Regency Beauty Institute Our Facility & Equipment Each Regency campus is an expression of Regency's image and educational philosophy. This philosophy is based on rewards for excellence, personalized guest attention, and intense hands-on service and retail product knowledge. The campus is approximately 6,064 square feet and contains a modern Performance Floor where students, once they qualify, perform services on guests and clients in a high-end salon setting. In addition, the campus contains a reception area, two classrooms, service areas for performing hair, nail and skin services, student lounge, student support kiosks, and staff offices. Equipment in the campus includes, but is not limited to, salon work-stations with styling chairs, shampoo bowls, hair dryers, nail tables, pedicure stations, and esthiology beds. The campus also includes a student break room and student storage lockers. Smoking of any type, including e- cigarettes, is not permitted in the campus. Smoking is only permissible in designated areas outside of the building. 1

It s the best way to experience what makes the Studio Luma way to learn cosmetology different. And, why that matters. On the tour, watch how our teachers and students use each part of the campus. Then get to know us. Meet with the campus staff and ask questions. Admissions Requirements Regency generally begins new students every 4 weeks and accepts applications on an ongoing basis. You may begin the application process by requesting information through our website at or by visiting the campus. Regency representatives will discuss the admissions process and requirements with you and encourage you to schedule a campus tour. Our representatives will guide you through these steps:! Submit documentation showing that you are at least 16 years of age (birth certificate, driver s license, state issued ID, or passport)! Submit documentation demonstrating award of a high school diploma or an equivalent educational attainment (high school diploma or transcript, GED certificate) 2! Sign a completed Enrollment Agreement (once you have selected a start date). 3! Complete any forms required by state rules or regulations. 7 as defined by local, state or federal law. Rather, we make admissions decisions based on merit, including each applicant s interest, aptitude, and ability. Applying as a Transfer Student Regency does not recruit students admitted to or currently attending another school offering a similar program of study, but we do accept applications from prospective transfer students. If you wish to request transfer credits based on prior enrollment in a third-party cosmetology program, you must provide an official transcript from that prior cosmetology program prior to enrollment with Regency. 4 The transcript must include a detailed listing of all hours, courses, tests, and practical services completed, as well as of grades received, in your prior program. The required documentation must be fully received and a determination issued before you enroll with Regency. This section sets forth Regency s general policy governing admission as a transfer student. However, this policy varies in certain states due to the requirements of state law. Upon submission of your transcript from your prior cosmetology program, Regency will provide you with information regarding any additional or different requirements that may apply in your state. Prospective transfer students are responsible for identifying and fulfilling all applicable state requirements for transfers. We will review the transcript and grant you transfer credit where, in our judgment, your prior experience satisfies our program requirements. Regency will accept up to one-half of the program hours required by the state in which you wish to attend our cosmetology program. We may accept more than one-half of the program hours if we have entered into a teach-out agreement with another school. We will generally accept 100% of hours earned from other campuses doing business as Regency Beauty Institute. 5 Regency s curriculum consists of courses and classes, with each course consisting of four classes. Course requirements and guest service eligibility are determined at Regency s discretion based on the number of transfer hours granted. o o If you are granted less than 260 hours of transfer credit, you must complete all requirements in courses 1 and 2, in addition to any other requirements under state law, prior to performing services on guests If you are granted more than 260 hours of transfer credit, you may take an evaluation which, if passed at 80%, will allow you to be eligible to perform guest services. A student who wishes to be guest service eligible must complete the evaluation prior to his or her first scheduled day in the Regency program. Additionally, transfer students may receive credit for individual practical service requirements completed at another school that, based on a review of the student s transcript, appear to cover substantially similar practical service requirements included in the Regency curriculum. Regency does not grant specific transfer credit for tests or evaluations completed in a prior program. Because tests and technical evaluations in Regency s program are tied to classes, if a class is assigned (based on transfer hours granted), then all tests and technical evaluations associated with that class are also required. If you have an active esthetician or nail-technician license, or if you have previously attended an esthetician or nail-technician/manicuring program, you may receive transfer credits for a maximum of 100 hours in our program. 4 Occasionally, enrolled students may wish to request transfer credits after enrollment and/or to be evaluated for guest eligibility after the deadlines stated in the policy. Such requests are for a policy exemption and must go through our appeals process, described in the Grievances and Student Appeals Section of this catalog. If the appeal is granted and transfer credits are approved, an amendment to the Enrollment Agreement will be required. (No appeal is required, however, for approval for transfer credits from a high-school cosmetology program, assuming the request for transfer credits is received by Regency prior to the student s start date.) Please note that Regency will not consider a request to transfer hours from a third-party school for a student who has completed two courses in the Regency program. 5 Regency reserves the right to deny a request to transfer from one Regency campus to another Regency campus based on (i) differences in the curriculum or program between the two campuses; or (ii) the student s progress, including attendance, academic, or program standing, or (iii) the student s disciplinary record. 3
8 In addition, if you have an active esthetician or nail-technician license, you may not perform services on guests for which you are licensed. We reserve the right to deny transfer credits from third-party schools. Additionally, we will deny all transfer credits if any of your hours, tests, and/or practical services from your prior program were completed more than 4 years prior to the date that we receive your request for transfer credits. Likewise, in the event that you withdraw or are terminated from Regency and later re-start, Regency will repeat this review and will deny all transfer credits if any of your hours, tests, and/or practical services from your prior program fall outside of the 4-year period as of the date you requested to re-start. Transferring to another Regency Institution Regency has over 80 campuses in 20 states. If you move or otherwise need to transfer to another Regency campus after starting the program, Regency s Student Services Department will work with you to try and accommodate your request. There are limits and restrictions on transfers between Regency campuses (for example, students may not transfer before course two and students in the Studio Luma program may not transfer to a campus offering a different cosmetology program). For more information about transferring between Regency campuses, please contact a student services specialist. Transferring to a non-regency Institution Regency cannot guarantee that hours earned in our program will be accepted as transfer hours by another institution. Transfer of hours is at the discretion of the accepting institution. It is the responsibility of any student seeking to transfer to confirm with the other institution whether or not it will accept hours from Regency. Occupational Demands In keeping with our commitment to equal opportunity, no student will be denied admission based on membership in a protected group. At the same time, it is our mission to teach our students the skills necessary for success in the beauty industry. In order for you to meaningfully participate in the program and receive the full benefit of the education delivered, there are certain physical demands that you must be able to meet. Specifically, to perform the essential requirements of the cosmetology curriculum, you must have the physical ability, with or without a reasonable accommodation, to: Tolerate exposure to and handling of products having or emitting odors, fragrances, and chemicals Stand and walk for up to 8 hours consecutively, except during 15-minute or 30-minute breaks Manually grasp, handle, control, move, and manipulate objects or tools standard within the beauty industry Bend or twist the body, and engage in repetitive motions, as needed to perform and demonstrate the full range of services standard within the beauty industry Hold the arm and hand in one position and, at other times, hold the hand steady while moving the arm Lift up to 30 lbs Perform and demonstrate the full range of esthetics related services standard within the skin care industry (e.g., facial and body skin care with massage, facial and body waxing, and make-up application) View detail in objects within arm s length and discern the differences between colors, shades, and brightness within and between those objects Perform and demonstrate the full range of hair-related services standard within the cosmetology industry (e.g., shampoo with massage, comb, haircut, hairstyle, hair color and chemical texturing, braid, sculpt, hairdryer, etc.) Perform and demonstrate the full range of nail-related services standard within the cosmetology industry (e.g., file, buff, clip, clean, and apply polish to nails and/or attach artificial nail products) Any student who may require an accommodation of disability in order to meet these occupational demands should carefully review the Academic Adjustments Based on Disability Section of this catalog. 9 Academic Calendar Regency s cosmetology program runs year-round and we generally start a new class of students every month. The following list shows the start dates for new classes in Start Dates 1/5/15 2/2/15 3/2/15 4/6/15 5/4/15 6/1/15 Full Time 7/6/15 8/3/15 8/31/15 9/28/15 10/26/15 11/23/ School Breaks To accommodate our students and their families and friends, Regency will be closed on the following dates: Winter break Monday-Sunday, December 22, January 4, 2015 Spring break TBD (1 week) Memorial day Monday, May 25, 2015 Summer break Monday Sunday, June 29 July 5, 2015 Labor day Monday, September 7, 2015 Thanksgiving holiday Thursday, November 26, 2015 Winter Break Monday Sunday December 21, 2015 January 3, 2016 The class time for this holiday will be fulfilled during scheduled hours required on the subsequent Saturday. Severe Weather and School Closings A voic message will be sent to your primary phone number informing you of school closures due to inclement weather or other school events. A notice will also be placed on the campus website page. Students will be required to fulfill course obligations by attending a required session on one or more weekend days, or during make-up time, following the closure. Student Financing 2015 Program Tuition & Costs The total tuition and fees for the school s cosmetology program are as follows: Tuition & Costs Tuition $13,550 Kit Fee 6 $ 1,425 TOTAL $14,975 The kit includes various cosmetology supplies and tools that will be used throughout the program. Students are required to have all items in the kit for program participation, but may obtain a list of items from the campus manager and purchase them from an outside vendor of their choosing. Students are responsible for maintaining their kit items, and will be charged the current market price to replace any lost/damaged item. Students must pay the full tuition and kit fee before starting the program or in accordance with Regency s interestfree monthly payment plan. 6 Appropriate state sales tax will be applied to the kit cost. 5
10 Regency accepts payment in the form of personal cash, check, cashier s check, debit card, and credit card (Visa, MasterCard, or Discover). Payments by credit card must be made through Regency s website or by telephone to Regency s business office. Payments by check must be mailed to addresses provided upon invoicing. If no invoice is provided (for example, a payment to remove a suspension for non-payment), payment by check may be made directly to the campus. All checks must have a student ID# clearly written on the check. All checks returned for insufficient funds will be subject to a NSF fee of $25. There are no other further charges, such as instructional supplies or equipment, service charges, activities fees, deposits or locker rental which have not been disclosed above. State Exam and Licensure Fees Regency pays all the legally required fees for a graduate of its Studio Luma cosmetology program to (a) take the state licensing exam(s) one time, and (b) obtain an initial cosmetology license. In order to benefit from this policy, the graduate must schedule to take the relevant exam(s) within three months of graduating from the program and also advise his/her career coach of the testing date(s). Regency asks that the graduate provide all test results to his/her career coach within one (1) week of receiving them.. Federal Financial Aid On the first day of class, or earlier if requested, students will receive information on how to complete the Free Application for Federal Student Aid (FAFSA) and information about whom to contact with questions about the financial aid process. Federal financial aid is available to those who qualify. The financial aid application process has 4 main steps:! Complete a Free Application for Federal Student Aid (FAFSA) at Turn in any documents requested by financial aid office. These may include federal tax return transcripts and financial information concerning you or your parents! Review and accept the aid offered, if you wish, with our financial aid office! Complete the federal loan paperwork online as directed, if applicable Regency s financial aid department and your campus manager will be available to guide you through the process. Refund and Cancellation Policy A. This refund policy applies to all terminations and withdrawals for any reason, by either party, including student decision, program cancellation or school closure. B. If an applicant is not accepted by the School, or the Student s Program is cancelled before the Student starts the Program, the Student is entitled to a refund of all monies paid to the School. C. NOTICE OF CANCELLATION: A Student (or in the case of a Student under legal age, his/ her parent(s) or legal guardian(s)) has the right to cancel this Agreement in writing and receive a refund of all monies paid until the Student s Start Date or the fifth (5 th ) business day after signing this Agreement, whichever is later. The postmark on the written notification or the date said information is delivered to the School in person will determine the cancellation date. D. If the Student is new to the Program (i.e. not restarting in the Program) and withdraws or is terminated from the Program during the first (1 st ) week, the School will retain/charge the full kit fee but refund all tuition. E. If a Student withdraws or is terminated after the first (1 st ) week of the Program, the School will retain/charge the full kit fee (the kit becomes the property of the Student), and retain/charge the Student tuition for a percentage of the Program using the following tuition refund policy. F. Tuition refund policy: The School will retain a percentage of tuition charges based on the number of courses a student attended (calculated using the student s cumulative actual hours in the program), as follows: Course(s) Attended Percentage of Program Completed Percentage of Tuition Owed to the School to 40.00% Percentage of program hours actually completed to 50.00% 50.00% 6

Termination of Enrollment: Student s enrollment will be terminated if the Student: o exceeds 14 consecutive calendar days of absences (excluding scheduled School breaks); o fails to return as scheduled from an approved leave of absence; o withdrawals from the Program; o makes unsatisfactory progress in the Program; o fails to make timely payment of tuition; or o fails to comply with established School rules and policies. H. The refund calculation will be based on the last date the Student attended the Program. I. The official withdrawal or termination date for a Student on an approved leave of absence is the earlier of the date of expiration of the leave of absence or the date the Student notifies the School that he/she will not return to continue the Program. J. Pursuant to the Code of Federal Regulations, 34 CFR Section of the Higher Education Act Section 484B, Return of Title IV Funds, the School must calculate the amount of Title IV funds the Student has earned at the time of withdrawal or termination from the School. Title IV funds are funds received from one or more of the following programs: Pell grant, FSEOG, or Federal Direct Loans. This amount is based upon the number of School hours the Student is scheduled to complete in the payment period. After 60% of the payment period has been completed, 100% of Title IV funds may be retained. The School must return any portion of unearned Title IV funds for which the School is responsible within 45 days. The School will advise the Student of any amount of unearned Title IV grant aid that must be returned by the Student, if applicable. K. Refunds to the Title IV programs and others will be disbursed in the following order: 1) Federal Direct Unsubsidized Stafford loan 2) Federal Direct Subsidized Stafford Loan 3) Federal Direct Plus Loan 4) Federal Pell Grant 5) Federal SEOG 6) Other Title IV aid. L. If applicable, the Student will receive the proportional share of any remaining refunds (after Title IV distribution) from non-title IV programs (i.e. state grants, scholarships, other agency grants). M. All monies due the Student will be refunded within 30 calendar days of his/her official withdrawal or termination date. N. In the event of a permanent school closure, the Student will receive, at the School s option, either a teach-out at a comparable school or a pro-rata refund of his/her Tuition. O. Under certain circumstances, the School may choose to enter into modified Tuition payment arrangements with a Student. In order for the School to consider a modification, the Student must submit to the School a written request of the modification sought, the circumstances supporting the request, and any supporting documentation. Cosmetology Program Name of Program Cosmetology Program Educational Objectives Students will be provided a curriculum that includes theory and practical experiences in all phases of cosmetology necessary to meet licensing requirements Prepare students to pass the required written and practical state exams To teach entry-level skills and efficiency needed to work in a professional salon or related occupation To develop employer-employee relationships, effective communication and client-building skills Program Description The full-time program consists of ten (10) four-week courses, each of which is 120 hours (for a total of 1200 clock hours. 7 A student with perfect attendance (through attending scheduled class days and/or makeup time) will 7 Florida law defines a clock hour as a period of 60 minutes with a minimum of 50 minutes of instruction in the presence of an instructor. 7
12 complete our program in 10 months, which includes up to 4 weeks of scheduled breaks. The normal time to complete the full-time program is 12 months. Courses have time dedicated to study theory and hands-on practical work (see below). The program is designed so that courses build upon one another. Students apply what they learn in each course towards the remaining courses and through guest service on the Performance Floor. Instructional Methods The cosmetology program is delivered in a progressive, individualized format. Teachers guide students through classes with support of an ipad-based Filmbook. Students demonstrate their knowledge of cosmetology theory and technical skills through practical and clinical activities, graded technical evaluations and Filmbook class tests. 8 Grading Students will complete Filmbook class tests in each course, and technical evaluations throughout the program, which are conducted by a teacher. Technical evaluations are conducted as part of a guest service and are graded as pass/fail. The required criteria and minimum passing score are set forth in the technical evaluation process. A student is awarded points for each criterion that is performed properly. A student will receive a pass if he/she passes sanitation requirement and the total score on the technical evaluation form meets or exceeds the minimum score. Students are required to complete a total of four (4) Technical Evaluations in the program. Filmbook class tests are completed at the end of each class to demonstrate the knowledge gained in that class. Class tests are scored on a 100% scale. Students must achieve a score of 80% or above on the test to successfully pass the class. If a student does not pass a class, they must retake the test (with different questions), until achieving a passing score. Students will take 4 Filmbook class tests in each course, for a total of 40 tests in the program. Students are expected to retake technical evaluations and tests until they have earned a passing score. Students must pass these requirements in order to complete each course. The school maintains grade records as part of your electronic student record. Grading Scale Satisfactory 79 and Below Unsatisfactory Program Materials While in the campus, students are provided with a Regency-owned Apple ipad which contains Regency s cosmetology Filmbook. The Filmbook includes live-action film scenarios, a cosmetology e-textbook and exercises. Students also receive a copy of the current Florida cosmetology statutes and regulations. Additional reference materials are available for checkout through the school library, including printed copies of the e-textbook procedures, an unabridged dictionary, a thesaurus, the Milady Standard Cosmetology Textbook and Study Guide (2008 Edition), the Milady Standard Cosmetology Exam Review, and Regency Technical DVD Set. Course Requirements In order to successfully complete each course, you must satisfy the following requirements: Clock 120 hours; Successfully pass any technical evaluations (graded as pass/fail only) for the course; Complete all required tests and achieve at least an 80% grade point average on each test; and 8 The program is taught in English and Regency does not offer remedial courses or courses in English as a second language (ESL). 8
13 Successfully complete all service requirements for the course (see Appendix 1: Service Requirements by Course). As a student, you will be given ample opportunities to make up work and stay on track to finish your program on time (see Makeup Policy Section below). In the event you do not complete all program requirements within the time allowed in your agreement, you will be terminated, with an option to appeal to request a restart with a new enrollment agreement. Course Numbering System Regency Beauty Institute measures the length of its programs in clock hours; and follows an independent course numbering system to assign courses in accordance with program requirements. The system is designed to identify courses and differentiate the level of study. Courses are numbered sequentially and assigned a letter prefix depicting the program title. Each student is assigned to attend one of the following rotation schedules: Schedules for Each Course Rotation A Week 1, 3 Week 2, 4 Rotation B Week 1, 3 Week 2, 4 Hours Per Week Scheduled Course Hours Monday Thursday, 9:00 4:30 Monday Friday, 9:00 4:30 Monday Friday, 9:00 4:30 Monday Thursday, 9:00 4:30 Students are assigned to a rotation based on space availability. All students are scheduled for 6.75 hours of instruction per day and are expected to clock out for a 45-minute lunch each day. The school office and library are open 9:00 a.m. 4:30 p.m. Monday Friday. Graduation Requirements To graduate from the program, receive a diploma in Cosmetology Arts and Sciences and final transcript, and be eligible for the state license examination, you must: 1. Complete 1200 clock hours (including both hours actually clocked at the School plus any approved transfer hours, if applicable); 2. Successfully complete the following requirements in each of the ten (10) courses: a. Clock at least 120 hours; b. Successfully pass any technical evaluation (graded pass/fail only); c. Achieve at least an 80% score on each of the four (4) tests; and d. Successfully complete all practical service requirements; 3. Satisfy all financial obligations to the school; and 4. Pass a final skill certification with a score of 80% or above. 10
15 Final Skills Certification All students are required to take the Regency final skill certification as part of the program completion and licensure requirements. A student must score at least 80 percent on the final skills certification exam to pass. A student may take the final skills certification exam any time in their 10 th course. Students must sign up for a scheduled school final skill certification with the campus manager at least two weeks before the exam date. There is no fee to take the final skills certification. Measuring Program Progress Student Progress and Consultations Regency measures student progress following each course against all course requirements, including time clocked, technical evaluations passed, academic average and services completed. At designated points in the program, the campus manager (or a teacher) reviews each student s progress in the program. During the review, the campus staff member provides the student with a copy of his/her progress report, and another copy will be placed in the student s permanent record. Students also have the ability to view and print their progress daily at the student computer kiosks in the campus. Each student s record is updated nightly. Additionally, if the teacher or campus manager believes that a student may benefit from outside resources (for example, suicide prevention or domestic abuse advisors), he/she may provide that student with a list of local thirdparty resources that may be able to provide assistance. A list of outside resources is also posted in the campus. Program Standing Policy To maintain good program standing, students must complete the requirements for each course during the 4 weeks that the course is scheduled. Academics. Each test must be passed with a minimum score of 80%, and students cannot progress in the course until each test has been passed. A course that is not completed during the scheduled time period will be marked in the student s record as incomplete. Attendance. If a student clocks less than 100-hours, then he/she will be placed on remediation and cannot advance to the next course. If a student clocks more than 100 hours but less than the required number of hours in any course, that course will be marked incomplete. Incomplete Courses. If at the end of a course a student has not completed all the course completion requirements, then that course will be marked incomplete and the student must make up the unfinished requirements before the end of his/her next course. If a student fails to complete all the requirements for the incomplete course by the end of the next course, then that student will be placed on remediation and cannot advance to the next course. Further, any student that clocks less than 100 hours in any course will be placed on remediation. If a student has not completed all program requirements at the end of his/her last scheduled course, then he/she will be placed on remediation and will have four (4) additional weeks (8 additional weeks for part-time) to complete all the remaining program requirements. If he/she fails to complete the requirements during that time period, he/she will be terminated. Remediation. If a student is placed on remediation, he/she cannot continue to the next course and must make up all outstanding course requirements within the next 4 consecutive weeks. A student on remediation may attend school during his/her normal scheduled time until any outstanding requirements from incomplete courses have been fulfilled. Once those outstanding requirements have been met, the student must wait for the beginning of the next scheduled course to continue attending. If necessary, Regency will place a student on an administrative leave during this period to avoid a 14-day termination. Any student that fails to complete the outstanding requirements during the remediation period will be terminated from the Program. 11
16 Unfinished Courses. If a student receives an approved leave of absence in the middle of a course, his/her clocked hours, services and academics for that course will be suspended and reactivated when he/she returns from the leave. That student would return from a leave at the same point in the Program as where your left. If you miss one or more entire courses during a leave, you will complete the courses in the order they were missed. For more information on how leaves are handled, please see the Student Leaves section below. Appeals. Any student that is terminated under the Program Standing Policy may file an appeal to the Student Appeals Committee requesting permission to restart in the Program. If the appeal is granted, the Student Appeals Committee may impose conditions on the restart to ensure that the student will regain satisfactory Program Standing status. Additional information regarding appeals is set forth below in the section titled Appealing Terminations. Time & Attendance Clocking Time In order to receive credit for all time attended, students must record their own time using a computerized time clock. Regency uses this system to track student attendance and update student records daily. Each student must clock in and out as appropriate to record time in school and to exclude time periods that do not count for attendance purposes. The time clock measures time in increments of hundredths of hours. Students have a 45-minute meal break and two 15-minute breaks each day. The following procedures apply to clocking time for breaks: Students must clock out during the 45-minute meal break and may leave the school. Students in each setting will generally take a lunch break at the same time. Unless otherwise directed, all students are expected to take their breaks at the designated time. Students will not clock out for 15-minute breaks and must stay in the school building. If your learning setting goes to their assigned meal break you must clock out and return after 45 minutes Attendance Policy Students are expected to attend all scheduled school hours according to the Course Schedule section of this catalog. If a student is absent, comes late, or leaves early and miss time during his/her scheduled hours, this time is counted as absent time. Teachers will not repeat class materials to accommodate absent or tardy students. Each student is responsible for any missed content. Please see the Makeup Policy section below for details. Makeup Policy When a student misses scheduled time in the program, it is that student s responsibility to obtain a list of missed in-class assignments and materials from his/her teacher and make up the work. Failure to makeup work in the time frame as described under the Program Standing Policy may lead to remediation or termination from the program. For full-time students: We offer the following opportunities to work with a teacher in the classroom to make-up hours, services, or other requirements as needed: Friday Makeup Days. Every other Friday from 9:00 4:30 is designated as an optional makeup day. Evening Makeup. Monday evening from 5:15 8:30 is designated as an optional makeup session. Space during makeup time is available on a first-come, first-served basis in accordance with the required instructor to student ratio of 1:25, and students who sign up for makeup time will receive first priority. Participation is limited to students who require additional time to meet their current course completion requirements or have incomplete courses. 12
17 14-Day Absence Policy If a student misses fourteen (14) consecutive calendar days (excluding days for an approved leave and scheduled school breaks) that student will be terminated from the program, with the option to appeal. In the event that a student is suspended from the program for disciplinary or financial reasons, the suspension days are counted in calculating days absent under this policy. Student Leaves For absences students may be eligible to request an approved leave of absence for the following reasons: Medical leave (maximum of two leaves (2), and no less than five (5 school days, when approved and documented for one or more of the following reasons: o The student s own medical condition; o To care for a child, spouse, domestic partner, or parent with a medical condition; o The birth, adoption, or foster placement of a child (provided that the leave begins within 12 months of the qualifying event) o The death of a child, spouse, domestic partner, parent, sibling or grandparent Military duty or jury duty (administrative leave) Students are strongly encouraged to call Student Services 30 days before the start of a desired leave or, where that is not possible, as soon as the need for the leave is known. Student Services will assist with preparing a request for an approved leave of absence and determining what supporting documentation is required, such as a medical certification, a birth certificate, an adoption order, obituary, etc. In cases where the need for a leave is unforeseeable (for example, a sudden illness), students must contact Student Services within 7 days or as soon as circumstances otherwise permit. Each student may request up to two (2) leaves for approved and documented reasons. Students may not take a leave during course one. Any student that requires an absence of a week or more during course one will be required to withdraw from the program. If a student is granted an approved leave, he/she must sign an amended enrollment agreement which will extend the agreement end date by the same number of days as the approved leave. After an approved leave, students come back to school on the Monday of the same week in the course where they left off. For example, if a student s leave begins in the middle of week 2 of a course, that student will return on the Monday of week 2 of the next scheduled course. The maximum combined duration of all approved non-administrative leaves cannot exceed 84 calendar days within 12 months. If a student does not return to school immediately at the end of an approved leave, that student will be terminated. During an approved leave, students will not receive disbursements of Title IV loan funds (i.e., Stafford loans or PLUS loans), as applicable. Regency reserves the right to temporarily suspend its leave policy. Should such a need arise, Regency will provide students at least thirty (30) days advanced notice of its intent to suspend this policy. Administrative Leaves Regency reserves the right to place a student on an administrative leave under circumstances justifying such a leave. For example, when you return from a leave of absence, Regency may place you on administrative leave until the week you will begin that course (the same week in the course where you took the leave). Restarts Applying to Restart after Withdrawal or Termination Any student that wishes to restart in our cosmetology program must call Student Services to initiate the process. We will first determine whether the student is eligible to restart based on his/her rate of attendance, academic average and other objective metrics or credentials. If the student is eligible, we will review the student s record, including any disciplinary history and the reason for the student s termination or withdrawal (as applicable), in order to determine whether the request to restart may be granted without an appeal or must be submitted to the 13
18 Student Appeals Committee in accordance with the Appealing Termination section of this catalog. We retain discretion to halt all restarts at the school and also to grant or deny any individual request to restart for any reason, based on the totality of the circumstances. Students that are permitted to restart will enter the course on the Monday of the week they withdrew or were terminated. A student who restarts at Regency after a withdrawal or a successful appeal from a termination: " Retains his or her prior attendance and academic history, including, but not limited to, his or her Program Standing and SAP status " Returns at the same SAP status he or she had prior to leaving the program unless the restart is granted on a SAP appeal, in which case they will return at a SAP status of probation. " Retains his or her prior disciplinary history. " Must pay any balance owed to Regency prior to restart, unless other payment arrangements have been made. " Must verify his or her financial aid status with Regency s financial aid department. This process can take up to 3 weeks and will be initiated by a student financing representative once the student s restart eligibility has been confirmed. " Must sign a new Enrollment and Dispute Resolution Agreement ( Enrollment Agreement ). The new Enrollment Agreement will supersede the student s previous Enrollment Agreement(s) and the terms and conditions of the new Enrollment Agreement will be based on the program requirements at the time the new Enrollment Agreement is signed. This means that the new Enrollment Agreement may reflect any applicable tuition increases or other program changes. Regency reserves the right to adjust completed hours if they occurred more than 4 years before your restart date. Appealing Terminations Any student that has been terminated and wishes to restart, must follow the restart process as defined above to determine if he/she is required to submit an appeal. If an appeal is granted, that student will be allowed to restart the program. All appeals must be submitted in writing to following: Student Appeals Committee c/o Regency Beauty Institute 1600 Utica Avenue South, 9th Floor St. Louis Park, MN Fax: [email protected] Appeals may be submitted to the Committee by fax, or mail. With the appeal, the student will be required to provide a detailed Goal Statement explaining (1) the reason for the appeal (for example, the death of a relative, an injury or illness of the student, or other special circumstances), (2) any changes the student has made that will allow him or her to be successful, (3) the steps the student will take to meet and maintain acceptable attendance and academic levels if he or she is permitted to restart program, and (4) any additional information the student believes is relevant to the Committee s determination. Each appeal will be decided by the Student Appeals Committee. The factors considered to determine whether a student may restart may include, but are not limited to, (1) the current rate of attendance and academics, (2) the extent of unfulfilled program requirements, (3) the current capacity at the campus, and (4) the student s disciplinary record. The Committee reserves the right to deny an appeal to restart following termination, at its sole discretion. Additionally, if the Student Appeals Committee grants a student s appeal from a SAP termination, it may impose conditions on the student s restart intended to ensure that he or she will regain Minimum Satisfactory Progress under SAP. The student s progress after restart, including whether he or she is meeting the conditions required by the Student Appeals Committee, will be evaluated once a month on or about the student s monthly anniversary date of the restart date. 14
19 The Student Appeals Committee will issue a written decision including any and all conditions on the decision to the student. All decisions of the Student Appeals Committee are final and are documented in the student s electronic file. After Graduation State Licensure Requirements A student applying for a cosmetology license in the State of Florida must meet the following requirements: AGE: Cosmetology applicants for licensure by examination must be at least 16 years of age or have received a high school diploma. EDUCATION: Applicants must complete 1,200 school hours in a cosmetology program. EXAMINATION: Applicants must complete both portions of the examination (written theory and written clinical) within a two-year period from the date of the first attempt of either portion of the exam. The written examination is administered by Pearson VUE If both portions of the examination are not passed within a two year period, the applicant will be required to pass both portions of the examination again. If the applicant fails either portion of the examination on the first attempt, the applicant can submit a reexamination application without any additional signatures from the cosmetology school. Remedial hours may be required with verification by the school. HIV/AIDS COURSE: Applicants who completed their education at a Florida school should submit the initial HIV/AIDS course completion certificate received from their school. The HIV/AIDS course must have been completed within two years prior to submitting an application and must be at least four hours long. For more information, please refer to our list of board-approved Initial HIV/AIDS Courses. GRADUATE CERTIFICATION: A completed Graduate Certification Form must be submitted with your application. FEE: Pay the $78 fee as provided in the application. APPLICATION: Complete the application for licensure by examination. Please note that criminal convictions may affect an applicant s ability to become licensed. Hour Marks When Exams May Be Taken State Practical Exam: Administered by Regency as the Final Skills Certification (see above) State Written Exam: 1,200 hours The state of Florida administers the written licensing exams. You must complete the application for licensure by examination before scheduling your written exam. The application can be found at the following website: HIV/AIDS Course: In accordance with Florida Administrative Rule 61G , each applicant for licensure or registration as a Cosmetologist shall complete a board-approved educational course on Human Immunodeficiency Virus (HIV) and Acquired Deficiency Syndrome (AIDS) and shall submit proof thereof in the form of a certificate of completion from the provider of such course with their application. Regency does not provide the above mentioned course as part of its curriculum, but it does seek to notify students of their future obligations for additional education in their quest for licensure as a cosmetologist in the state of Florida. Information regarding providers and payment (Regency will only pay for, one attempt) of this course may be obtained from your campus manager. You must schedule an exit interview with the campus manager after you have successfully passed all state required exams to complete the licensing process. In this interview, we will solicit your feedback on our program through a confidential graduate survey, discuss your licensing exam results and career plans, and review the career services available to you as a graduate! Career Paths Once you graduate from our cosmetology program and secure a cosmetology license, these achievements open the way to many possible career paths. They may include: 15
20 Cosmetologist Stylist Cruise ship stylist Color technician Chemical texturizing specialist Cosmetology educator Salon manager or owner Esthetician Nail technician Cosmetology or product consultant/trainer Cosmetology or product sales representative Research cosmetologist Platform artist Video, magazine, or film artist Some of these career paths may require additional training, experience, or licensure. Students should also be aware that (1) the law may differ by state as to the training, experience, or licensure that will be required for any of the career opportunities in the list and (2) state reciprocity laws may limit the application of program completion in one state to licensure in any other state. Regency has not made and will not make any guarantees of employment or salary upon graduation. Career Services Regency s career services department actively works to establish relationships with local potential employers and to provide students access to and information about these potential employers through salon fairs and the Salon Partners Program. In addition, Regency provides you with access to employment opportunities through participation in Your Career Coach is available anytime during your program to assist with resumes, interviewing tips and job openings. After the completion of Course 8, you should contact your Career Coach to create a post-graduation plan. Student Behavioral Expectations Honor Code Regency expects its students to uphold high standards of integrity, professionalism, and honesty. Students who cheat on tests, clock other students time, steal services, retail items, or equipment from Regency, or steal equipment or other items from other students, may be terminated, prosecuted, or both. Regency expects students and staff who observe honor code violations by other students to report these violations to the campus manager. Drug and Alcohol-Free School Policy Regency embraces the spirit of the public law that requires schools to provide a drug-free school and workplace. As part of our philosophy, we are dedicated to the advancement and well-being of the population we serve. The use and abuse of alcohol and other drugs can lead to physical and psychological health risks. These risks depend upon the type of drug used and the intensity of the use. Long-term use of drugs can lead to organic damage to the body and psychological problems. And even short-term use carries a risk of an overdose that can result in effects as serious as death. Drug and alcohol use also carries other direct and indirect health risks, including a heightened risk of blood-transmitted disease for users of intravenous drugs, the risk of pregnancy complications and birth defects in women who use drugs or alcohol while pregnant, and the impairment of the ability to operate motor vehicles. As such, all students and employees are encouraged to abstain from the use of illegal drugs and irresponsible use of alcohol. Students are prohibited from the unlawful manufacture, distribution, dispensing, possession or use of a controlled substance or alcohol anywhere on Regency property including grounds, parking areas, anywhere within the buildings or while participating in school-related activities. Students are also prohibited from being present at school under the influence of alcohol or controlled substances. Students who violate this policy will be subject to disciplinary action up to and including termination of enrollment. If there is a reasonable suspicion that a student 16
21 of Regency is under the influence of alcohol or a controlled substance while on Regency s premises, the student will be required to clock out for the day asked to leave and may be subject to formal discipline. In all cases, Regency will abide by local, state, and federal sanctions regarding unlawful possession of drugs and the consumption of alcohol, including the full enforcement of applicable underage drinking laws. Any student who suspects they or someone else may be at risk is invited to seek services that can be of help. Regency maintains drug and alcohol education information and a list of counseling and support services, including local services, which can be obtained from the campus manager. Title IV Implications of Illegal Drug Convictions Any students convicted of possessing or selling illegal drugs while receiving Title IV assistance will be ineligible for federal financial aid based on the chart below. If convicted of both possessing and selling illegal drugs and the periods of ineligibility are different, the longer penalty will apply. Violence Prevention Policy Regency does not tolerate threats by or against students, staff or guests. In addition, Regency prohibits students from carrying, possessing, or using firearms and/or weapons while on school premises and while attending school-related events off Regency premises. This policy applies to all students, including those with a valid permit to carry a firearm. Campus Practice & Procedure Compliance with School Practice & Procedure You are also required to comply with all school practices and procedures regarding observance of no-smoking areas, use of cell phones and other personal digital devices, storage of personal belongings, dress code, and the like, as explained during new student orientation. Additionally, students are required to come to school in dress code and with the kit items required to perform the work scheduled for their session. Students in violation of these practices and procedures will be required to clock out and leave the school, and repeated violations may result in disciplinary action under the Complaints, Investigations & Disciplinary Actions Section below. Smartphone and Cell Phone Usage Policy General Rule Smartphones, cell phones and other personal electronic devices (collectively, Cell Phones ) may be used within the Regency campus, provided they are not distracting to other students or the learning environment. You should use your best efforts to use cell phones at times, and in areas, that do not interfere with the scheduled learning sessions. While in the campus, Cell Phone ringers and audible alerts must be turned off. Taking photographs or videos of Regency s test materials, proprietary documents and technologies are prohibited. Students may not audio-record, video-record, or photograph a fellow student, Regency staff, or third-party with whom they interact in the course and scope of their enrollment, without the knowledge and prior consent of that person. 17
22 A. Photos: Regency understands the importance of a strong visual portfolio in the cosmetology industry and students are encouraged to begin building their portfolios while still in school. However, please be mindful that Cell Phones may be used to take photos of student work only if: 1. Performance Floor: (i) a Teacher grants the student permission to take the photo, (ii) the client grants the student permission to take the photo (while the Teacher is present), and (iii) the client/model confirms that he/she is at least 18 years old. 2. Classroom: a Teacher grants the student permission. (Photos of work performed on another student also requires permission from that student.) B. Uploading Photos: Cell phones may be used to upload photos to online portfolios at appropriate times: 1. Uploading should not be a distraction to other students; 2. Uploading should not interfere with a client service; and 3. Uploading should not disrupt the learning environment. Taking and uploading photos of student work is optional and will not impact a student s grades or program progress. Campus Manager will make a final determination of what constitutes unacceptable Cell Phone usage. Regency is not responsible for lost, stolen, or damaged Cell Phones. Any Cell Phone found unattended in the campus must be turned in to Regency staff. Regency Photographs & Recordings From time to time, Regency takes photographs, videos, and audio-recordings of campus activities including its students, guests, and others for promotional or other purposes. If you wish not to be included in such photographs, videos, and audio-recordings, please notify your manager or a student services specialist for directions. Dress Code Preparing for your career includes building the work habits you will need to succeed. Accordingly, we require you to come to school dressed in a professional manner. For example, students are not preparing for success if they look like they just woke up! Like your future employers, we require that if you choose to wear makeup or style your hair, that you perform all your grooming at home. The following requirements apply to all students while in the campus, including STAR students and students on a leave of absence. Dress Code Details: A Regency-issued smock or solid black smock that is substantially similar in length and style, which is clean and in good condition. A Regency issued Nametag/ID card must be worn at all times. No jeans except for on designated jeans day and only in states that allow jeans. Pants cannot touch the floor. Students should look clean and professional in appearance The following is not allowed: o Open toed shoes, headscarves, hats and bandanas Campus Manager will make a final determination of what constitutes acceptable dress under this policy or is otherwise inappropriate for safety, sanitation or regulatory reasons. Equal Opportunity Policies Policy against Discrimination, Harassment, Bullying & Offensive Behavior Regency strives to provide an environment in which all students can achieve their goals and thrive. Discrimination, harassment, bullying, and any significantly offensive behavior create barriers to education and achievement and are contrary to Regency s core values. In certain cases, such conduct may also violate the law. Regency is committed to complying with all applicable laws providing equal educational opportunities to individuals regardless of race, color, religion, national origin, sex, age, disability, military or veteran status, or other applicable protected classes under federal, state, or local law. Discrimination means treating an individual or group less favorably than another because of a membership in a legally protected class. Harassment is verbal 18
23 or physical conduct that demeans, stereotypes, or shows hostility or aversion toward an individual or group because of a membership in a legally protected class and which creates an offensive, intimidating or hostile school environment. Bullying is the continuous or repeated infliction of physical harm or psychological distress on one or more students or employees. For all these reasons, Regency will not tolerate discrimination, harassment, bullying or other significantly offensive conduct directed towards students in connection with their educational programs and activities at Regency, without regard for the intent or basis for the conduct. If you believe you have been subject to conduct in violation of the policy and wish to make a complaint, please see the Complaints, Investigations & Disciplinary Action section of this catalog for direction. Title IX and Sexual Misconduct Policy: Discrimination, Harassment, & Other Misconduct on the Basis of Sex In addition to its general prohibition of discrimination, harassment and offensive behavior, Regency has instituted policies and procedures applicable to sexual misconduct, including sex discrimination, sexual harassment, sexual assault, dating violence, domestic violence, and stalking. These policies and procedures are contained in Regency s Title IX and Sexual Misconduct policy, available in full at Regency is committed to complying with all laws that prohibit discrimination on the basis of sex in admission to, employment with, and otherwise in the operation of its educational program and activities. Among the applicable laws, Title IX of the Education Amendments of 1972 prohibits sex discrimination and sexual harassment in schools. Sex Discrimination Sex discrimination means treating an individual or group less favorably than another based upon the sex or gender of that individual or group. For students, it involves conduct or statements that deny the student(s) an equal opportunity to fully benefit from our program and activities. For employees, it involves conduct or statements that deny the employee(s) an equal opportunity in employment. Sexual harassment is one form of sex discrimination. Sexual Harassment Sexual harassment is one form of sex discrimination. Sexual harassment is unwelcome conduct of a sexual nature. It includes unwelcome sexual advances, requests for sexual favors and other verbal, nonverbal, or physical conduct of a sexual nature when: (1) submission to such conduct is made a term or condition of enrollment or employment; (2) submission to or rejection of such conduct is used as a basis for enrollment or employment-related decisions; or (3) such conduct has the effect of unreasonably interfering with school or work performance or creating an intimidating, hostile, or offensive environment. Sexual harassment also includes sexual assault, as defined below. Sexual harassment is defined by the sexual nature of the conduct or communication, not the gender of the individuals involved. Therefore, it may be perpetrated by and against individuals of either gender and between individuals who are of the same gender. The determination of what constitutes sexual harassment will vary with the particular circumstances. Uncivil conduct or common profanity generally do not meet the definition of sexual harassment, except where sexual in nature and so severe or pervasive as to deny or limit the individual s ability to participate in or benefit from our program (in the case of a student) or to enjoy equal employment opportunity (in the case of an employee). A series of incidents may constitute sexual harassment, even if one of the incidents on its own would not rise to the level of harassment. At the same time, a single or isolated action may constitute sexual harassment when that action is sufficiently severe. While hostile-environment sexual harassment may encompass a wide range of conduct, some examples of specifically prohibited conduct include: Acts of verbal, non-verbal or physical aggression, intimidation or hostility based on sex or sex-stereotyping Use of offensive or demeaning terms that have a sexual connotation Objectionable physical closeness, behavior, actions, or contact Inappropriate or repeated suggestions regarding or invitations to, social engagements or events, whether or not Regency-related 20
25 Suggestions, express or implied, that the terms or conditions of enrollment or employment may depend on the granting of sexual favors Actions relating to a student s or employee s status with Regency which is in fact impacted by the grant or refusal of sexual favors Jokes or remarks of a sexual nature Showing or sending materials that have a sexual content or are of a sexual nature (such as cartoons, articles, pictures, etc.), either by , interoffice mail, internet or otherwise This list is not exhaustive. Regency reserves the right to determine whether particular conduct violates its Policy against Discrimination, Harassment, Bullying and Offensive Behavior, the Title IX and Sexual Misconduct Policy, or other policies. Additionally, sexual harassment may include, but is not limited to, sexual assault, domestic violence, dating violence, and stalking, as defined below. Sexual Assault Sexual assault is a form of sexual harassment. A range of conduct falls into the category of sexual assault, including without limitation sexual violence, sexual battery, sexual coercion, rape, or other sexual contact involving force, threat, or intimidation, or without consent. Consent to sexual activity means words or overt actions indicating that the person is agreeing to a particular act. Consent is informed, knowing, and voluntary. Consent is active and not passive. Silence, in and of itself, cannot be interpreted as consent. Domestic Violence Domestic violence includes crimes of violence committed by: A current or former spouse or intimate partner of the victim A person with who a victim shares a child in common A person who is living with or has lived with the victim as a spouse or partner A person similarly situated to a spouse of the victim under the domestic or family violence laws of the jurisdiction in which this policy applies, or Any other person against an adult or youth victim who is protected under the domestic or family violence laws of the jurisdiction in which this policy applies Dating Violence Dating violence means violence committed by a person who is or has been in a social relationship of a romantic or intimate nature with the victim; and where the existence of such a relationship shall be determined based on the following factors: The length of the relationship The type of relationship The frequency of interaction between the persons involved in the relationship Stalking Stalking means engaging in a course of conduct directed at a specific person that would cause a reasonable person to (1) fear for his or her safety or the safety of others; or (2) suffer substantial emotional distress. Academic Adjustments Based on Disability As set forth in the Policy Against Discrimination, Harassment, Bullying and Offensive Behavior, Regency policy prohibits unlawful discrimination against students based on protected characteristics, including disability. Regency is committed to working with qualified students with disabilities to provide equal access to the education that Regency provides and to create an inclusive environment for all. Investigations by the School Upon receipt of a complaint or other direct knowledge of a potential policy violation, Regency will undertake a prompt and thorough investigation. Regency seeks to handle each complaint and investigation with professionalism and discretion. A fair and effective investigation often requires that the details of the complaint and/or the identity of the complainant be shared with those individuals involved in and/or interviewed in the investigation. Such individuals will, however, be instructed that they are expected to maintain the confidentiality of the matter as directed. Students are expected to cooperate fully with any investigation of inappropriate conduct. Except in extraordinary circumstances, Regency seeks to conclude each such investigation within 30 calendar days. Upon conclusion of an investigation Regency will, when appropriate, take corrective action. 28 Absence for Investigation In circumstances where the student is the subject of a complaint, Regency reserves the right to place the student on an absence during the investigation. During that time, the student will not be allowed to return to the school and may not complete hours until the investigation is complete and any disciplinary action has been taken. Such an absence counts against the student s attendance, but is limited to twelve (12) calendar days. If a student continues in the program at the conclusion of an absence for investigation the student may petition for an administrative leave for the absence so that it does not count against the student s attendance rate. If an administrative leave is granted, the student will return to the program on a date determined at Regency s discretion to maximize the student s ability to successfully resume and complete the program. Disciplinary Action by the School Every incident of student misconduct presents unique facts and circumstances, and we review the facts of each incident on its own merits. Regency will consider aggravating (making more serious) and mitigating (making less serious) circumstances when determining appropriate discipline. Factors considered can include the student s history of prior misconduct, the severity of the conduct at issue, the harm or risk posed by the misconduct, the student s acceptance of responsibility for his or her actions, and the impact upon other students in the school. In each instance, Regency retains full discretion to determine what discipline (up to and including termination) is warranted. Further information concerning factors considered in the investigation of complaints of sex discrimination or sexual harassment under Title IX policy is provided at Regency commonly employs 5 levels of disciplinary action, set forth below. Regency does not provide students with progressive discipline. Additionally, these levels of discipline are not exclusive; rather, Regency may implement other types of discipline (e.g., transfer to another Regency school or campus) as appropriate to the situation. Thus, while multiple disciplinary actions issued to an individual student may progress up the levels described below, such step-by-step progression is neither promised nor required. Regency s common disciplinary actions for students are as follows: 1. Verbal Warning. Generally, a verbal warning is appropriate when Regency has no knowledge of prior misconduct by the student and Regency determines that the misconduct is moderate in nature. A memorandum of the verbal warning will be placed in the student s record. 2. Written Warning. Generally, a written warning is appropriate when it is determined that the student has engaged in a prior act of misconduct or the misconduct is serious in nature. 3. One-Day Suspensions. Generally, a one-day suspension is appropriate when the student has engaged in a prior act of misconduct or the misconduct is serious in nature. 4. Three-Day Suspension. Generally, a three-day suspension is appropriate when the student has engaged in repeated acts of prior misconduct or the misconduct is severe in nature. 5. Termination. Generally, termination of enrollment is appropriate when the student has engaged in repeated acts of prior misconduct and the misconduct is severe in nature. However, certain types of misconduct are so severe that Regency may choose to terminate the student s enrollment upon a first offense. The following nonexclusive list provides examples of conduct that, when committed by a student, are likely to result in disciplinary action: Statements or conduct towards a fellow student, staff member, or guest that demeans, harasses, or discriminates against that person on the basis of race, color, national origin, ethnicity, religion, disability, gender, sex, sexual orientation, or any protected characteristic; Sexualized, abusive, bullying, profane, or other offensive statements or conduct towards a fellow student, staff member, or guest; Refusal to follow reasonable directions from a staff member or other insubordination; Theft or destruction of property belonging to Regency, students, guests, or co-workers; Acts or threats of violence towards a fellow student, staff member, or guest or on Regency property; Violation of the drug and alcohol-free school policy; 24
29 Possession of alcohol, illegal drugs, firearms, explosives, weapons or other unlawful or dangerous materials or items on Regency property; Appearing in the school intoxicated or under the influence of a controlled substance; Cheating on an exam; Copying or photographing test materials; Disrupting the learning environment; Refusing a client Violation of the clocking in and out policy, including but not limited to clocking in or out for another student, or asking another student to clock in or out for you; Other acts in violation of Regency policy; and Acts in violation of applicable federal, state, and local law or accreditor standards. 35 Appendix 3: Satisfactory Academic Progress Policy ( SAP ) To remain in good academic standing at Regency and retain eligibility for Title IV financial aid, students must maintain a minimum grade point average (GPA) in the program, maintain a minimum rate of attendance in the program, and complete their program of study within a reasonable period of time. This policy applies to all students (those who receive Title IV financial assistance as well as those who do not). For those students that receive federal financial aid, it is critical to understand that failure to meet the minimum rate of attendance and academic average requirements under SAP may lead to termination of eligibility for financial aid. The SAP evaluation point for a transfer student that contracts with Regency for less than a full academic year (900 program hours) will occur upon the completion of half of the student s total contracted program hours. o Regency Restart Students ( Special Restarts only):! A Special Restart is one of the following: A student that restarts after being out of the program more than 180 consecutive calendar days; or A student that restarts from a different Regency campus, which results in a material change in the base program requirements, tuition or length; or change in U.S. Department of Education school code.! The SAP evaluation point for a Special Restart student that contracts for less than a full academic year (900 program hours) will occur upon the completion of half of the student s remaining program hours. Academic Average In order to meet the academic average requirement, you must have a program GPA of at least 66.67% at each evaluation period. All tests scheduled to be competed as of the evaluation point are included in your academic average calculation under SAP. This means that tests from any incomplete courses are counted as zeroes in your GPA. A student s program GPA is calculated by adding up all test scores completed in the program up to the evaluation period and then divide the total by the total points possible on all the tests. For example, if a student at her 450 hour evaluation had completed 13 tests with a combined score of 1,131 out of a possible 1,300, then her program GPA would by 87% (1131/1300). Technical evaluations are graded on a pass/fail basis and, therefore, are not included in your academic average calculation. Rate of Attendance The minimum rate of attendance for SAP is 66.67%. Your attendance rate is calculated by dividing your total clocked hours in the program by the total number of scheduled hours in the program. Any scheduled hours occurring on days of an approved student leave will not be counted in your SAP measurements. Hours you were scheduled and actually clocked in an incomplete course are counted in your rate of attendance calculation under SAP. Maximum Timeframe The maximum timeframe for SAP is 150% of the program length (1200 hours) or approximately 15 months. Transfer hours are counted as both attempted and completed for the purpose of determining the maximum timeframe for SAP. Warnings & Termination Under SAP. If at any measurement point you do not meet the stated minimum requirements, you will be placed in a SAP status of Warning. If you are in a SAP status of Warning at the time of your next measurement period and still do not meet the minimum rate of attendance and academic average requirements, your enrollment and eligibility for federal financial aid will be terminated with the option to appeal. If, however, you meet the Minimum Satisfactory Progress levels at your next required evaluation point, your SAP status will change from Warning to Satisfactory. 31
36 Course repetitions and non-credit remedial courses are not offered at Regency and therefore they have no effect upon the SAP policy. Appeals under SAP Should you be terminated under SAP for failing to meet the minimum requirements for rate of attendance or academic performance, that termination is not necessarily final. Rather, you may have the option of filing an appeal seeking to restart the program. A student can appeal a termination under SAP one (1) time. If you appeal a SAP termination and your appeal is granted, you will restart the program with a SAP status of Probation and must regain Minimum Satisfactory Progress levels for both attendance and academics before the next required SAP evaluation point. Additionally, the Student Appeals Committee may impose conditions (i.e., an academic plan) on any student that is allowed to restart the program after a SAP termination. Students with a SAP status of Probation will have their progress, including whether you meet the conditions required by the Student Appeals Committee (i.e., the academic plan), evaluated once a month. In order to file an appeal following a SAP termination, you must submit a written appeal to: Student Appeals Committee c/o Regency Beauty Institute 1600 Utica Avenue South, 9th Floor St. Louis Park, MN Fax: [email protected] Appeals may be submitted to the Committee by fax, or mail. The Campus Manager will also receive notice of the appeal, and the final decision from the Student Appeals Committee. All appeals submitted to the Student Appeals Committee must include a detailed Goal Statement explaining (1) the reason for the appeal (for example, the death of a relative, an injury or illness of the student, or other special circumstances), (2) any changes the student has made that will allow him or her to be successful, (3) the steps the student will take to meet and maintain acceptable attendance and academic levels if he or she is permitted to restart program, and (4) any additional information the student believes is relevant to the Committee s determination. Each appeal will be decided by the Student Appeals Committee. The factors considered to determine whether a student may restart may include, but are not limited to, (1) the current rate of attendance and academics, (2) the extent of unfulfilled program requirements, (3) the current capacity at the campus, and (4) the student s disciplinary record. The Committee reserves the right to deny an appeal to restart following termination, at its sole discretion. The Student Appeals Committee will issue a written decision including any and all conditions on the decision to the student. All decisions of the Student Appeals Committee are final and are documented in the student s electronic file. Restart after Termination under SAP If you restart at Regency after a termination under SAP, you will retain the attendance and academic history from your previous enrollment at Regency, and will restart the program with a SAP status of Probation. Student Leaves under SAP If you receive an approved leave of absence, your maximum timeframe for course completion and enrollment agreement end date will be extended by the same number of calendar days as the leave of absence. An approved leave of absence does not affect your SAP status. Thus, if you return from an approved leave of absence, you will have the same SAP status as you had prior to your leave. SAP Statuses Students with a SAP status of Satisfactory, Warning and Probation are considered to be making satisfactory academic progress and are eligible to receive federal financial aid. If your SAP evaluation results in an adverse change in your status, you will be notified. 32
