10 LDU & Surgery Planning by Dr Paul Cruci The Principal of Progression Endodontic Specialist Dr Paul Cruci, MSc Implant Dent. BDS FDSRCS DPDS MGDSRCS MFGDP LDS discusses his experiences of meeting best practice. Since the introduction of the HTM guidelines in 2009; intended to progressively raise the quality of decontamination in primary care dental services, we have seen dental practices up and down the country categorise themselves accor ding to two specific benchmarks; Essential quality r equirements and Best practice. Whilst the majority initially placed themselves in the former, it is the latter which many aspire to. When aiming for best practice the key word to remember from the above statement is progressively. Moving from essential quality requirements to best practice should be considered a progression ; it requires the ability to firstly want to change and secondly to acquir e the right guidance and support to implement these changes. At the time of the HTM document being r eleased, I was not in a position to adopt best practice r ecommendations immediately. Since setting up my single-handed specialist practice in 1992, an array of developments with high-tech equipment had been made and I was performing mor e dental implant treatment, added to this was an increased array of stock and consequently storage and space started to become an issue. With the HTM guidelines stating wher e dental practices use the same r oom for patient tr eatment and decontamination (essential quality requirements), they need to develop a plan that facilitates a move towar ds a separate and controlled decontamination room, we decided it was a perfect time to add an extension to the practice and make this our dedicated decontamination room. Having identified the impr ovements needed and pr epared a feasible plan to implement such changes, I r ealised I still required the necessary support and operational management Dr Paul Cruci to perform this project. And since this was to be one of the largest and most valuable projects I had ever embarked on, I did what any businessman would do and sear ched for some external help. A couple of the dental dealers we first approached seemed rather reluctant to try and work with us; it felt as if they were either waiting for us to spend money unnecessarily, or they wanted to gr eatly complicate our lives, a pr essure we just didn t need at the time. However, from the moment we met our r egional sales representative, Dan Payne, from Minerva he instantly helped resolve some of our issues and despite having only bought a few sundry products from this company before, I knew they were the ideal partner in helping to move my project forward. In spring 2010 we were still only in the planning stages but wanted Dan to be fully involved in the whole project. Despite only having the measurements, entrances, exits and windows planned, Dan went away and mapped out our entir e decontamination r oom and surgery and on top of this was able to provide an idea of costs that turned my paper dream into reality. A complete practice refurbishment was evidently going to cause some disruption to the practice, so we decided to close for 2 weeks and fortunately had the support of other practices that were able to treat patients if and when an emergency occurred. However I planned to be onsite for the duration of the refurbishment and when the building began in July 2010, myself 10

11 LDU & Surgery Planning and Dan, together with compr essed air technicians, dental engineers, plumbers, electricians and builders were all on hand to work on the surgery and decontamination room. One of the biggest challenges we faced to achieve best practice, was that the dirty and clean areas have to be divided into separate rooms each with a door and no contaminated instruments or personnel are to pass through the clean zone. So whilst we were creating a separate decontamination r oom from the clinical treatment area, we still had the issue that dirty items awaiting decontamination and cleaned/sterilized instruments ready for use, were being transported via the same space. It had been suggested that we required separate in/out doors in the decontamination room or a hatch through the wall for dirty instruments to go in and clean instruments to come out but we just didn t physically have the facility to implement this. One solution, recommended to me by a Department of Health dental cross infection adviser was to implement a double-door stacking cabinet close to the entrance of the room. This meant that personnel would be able to stor e sterilized, sealed instrument cassettes into the cabinet fr om inside the decontamination room which could still be r emoved for use without the operator having to physically enter the r oom. Ultimately, best practice is to r emove the need for physical entry by an operator into the dirty area and with the double-door stacking cabinet we are able to follow the recommended dirty to clean workflow in order to comply. The extra storage facilities and decontamination equipment has enhanced productivity within the practice, particularly with the washer disinfector which has decreased our turnaround time for instruments. Now we don t have to spend valuable time manually cleaning instruments, the nurse simply places them in the washer disinfector in the recommended manner, presses the button and walks away; everything is easily controlled and fully validated by data logging. Staf f safety is also enhanced as handling sharp contaminated instruments is significantly reduced and the automated cleaning cycle ensur es every process is completed corr ectly. We also have two V acuum autoclaves to ensure continuity should one unit fail. Next on the agenda was the r efurbishment of the sur gery! Although we had made some modifications in the past, the cabinetry was the same as when we set up in 1992 and was in need of a revamp. Minerva together with their specialist digital and laser division Dental innovations was our pr eferred supplier for the renovation, from helping us choose the Belmont Clesta II Treatment Centre and the Kodak 2200 Intraoral X-Ray System, through to complete installation. W e also decided on the Durr Vista Scan digital radiography system as being particularly suitable to endodontics and implant dentistry. Premier Cabinets, a division of Minerva, also played a key r ole, helping introduce innovation and flexibility into both the surgery and decontamination room. Despite being short of space, they presented me with an extensive range of colours, materials, fixtures and fittings to cr eate a bespoke design and manufacturing solution that suited my individual needs and ensured we adher ed to decontamination guidelines with all cabinetry and work surfaces. In the surgery we had a specific template to follow detailing where everything had to be positioned; ultimately this was centred around the chair. It was critical the chair was in the right position for the operating micr oscope and x-ray equipment to function together corr ectly, and despite having so many technicians in one room, everyone managed to make the underfloor connections successfully meet within a limited space. It wasn t easy though and ther e were times when we kept returning with a tape measure again and again just to make sure the fit was perfect. The whole pr oject required a lot of careful planning to synchr onise all the services but as a team we overcame these challenges. The Belmont Clesta II, short for clean station, includes a handpiece line flush-out system which enables us to regularly flush fresh antimicrobially-treated water through the handpiece tubings and purge the lines with air. This feature together with the rotatable cuspidor and detachable bowl has enhanced cross infection in-surgery, improving hygiene and ease of cleaning. Since the refurbishment, patients have commented that it is evident a lot of time and money has been invested into this project and that the surgery looks extremely high-tech. I assure them, it has all been done for their benefit and to incr ease our ability to pr ovide consistent high quality car e. That alone impresses them hugely and has proved a very positive message for new and existing referring colleagues. If one can be seen to have gone further than is required, I feel this tells your patients a lot about you as a dental professional. The whole pr oject, from start to finish, wouldn t have been successful without the support of the Minerva Equipment team. By supporting me in designing the layout and maximising the space available I now comply with best practice requirements under HTM They were quick to resolve any issues and I was grateful for their support and expertise which freed up my time immensely. I could walk away and do something else, safe in the knowledge that everything was being done to the best standards. I can honestly say we are where we have always wanted to be meeting best practice. During the external wash the instruments are washed twice with cold water (pulse wash) and once with hot water. This process removes biofilm and dirt faster and more effectively. After cleaning, the handpieces are lubricated. Following the lubrication the autoclave function of the DAC comes into it s own. Once autoclaved the complete cycle is finished. With DAC there is no more manual manipulation of handpieces and from dirty handpiece to autoclaved handpiece the whole cycle takes only 12 1/2 minutes. DAC Universal - HTM page 23 section 4.5 states Type S: these sterilizers are specifically designed to reprocess specific loads. Dependent on the age of the autoclave this may be carried out automatically. If you are unsure please consult the manufacturer. CP(D) Engineer Validation: The validation should be carried out by a qualified engineer and is designed to show that the equipment is working within pre set parameters and as dictated by the manufacturer. What is it? Cleaning Efficacy Visual Check: This happens after the washer disinfector or ultrasonic bath and before the autoclave. Each instrument that has been cleaned must be inspected under an illuminated magnifier to ensure that all dirt and debris has been successfully removed. Any instruments that are found to still have debris must go through the cleaning stage again. Protein Residue Test: This test is used to ensure that the instruments which have been cleaned are free from protein that are not visible to the naked eye. After the cleaning process a random instrument is taken from the load and a Protein detection method is used to determine the cleanliness. Please use products such as Clean-Trace, Protest Quick and Medi Check. Automatic Control Test: This test is used to determine that all the stages of the sterilizer are consistent with previous days. This will involve the user timing each stage and recording the times in a log book. The test will then be signed of daily, but only if the results are consistent with the previous day. If the sterilizer has a printer or data logger this is carried out automatically, providing that cycle monitoring is present (please consult the manufacturer if you are unsure). Cleaning Efficacy Test: This test is designed to challenge the cleaning efficiency of the washer disinfector and ultrasonic bath. A test soil strip is placed into the equipment and a cycle is commenced. Once finished the test strip is checked for any remaining soil residue and the results are recorded. Please use products such as STF Load Check, or PCD. Ultrasonic Activity Test: This is designed to ensure that the cavitation effect of the ultrasonic is working effectively and evenly. Using strips of adhesive tape across the top of the bath, suspend nine strips of prepared foil in the bath in a 3 x 3 grid. Ensure that the rolled bottom end of each foil strip is no more than 10mm above, but not touching, the bottom of the bath. Run the bath for a pre-determined time, remove the foil strips and blot dr y and record the results. The strips should all reflect similar erosion patterns. Steam Penetration Test: This test is used for Class B and S sterilizers only. This is designed to ensure that a successful vacuum has been achieved so the steam is able to penetrate an internal lumin. This will involve one of 2 tests, either a Helix or Bowie Dick. Please consult with your autoclave manufacturer as to which test is needed. 21
