETOLOGY TEACHER The Pennsylvania Academy of Cosmetology Arts and sciences also offers a course of study that enables you to become certified as a beauty, and hair design teacher. To take the Pennsylvania State Board Examination you must meet the following requirements: Be at least 18 years old Hold a high school diploma or its GED equivalent Be a licensed cosmetologist in the state of Pennsylvania (either solely trained in Pennsylvania or through reciprocity). Enrollment Requirements are as Follows: Hold a high school diploma or its GED equivalent Have completed 1250 hours of cosmetologist operator training The teachers training course consists of specialized teaching methods, involving all phases of Cosmetology, lesson planning, professional conduct, classroom management, demonstrations, and lecturing methods. This 600 hour course will focus on an in depth and extensive study of the latest techniques from top hair designers from around the world, with hands-on workshops and demonstrations with an emphasis on mastering the skills of hair sculpture, hair design, permanent waving and hair coloring. 11 weeks and extends 28 weeks. If you currently hold a teacher's license in another state, you will need to apply for reciprocity with the state of Pennsylvania in order to receive credit for the training you have. If there are additional hours of training needed in Pennsylvania, then you will need to meet the above requirements. A holder of a cosmetology teacher license is qualified, without further licensure to perform the functions of a teacher, cosmetologist, esthetician, nail technician and natural hair braider. The instruction covers the following areas: Classroom Training hours Students assist instructors in classrooms and observe teaching techniques 100 hours Performance of Demonstration for all courses 100 hours Technical Supervision of Clinic Practices 100 hours Salon Management 100 hours ESTHETICIAN COURSE The Pennsylvania Academy of Cosmetology Arts and sciences also offers a course of study that enables you to become a licensed esthetician. Enrollment Requirements are as follows: Be at least 16 years old 10 th grade education or equivalent; or waive the 10 th grade education requirement by providing proof of being 35 years of age or older. (Proof of age: copy of your Driver s License, Birth Certificate, Visa or Passport) A holder of an esthetician license is qualified, without further licensure to perform esthetician services only. The 300 hour Esthetician Course is divided into a 17 weeks and extends to 20 weeks. The course begins with 5 weeks of basic training (90 hours) of classroom instruction in both theory and practical practices of basic techniques and procedures. The remaining 210 hours are spent in advanced workshops, machine and specialized facials, customer relations, managing an esthetics business, state boards, scheduled theory classes and supervised clinic floor activities. Following your training, you will qualify to take the State board exam for licensing as an Esthetician. The cycles of instruction cover the following areas: Basic Esthetics - 90 hours Theory 37 hours - Practical 48 hours - Sanitation 5 hours. Skin Care and Anatomy - 72 hours - Theory 24 hours - Practical 43 hours - Sanitation 5 hours Skin Care, Chemistry and Advanced Treatments - 72 hours - Theory 24 hours - Practical 43 hours - Sanitation 5 hours Skin Care, Business and State Boards 66 hours - Theory 24 hours - Practical 36 hours - Sanitation 6 hours STARTING DATES Classes begin every six weeks for cosmetology/cosmetology teacher and twice a year for esthetician. For specific start dates, please refer to the catalog page entitled "Starting Dates, Registration Fees, Tuition and Kit Costs" catalog page 21. EDUCATIONAL COMMUNICATION We solicit feedback from current students yearly in the fall between September and November by having them fill out an evaluation form. We ask for your feedback concerning your education and your experience in the previously completed term. We will use your feedback to help us develop ways to make things better. The faculty members are sincerely interested in your opinions. We work hard to make each course clear and interesting. 12 STATE BOARD EXAMINATION To receive your license you must satisfy the requirements of both the Pennsylvania Academy of Cosmetology Arts and Sciences and the Pennsylvania State Board of Cosmetology. At the Academy, you must complete 1250 hours of operator's training and all required units of education within the curriculum. In addition, for the teacher's exam, you must hold a valid Pennsylvania operator's license and must complete 600 hours of teacher's training and all required units of education within the curriculum. You are required to pass all examinations during these training periods including the "Mock State Board", where we test you on theoretical and practical standards of the Pennsylvania State Board. To fulfill the requirements of the State Board Test you are required to bring a mannequin head and a mannequin hand for the practical portion of the test. For the 300 hours of esthetician training and all required units of education within the curriculum. The Pennsylvania State Board of Cosmetology requires a mannequin with shoulders and décolleté for all esthetics practical examination services. To apply for the State Board Exam you must submit all pertinent forms and payment made payable to Pearson VUE in the form of a cashier s check, money order only. STUDENT CODE OF CONDUCT/RULES AND REGULATIONS We expect all Pennsylvania Academy of Cosmetology students to maintain a professional standard of conduct while attending school. For specific information on Student Code of Conduct/Rules and Regulations, refer to catalog page 22. GRADUATION REQUIREMENTS: In order to qualify for graduation from the Pennsylvania Academy of Cosmetology and receive a certificate, you must complete the following requirements: Acquire 1250 documented legal hours for the cosmetology operator's course or (600 for the cosmetology teacher's course) or (300 for the esthetician course) in the specific areas of training required by the State of Pennsylvania. Graduate with a GPA of 80% or higher and have an attendance average of 67% or higher. Cosmetology operators and estheticians students must complete the required number of clinic units, practical and theoretical segments of the course. Cosmetology Teachers must complete the practical and theoretical segments of their course. Must be paid in full for tuition, supplies and any overage charges prior to graduation. Attend an exit interview and complete all paper work needed to graduate. Also understand that he/she is responsible for the Pennsylvania State Board exam fees and any other examination or licensing related expenses. The school will grant a certificate of degree and an official transcript of grades and hours for the applicable course when the student has successfully completed all phases of study and graduation requirements. 13 TILA Requirements are not applicable. The PENNSYLVANIA ACADEMY OF COSMETOLOGY has approval for a 1250 clock hour program with the DEPARTMENT OF EDUCATION. The cosmetology course is 1250 clock hours long. Our students advance to grade level two (2) after completing 900 clock hours. This allows the student to apply for additional loans prorated for the final 350 hours. REFUND POLICY For Applicants who cancel enrollment or students who withdraw from enrollment a fair and equitable settlement will apply. Applicants not accepted by the school shall be refunded all monies paid to the school. If a student (or in the case of a student under legal age, his/her parent or guardian) cancels his/her enrollment and requests his/her money back within three business days of the signing of the enrollment agreement or contract, all monies collected by the school will be refunded. The cancellation date will be determined by the postmark on written notification or the date said information is delivered to the school administration in person, or the date of the expulsion by the school. If a student on an approved leave of absence does not return on designated return date, they will be terminated and the schedule below will apply. NOTE: The Leave Of Absence time will be counted as part of the six month grace period, for repayments on all loans. Any student who misses 10 consecutive days without notifying the office and providing proper documentation will be terminated. If a student cancels the enrollment more than three business days after signing the contract but prior to starting classes, a refund of all monies paid to the school less the enrollment fee of $ will be made. For students who enroll and begin classes but withdraw prior to course completion (after three business days of signing the contract), the following schedule of tuition will apply: PERCENT OF SCHEDULED TIME TOTAL TUITION SCHOOL ENROLLED TO TOTAL COURSE SHALL RECEIVE/RETAIN.01% to 04.9% 20% 5% to 09.9% 30% 10% to 14.9% 40% 15% to 24.9% 45% 25% to 49.9% 70% 50% and over 100% Any monies due the applicant or student shall be refunded within 45 days of formal cancellation date as defined above. In the case of disabling illness or injury or other documented mitigating circumstances, a reasonable and fair refund settlement may be made at school s discretion. If permanently closed or no longer offering instruction after a student has enrolled, the school will provide a pro rata refund of tuition to the student. If the course is canceled subsequent to a student's enrollment, the school will either provide a full refund of all monies paid or completion of the course at a later time. The school does not participate in any teach-out plans with other institutions. If a Title IV financial aid recipient withdraws prior to course completion, a calculation for return of Title IV funds will be completed and any applicable returns by the school shall be paid, as applicable, first to unsubsidized Direct Student Loan Program; second to subsidized Direct Student Loan Program; third to Federal Pell Grant Program; fourth to other Federal, State, private or institutional student financial assistance programs; and last to the student. After all applicable returns to Title IV aid have been made, this refund policy will apply to determine the amount earned by the school and owed by the student. The State of Pennsylvania requires 1250 clock hours to complete the Cosmetology Course. According to regulations set forth by the Department of Education, the definition of our academic year requires a minimum of 30 weeks and 900 clock hours instructional time. Enrollment time is defined as the time elapsed between the actual starting date and the date of the student's last day of physical attendance in the school. Any monies due the applicant or student shall be paid within thirty days of formal cancellation by the student; or formal termination by the school, which shall occur no more than 30 days from the last day of physical attendance, or in the case of a leave of absences, the earlier 02/14/
14 of the date that the student notifies the institution that he/she will not be returning or the documented date of return. The student will pay a re-entry fee of $50.00 if a student re-enters within one year from termination date. After one year, the same policies will apply, but you will pay $ re-entry fee. This refund policy applies to tuition and fees charged in the enrollment agreement. Other miscellaneous charges the student may have incurred at the institution (EG: extra kit materials, books, products, unreturned school property, etc.) will be calculated separately at the time of withdrawal. All fees are identified in the catalog and in the enrollment agreement. There is no Enrollment Fee charged for second time students who graduated from our school. The termination is done as follows: We are required to first do the Department of Education Return of Title IV Refund Policy (R2T4). There are three (3) payment periods, ( clock hours). The termination will be done on which ever period of enrollment the student is in at that time. The hours they are scheduled to have completed in that time frame is divided by the payment period the student is in. For example, if the student is in their first 450 clock hour payment period and they should have completed 250 clock hours, the 250 clock hours will be divided by 450 clock hours. This results is 56% so, we will have to return 44% of all funding. If the percentage is 60% or more we can keep the full 100% to apply to the student s charges. The second part of the termination is the school refund policy. This is done on the number of hours a student has completed as of the last date of attendance, which will be divided by the number of hours the student has contracted for. For example, if the student completes 254 clock hours and their contract is for 1250 clock hours, we will divide 254 by The result is 21%. According to the termination schedule on all contracts, the student would owe us 45% of tuition, the $ enrollment fee and the cost of books and equipment issued to the student plus 7% for tax and shipping. We will deduct all financial aid that is available for the student to keep and any cash payment that has been made. If there is a credit balance, it will be returned to the lender to reduce the Student Loan; or, if the student is a cash pay, the credit will be issued to the student. All scholarships and Ace Grants; are awarded once a student completes 100% of the course. HOUSING A variety of housing options is available to our students. There are apartments located above the DuBois School, very reasonable priced. Both Johnstown and DuBois admission offices have a detailed housing information package that they will gladly give you. We will assist you in selecting the housing that best suits your needs. COUNSELING (ADVISING) Counseling is available whenever a student needs it. Students are tested and the teacher then records grades in their grade book. At the end of a cycle or the end of the month, they transfer grade averages to the Student Progress Record Master Grade Sheet. The Cycle Evaluation Record records not only their practical and cognitive skills but any difficulty students may have with speed, and customer relations. The instructor will make recommendations for improvement and comment on student progress. The students review their progress and have the chance to discuss any problems with the instructor. If problems arise during a cycle, the instructor would fill out a counseling report and discuss this with the student. If a student has financial or personal problems that get in the way of the students training, the instructor would fill out a counseling report and discuss it with the student as well. If we need to refer a student to a professional, we would record this on the counseling report. You are very important person to us...let us prove it. 02/14/
15 We evaluate cosmetology students for satisfactory progress at 450 hours, 900 hours and at 1250 hours cosmetology teachers at 300 hours and 600 hours and esthetician students at 150 hours and 300 hours. These evaluations record their progress as they continue their education. We evaluate Junior and Senior students, and our Esthetician and Student Teachers at the end of each month or cycle that may run 3 to 6 weeks depending on the cycle. These evaluations record clinic progress, customer relations, and cognitive skills as they continue their education. JOB PLACEMENT DEPARTMENT This Department is dedicated to your success as a professional hair-designer or esthetician. No school can guarantee employment or salary upon graduation. The school will provide you with placement assistance, which will consist of identifying employment opportunities and advising you on appropriate means of attempting to realize these opportunities. STUDENT GRIEVANCE POLICY & PROCEDURE We at the Pennsylvania Academy of Cosmetology take pride in providing the best educational staff, facilities, equipment and supplies. We have a great staff that is willing to resolve any problems that you the student may encounter during your education here. In some cases, you may feel that your instructor did not answer a concern of yours to your satisfaction in those cases you should than take it to the school supervisor. If you feel you still did not receive a satisfactory answer, you should follow the following procedure: 1. The complaint must be in writing to the school President Daniel J Leaser. At that time, you must outline the allegation or nature of the complaint. 2. The President will meet with the complainant within 10 days of receipt of the written complaint. If after careful evaluation, the problem cannot be resolved through discussion, we will refer the complaint to the schools complaint committee. 3. The complaint committee will meet within 21 calendar days of receipt of the complaint and review the allegations. If more information from the complainant is needed a letter is sent outlining the additional information. 4. If no further information is needed the complaint committee will act on the allegations and a letter will be sent to the complainant within 15 calendar days stating the steps taken to correct the problem, or information to show that the allegations were not warranted or based on fact We feel that all problems can be resolved through our schools complaint process. If you still feel after going through the following procedure that your complaint is still valid and wish to pursue the matter further, a complaint form is available through the accrediting agency. You must do this in person because it requires a signature. Leaves of absence are in increments of 30 calendar days, with the option of returning early. You are given a return date if you do not return on that day you would terminate your contract. The exact days a student is on a leave of absence will extend the student's schedule and contract. A leave of absence can extend up to a total, maximum 180 days due to certain circumstances. 02/14/
16 ATTENDANCE The classes at the Pennsylvania Academy of Cosmetology are from 9:00 am to 3:30 pm. (Monday - Friday) Saturday classes are from 9:00 am to 2:00 pm. We inform the student on the first day of class of the policy procedure. This policy appears in the Rules and Regulations #6. The instructor is responsible to see that her class follows the policy. TARDINESS POLICY & PROCEDURE Students arriving from 9:01 until 9:15 am cannot sign in until 9:30 am. Students arriving after 9:15 cannot sign in until 10:30 am. A student not signed in cannot be on the on the premises. Any student arriving after 12:30 would not be able sign in at all because students must be here for a total of 3 hours in order to receive credit for those hours. Students who must leave early due to illness or emergency will receive credit for the hours attended EXCUSED ABSENCES Death in the immediate family is spouse, parent, stepparent, child, stepchild, brother/sister, stepbrother/sister, grandparent, grandchild and in-laws. Please bring an obituary when you return. Mandatory Appointments (Example: Court Hearing) Weather Conditions (See Student Code of Conduct/Rules and Regulations #34 Weather Policy) MAKE-UP POLICY When you miss any scheduled classes it is your responsibility to meet with your instructor and see what you have missed. He/she will be able to help you set up a time to do your make-up. If you miss more than three days, in a basic cycle you will need to repeat that cycle. The next time we offer the cycle, we will try to schedule you for that class. This is why your attendance is so important. Late arrivals and absences of training classes could and will cause you to go over your contract date. Effective July 1, 2011 that hourly rate will be $12.00 for operator s course, and $11.50 for the teacher s course, and esthetician course $ EMERGENCY RESPONSE POLICY The following policy and procedure will be used to notify students and staff of a dangerous situation in the school. Any instructor/staff member can initiate an alert. Code 1: Is a lock down situation, students and staff are to remain calm and not leave the classroom or building. Code 2: Indicates the need to evacuate the classroom or building immediately and exit through the closest exit. Code 3: Indicates the need to evacuate the classroom or building immediately and exit through the back exit only. Code 4: Indicates the need to evacuate only through the front entrance. The Supervisor/or person in authority will notify the police who will notify the neighboring community of the impending danger. The instructors/staff members will make sure all students and clients follow instructions. Instructors: 1. If you must evacuate take grade book and student sign in sheet with you outside the building. 2. Take attendance to insure that no one is left in the building. 3. Be calm and reassure the students of their safety. 4. Clinic instructor must make sure that all clients are safely outside the building. FAMILY EDUCATIONAL RIGHTS & PRIVACY POLICY RELEASE OF INFORMATION Each student enrolled or graduated from Pennsylvania Academy of Cosmetology (or a parent or guardian, if the student is a dependent minor) can have access to the contents of the financial aid or academic file. In an instance, such as great distance where it is impossible for personally inspecting records, the school will 02/14/
17 provide copies (by written request.) Once a student is 18, the parents only have access with written consent from the student for each request, regardless of whether the student is a dependent. Eligible students or parents/guardians have the right to request that the school correct inaccurate or misleading information. The school provides a release form for eligible students or parents/guardians to sign as written permission for release of information from a student s record. However, the school may disclose without consent, to the following: School employees who have a need-to know; Other schools to which a student is transferring; Certain government officials in order to carry out lawful functions; Appropriate parties in connection with financial aid Accrediting organizations or organizations doing specific school studies; Individuals who have obtained court orders or subpoenas; Persons who need to know in cases of health and safety emergencies; State and local authorities to whom disclosure is required by state law. The school may also disclose without consent, directory-type information such as name, address, telephone, date and place of birth and dates of attendance. The school will advise which information is intended as directory information and the student or parent/guardian may or may not waive the right to release. DRUG AND ALCOHOL ABUSE Pennsylvania Academy of Cosmetology complies with all federal, state and local laws including those that regulate the possession, use, or sale of alcoholic beverages and controlled substances. Commonwealth of Pennsylvania law states that you must be at least 21 years of age in order to purchase, sell, possess, or consume alcoholic beverages. The use, possession, purchase and/or sale of illicit drugs on campus poses a threat to the health and safety of all members of the Pennsylvania Academy of Cosmetology and it is not permitted on campus property or as part of campus activities. Violations will be investigated and criminal prosecution may result. 22 STUDENT CODE OF CONDUCT/RULES AND REGULATIONS Professional conduct is the only level of conduct we expect from our students. Tardiness, discourtesy, and improper grooming are not traits of a professional. The following rules reflect the type of conduct and appearance you are to maintain throughout your career. Violation of these rules may result in your dismissal. Day Classes - 9:00am to 3:30pm - Monday through Friday Saturday's 9:00am to 2:00pm New Years Day Memorial Day July 4 th Labor Day The Academy closes on the following days: Thanksgiving Day and the day after Thanksgiving Christmas Days (determine yearly) Annual New York Trip 1. Students have two 10 minutes breaks, and 30 minutes for lunch each day at their scheduled time. 2. You must wear a clean uniform daily. Female students may wear black pants or skirts with all black shirts. Hair, make-up and nails must be professional looking at all times. 3. Male students must wear black trousers and all black shirts. PLEASE SEE DRESS CODE POLICY FOR MORE DETAILED INFORMATION. 4. At all times, sign in upon entering the classroom and sign out upon leaving. This applies to lunchtime also. The sign in procedure is necessary to meet State Board requirements for recording the number of hours attended. It is the sole responsibility of the student to complete this procedure daily. Signing in for another student is forgery. You must notify your instructor to sign out early. 5. Computed hours are posted monthly. Students have one week to check hours and inform the office of corrections. Documentation on the sign in sheet is the only valid proof of attendance. If you miss your scheduled classes, it is your responsibility to meet with your instructor, to see what you have missed. NOTE: See Make-up Policy on page The administrative office must approve any schedule other than a full time 30-hour week. 10. Students complete a minimum of 20 minutes sanitation daily. Each student is responsible for his/her work area. 11. Students must attend theory classes each week. 12. All students must have approved equipment and textbooks with them at all times. The Academy is not responsible for lost or stolen property. All equipment should be marked. We provide you with a locker you must provide your own lock. All kits must remain in the school. Students without equipment or books cannot attend class. 13. Keep coats and boots in the places provided they do not belong in the classrooms. 14. We will not tolerate profanity in the classrooms or on the clinic. It may result in loss of time. 15. Students cannot perform services without a signed release form. We offer services and products to you at student discount instructors will initial these slips. 16. Junior students are required to complete five services daily. Senior students are required to complete seven services daily. Students must be able to perform their services in a classroom or on the clinic floor, if for any reason you cannot; you not permitted to attend class. 17. Pennsylvania does not permit drugs or alcoholic beverages on the premises of any cosmetology school. We will send any student suspected of being under the influence while on the premises for a blood test 02/14/
23 at the Academy's expense. We will dismiss you if you refuse this procedure. NOTE: A student in violation of drugs or alcohol a locker search is permitted by law 18. This is a professional career; we do not permit gum chewing. You can only have food and drinks in the break area. 19. NO SMOKING permitted in the school. 20. Any student involved in stealing from the Academy, staff or fellow student will result in immediate dismissal. NOTE: If suspected, law permits a locker search. 21. Any student guilty of willful destruction of Academy property will result in immediate dismissal. 22. Disrespect or direct violation of an instructor's directions is cause for suspension. 23. Students are permitted one sixty-day leave of absence. Students must notify the office, and sign necessary papers. A longer leave would result in termination, and a $50.00 re-entry fee would be due upon return, as long as the student returns one year from date of termination. $ re-entry fee will be due upon return, for anyone returning beyond one year. 24. During a leave of absence, you must vacate your locker and your instructor must check your locker. The Academy is not responsible for any belongings left in lockers 30 days after a leave of absence. 25. You must maintain Satisfactory Progress Standards in an accumulative grade average of 80% of combined academic and practical evaluations. See page We test you following the completion of each chapter, or DVD. Students must maintain an 80% or above average. You need to have your Assignments completed before 1250 hours. We evaluate students at 450 hours, 900 hours, and 1200 hours (If a student is in an incomplete cycle this evaluation can be done anytime between 1200 clock hours and 1250 clock hours). Students MUST maintain satisfactory progress to be eligible for Title IV funds. 27. You must pay your tuition promptly according to your contracted enrollment date. The Academy reserves the right to interrupt training when a student is behind in payments. You must pay your account in full before you reach 1200 hours or you cannot complete the required hours necessary to take the State Board Exam. 28. In order to file for the State Board exam, you must meet the following requirements: Complete 1250 hours (operators), 300 (esthetician), and 600 hours (teachers) of documented training in the correct areas required by the State of Pennsylvania. Teachers must submit a copy of their cosmetology license If your name on high school transcript, GED, or diploma differs from your name on the Application, you must provide legal documentation. Complete all necessary requirements and have a GPA of 80% or higher. Submit fees with the Application and payable to Pearson VUE in form of a cashier s check, money order only. No personal checks accepted. 29. All students must complete their freshman, sophomore, Junior, and senior cycles stated in your contract to graduate. 30. After graduating, you may continue to attend school for theory and practice at no charge until you take Boards. You must be in uniform. 31. No student can take personal telephone calls during school hours. In case of emergency, the desk receptionist will take messages to return calls and immediately give that message to the student. The office personnel are very busy so please do not abuse this privilege. Cell phones must be off; you cannot use them during class. 32. We request that the students use the rear entrances to enter the school. Parking in a reserved area will result in towing. 33. Please give your name at the desk if you need to see anyone in the administrative offices. 34. WEATHER POLICY: Due to the areas our students come from, the academy may close or delay due to weather conditions. That determination; is based on talking to staff in the North and South of the schools. Example: If it is an hour delay, we expect you at the school by 10:00, 1 ½ hours 10:30, 2 hours 11: Any Student who misses 3 unexcused scheduled school days within the first 30 calendar days of their start date will be terminated. This change is being implemented to provide a classroom environment conducive to providing a positive learning experience to all of our students. 02/14/
