2 How to: Generate Historical Performance Data Reports (High Level Overview) Logging into the Data Portal 1. Go to the CUSP 4 MVP- VAP Online Portal URL: 2. Log into the portal using the user name and password that you received from the National Project Team (NPT). If you do not have the log in information, or are unable to log in, contact the NPT at cusp4mvp@jhmi.edu. How to Generate a Web Report 3. Click on the My Tools Tab. 4. On the Search Box on the Top Right Side, enter CUSP for Mechanically Ventilated Patients and find your appropriate Network (Hospital and Unit). Click on it. 3 5. Click on the Manage button for the appropriate tool (ie. Daily Process Measures, Daily Early Mobility, VAE Rates etc.) that you want to view the Performance Monitor Report. The example below is in regards to the Daily Process Measures tool. Page: 3
4 6. Click on the Measures tab under the appropriate tool that you want to monitor the performance. 7. Choose the appropriate measure that you want to monitor the performance. In this example, the measures include sedation scale, delirium assessment, compliance, contraindication and distribution. 8. Select the Time Period from a drop down menu that you want to view the performance monitor report. The web report will be shown as the following screenshot. Page: 4
5 9. In your web report, a trending chart will be shown under the column Trending describing the data from the beginning of the project till the time period you have chosen. If you have data for two or more months, a View Chart option can be clicked on. A green dot means that the data values are improving while the red dot means the data values are not improving. Page: 5
6 How to Download the Report as a PDF and Excel File 10. To download the file as a PDF, click on the Printer Icon located above the measures table. A new tab will open up with the report. This process may take a few minutes. PDF File: Your unit s rate is shown in blue color under My Rate. To view whether your rates are above or below than those of the average performance values of the cohort, look under the How Do I Compare? part of the table of the report. Page: 6
7 11. To download the file as an excel spreadsheet, click on the Down- facing arrow located above the measures table. The file will be downloaded to your computer can be retrieved from the appropriate folder on your computer. Excel File: The excel file is similar to the PDF file shown above. The numerator values correspond to the number of times the process measure was used in the unit, while the denominator values correspond to the number of times the process measure was supposed to be used. The rate is calculated based on the numerator and denominator values. The improvement opportunity score is the denominator subtracted by the numerator value. This corresponds to the number of times you can improve the measure for the unit. Similar to the pdf file, the average performance values, the my rate vs. average and my percentile values are also shown in the appropriate columns. Congratulations!
