2 Instructions New York State Office of General Services Group RFP Comprehensive Telecommunications Services Attachment C Cost Tables Instructions: The Cost Tables included in this Attachment are mandatory and must be submitted with the Bidder's Written Response to this RFP document. Failure to complete or submit these Cost Tables may result in the disqualification of the Bidder's Response from further consideration. The Bidder must complete each of the Cost Tables associated with each of the equipment types proposed. The Bidder need not submit Cost Tables for categories for which it is not submitting a response. The Bidder must not leave any spaces within the specific Cost Table blank. Should a cost not apply the Bidder must enter "N/A" for Not Applicable or "N/Q" for Not Quoted or "N/C" for No Cost other references will not be considered. ALL costs must be identified. For instances where a cost is dependent on various componets bidders must list the cost for all components known at the time of the bid response. NO Individual Case Basis (ICB), Call for Quote, To Be Determined (TBD), etc will be accepted without reference to a component unit cost list. The Bidder may add rows however may not change or alter these Cost Tables with the exception of adding like services in those Cost Tables which specifically provide additional spaces to do so.
