2 In rder t crrectly assess yur applicatin, and if we cannt, autmatically, thrugh ur links with ther gvernment departments, btain the infrmatin required t cnfirm details n yur applicatin frm, yu may, in certain instances, be asked t prvide dcumentatin in supprt f yur grant applicatin. This dcument ffers a guide t the dcumentatin which may be requested frm yu and the type f dcumentatin which is acceptable t SUSI t cnfirm yur applicatin details. When supplying yur dcumentatin yu shuld make sure that; Yur applicatin (W) number is included with all dcumentatin. All dcumentatin requested is psted tgether whenever pssible Yu send yur dcuments t us as sn as pssible. Yu are given a deadline date n the request letter, but if yu are experiencing a delay in surcing a dcument please cntact the SUSI helpdesk and infrm them f this. Yu ensure the declaratin frm enclsed in the request letter is signed by all relevant parties and returned. Incmplete declaratin frms will nt be accepted and this culd slw dwn yur applicatin prcess. The dcuments requested frm yu will depend n yur applicatin details. As sn as yu send in yur dcuments, yur applicatin will be reviewed in rder f receipt. At this stage, if the dcumentatin prvided is nt sufficient r new infrmatin arises, yu may be requested t send in sme additinal dcumentatin. We nly require phtcpies f riginal dcuments. Any dcuments prvided in a language ther than English will require a verified translated cpy. 2

3 Sectin A: Applicants Persnal Details A5-8: Birth Details If requested please supply a phtcpy f yur birth certificate. If yu were brn in Ireland the lng versin birth certificate is required. A13: Final Curse acceptance Frm Please prvide yur Final Cuse Acceptance Frm (FCA) t infrm SUSI f the curse n which yu have accepted a cllege place. CAO Applicants If yu have selected yur curse thrugh the CAO system and chse the ptin t share yur curse/cllege chice details with SUSI then yu d nt need t fill ut the FCA frm as SUSI can get yur final curse details directly n yur behalf. Cmpleting yur FCA Frm (applicants ther than CAO) This shuld be dne n line by lgging int yur SUSI Accunt and clicking the FCA link. Cmplete the n line FCA frm. Yu will need the fllwing infrmatin t hand t cmplete yur final curse acceptance frm (FCA) The unique username and passwrd fr yur SUSI accunt. Yur Curse cde and full title f curse The full name f the cllege yu will attend Yur W applicatin number (this will be n all crrespndence yu receive frm SUSI) A14a-c: Evidence f Independent Residency If yu have applied as an Independent Mature Student, yu must be able t prvide dcumentatin t cnfirm that yu were independent and rdinarily resident at an address. The required dcumentatin must be frm bth; the year prir t entry/re-entry t an apprved further/higher educatin curse and must be frm n later than Octber f that year, and the year f entry/re-entry t an apprved further/higher educatin curse. Example 1: First pint f entry/re-entry int further/higher educatin in 2015/2016: Dcuments frm 2014 and dated n later than Octber 2014 and dcuments dated frm current year Example 2: First pint f entry/re-entry int further/higher educatin in 2013/2014: Dcuments frm 2012 and dated n later than Octber 2012 and dcuments dated frm the actual year yu entered yur curse. (i.e. in this example, the year 2013). 3

4 Nte: Yu, the applicant must be 23 r ver n January 1 st f the year f yur first pint f entry/re-entry int further r higher educatin t be classed as an Independent Mature Student. The fllwing are examples f the types f dcuments which will be required frm yu as evidence f independent residency. A letter cnfirming that this address is registered with the PRTB(Private Residential Tenancies Bard) A Lcal Authrity Lease Agreement, fr example, under the Rental Accmmdatin Scheme, (RAS). A letter cnfirming the receipt and perid f rent r mrtgage supplement Official dcumentatin psted t yu at this address and relating t yur residence there, fr example, a letter frm the Department f Scial Prtectin cnfirming Rent Allwance. (we d nt accept bank statements alne fr this purpse). Utility Bill in yur wn name. If the utility bills in the husehld are in yur spuse's name, please prvide yur marriage certificate with the utility bill. Examples f Utility bills are; (1) landline telephne (we d nt accept mbile telephne bills); (2) fixed bradband; (3) gas; (4) electricity; (5) cable /satellite televisin bill. A19: VTOS (Vcatinal Training Opprtunities Scheme) Statement If yu are in receipt f a VTOS payment yu will be required t dwnlad the SUSI VTOS Statement frm at and request the Educatin and Training Bard wh made the payment t cmplete and stamp it. 4

5 Sectin B: Applicant s Natinality, Immigratin Status and Residency Details B2 Evidence f Natinality If requested yu will need t prvide ne r mre f the fllwing as evidence f natinality: An Irish birth certificate (cpy f lng frm birth cert., if yu were brn n the island f Ireland); A cpy f yur passprt A natinal identity card issued by an EU Member State. A certificate f naturalizatin. A freign births registratin certificate. B3 (a) Evidence f Family Reunificatin with Refugee Status Yu will need t prvide all f the fllwing: Department f Justice and Equality letter(s) sent t yu cnfirming details f yur immigratin status in Ireland; Department f Justice and Equality letter(s) cnfirming the leave t remain f the persn wh applied fr family re-unificatin n yur behalf; A cpy f current Garda Natinal Immigratin Bureau card (GNIB) I Certificate f Registratin Card; A cpy f yur passprt r travel dcuments. 6 B3 (c) Evidence f Permissin t remain as the Family Member f a Eurpean Unin, EEA r Swiss Citizen under the Eurpean Cmmunities (Free Mvement f Persns) Regulatins 2006 and 2008 and EU Directive (EU Treaty Rights prvisins) Yu will need t prvide all f the fllwing: A letter frm the Department f Justice and Law Refrm cnfirming yur immigratin status in Ireland; A cpy f yur Birth Certificate; A cpy f yur Garda Natinal Immigratin Bureau card (GNIB) I Certificate f Registratin Card; A cpy f yur passprt. B4/B5 Evidence f Residence in Ireland, the EU, EEA r Switzerland fr 3 f the last 5 years. Yu may be asked fr evidence f residency in Ireland, the EU, EEA r Switzerland fr 3 f the preceding 5 years. The fllwing dcuments may be used by yu as evidence f residence. Evidence that yu sat the Junir Certificate and Leaving Certificate exams in Ireland; r equivalent schl exams in the EU, EEA r Switzerland. This must have been in the last five years; A letter frm a schl principal in Ireland, the EU, EEA r Switzerland cnfirming yur attendance at a schl. This must have been in the past five years; Scial Welfare Statements r equivalents frm the EU, EEA r Switzerland dated fr the last five years; Utility Bills; Registratin with the Private Residential Tenancies Bard r equivalent frm the EU,EEA r Switzerland; P60 s Bank Statements with activity in the cuntry f residence. Please nte, in relatin t the abve dcumentatin, in the case f utility bills, P60s r equivalent EU, EEA r Swiss tax statements r bank statements, yu will need t supply a minimum f ne dcument per year f residence shwing three twelve mnth perids f residence (36 mnths in ttal). Evidence f yur residence in Ireland can nly be accepted in respect f perids f lawful presence in Ireland and the dcuments that yu prvide shuld be in respect f such perids. Perids f unlawful presence cannt be cnsidered. 6

7 Sectin C: Academic Histry and Other Surces f Financial Assistance C4 Evidence f yur Previus Educatin If yu have attended but nt cmpleted any curse, r yu have cmpleted a curse and hld a qualificatin yu must prvide ne f the fllwing fr each curse. Dcuments accepted as evidence f previus educatin are as fllws; If yu are currently attending a curse we need a letter frm the cllege/institutin cnfirming what years yu have cmpleted s far and what year yu are entering / attending at present. If yu have cmpleted a curse in the past and gained a qualificatin yu may be asked t prvide a cpy f the award yu received If yu previusly attended a curse and did nt cmplete it, yu need t supply a letter frm the cllege / institutin t cnfirm the start date and the date yu withdrew frm the curse. C7 Details f ther Financial Assistance If yu have applied r have been ffered funding frm anther awarding r funding bdy then yu must prvide us with a letter frm them with the fllwing infrmatin ; The details f the awarding bdy A detailed breakdwn f the award that yu may receive (fr maintenance, fees etc.) If yu have applied fr funding frm anther surce and were nt successful yu must prvide us with dcumentary evidence f this. 7

8 Sectin D: Evidence f Estrangement If, n yur SUSI Grant Applicatin Frm yu have declared yurself estranged frm yur parents then yu must prvide evidence f estrangement. Yu will need t supply the fllwing dcumentatin as evidence f estrangement; Letters and/r reprts frm Gardaí, i.e. incident reprts detailing specific dmestic disturbances; Letters and/r reprts frm Scial Wrker r Health Service Executive detailing any care placements, i.e. Fster Care, After Care; Curt Order, E.g. Barring Order. Nte: A letter frm the Gardaí stating that yu are estranged frm yur parents is nt sufficient unless supprted by ne f the dcuments listed abve. D5: Evidence f Separatin If requested t d s, yu must supply evidence f separatin. The fllwing dcuments are accepted as evidence f separatin. A Separatin Agreement A Divrce Decree A Decree f Disslutin fr a civil partnership Evidence frm the Department f Scial Prtectin cnfirming receipt f Deserted Wife s Allwance r a One Parent Family Payment If there is n legal agreement a letter frm yur slicitr cnfirming separatin and where applicable that legal prceedings are pending. Prf f living separately, i.e. Separate Utility Bills fr bth parties. Utility Bills must; Shw separate addresses Cver the same billing perid r part theref. 8

9 Sectin E Details f Dependent Children/Relevant Persn E1a Dependent Children Birth Certificates If requested yu must prvide birth certificates fr all dependent children in yur husehld. (If yur children were brn n the island f Ireland, then the lng frm birth certificate is required). E1b Dependent Children 16 years f age and ver in Full-Time Educatin If requested yu must supply a letter frm schl r institutin (stamped and signed) stating that they will attend in 2014 E1c Dependent Children Medically Certified as Permanently Unfit fr Wrk If requested yu must prvide a letter frm a GP (dctr) r relevant medical bdy stating that the dependent child (16 years r ver) is medically certified as permanently unfit fr wrk. E2 Dependent Children/Relevant Persn in an apprved Further / Higher Educatin Institutin. Yu must prvide a letter frm the cllege r institutin stating that the dependent child r relevant persn is attending Further r Higher Educatin fr the current academic year. Yu will nt be able t prvide this evidence until the dependent children/ relevant persns have registered, re-registered, fr the new academic year. Cllege letters must state that the dependent child r relevant persn is registered fr the cming academic year (2015/2016). Letters stating prvisinally registered r expected t register fr the cming year are nt acceptable. 9

10 Sectin F Recknable Incme F1: Dcumentary evidence fr Incme frm Emplyment SUSI may receive data frm the Revenue Cmmissiners t assist in the evaluatin and prcessing f yur grant applicatin. After the prvisinal assessment f yur applicatin yu may be asked t send SUSI the fllwing dcuments as evidence: P60 fr each emplyment which yu held at the end f the 2014 tax year which yu get frm yur emplyer(s); P45 if yu ceased emplyment in 2014; P21 Balancing Statement, r self-assessment statement, fr 2014 which yu can get n-line at r by cntacting yur lcal Revenue ffice; Payslips fr the applicant s earnings frm hliday emplyment utside f term time in If yu cannt prvide payslips yu can dwnlad the Statement f Hliday Earnings frm frm have yur emplyer cmplete it,and return this frm t SUSI. ( Deductin allwed fr hliday earnings is 2 weeks Easter, 12 weeks in Summer during June, July and August nly and 2 weeks at Christmas) In relatin t CE Scheme incme, yu must prvide us with a P60 frm the emplyer and dwnlad and have the emplyer cmplete the Incme frm Designated Prgrammes Frm frm which details the fllwing: the name f the persn receiving the payment; the amunt received in 2014; if the payment included a Qualified Adult Allwance and if s, the amunt; if the payment included a Child Dependent Increase and if s, fr hw many children and the amunt fr each child; the date the payments started; the date the payments stpped (if applicable). Incme earned in a previus tax year: A letter frm the emplyer stating that incme earned in 2013 was nt paid until 2014 but was included in the P60 fr 2014; A letter frm the emplyer stating that incme earned in 2014 was nt paid in 2014, but has been carried frward fr payment in Nn-recurring vertime payments: In rder t establish if a deductin can be made t yur recknable incme n the basis f nn-recurring vertime payments, yu will be required t prvide the fllwing dcumentatin: Ttal grss incme earned frm emplyment frm tax years 2014 and 2013 (i.e. we will require P60 s fr bth f these years) Breakdwn f amunt(s), if any, earned frm vertime in 2014; 10

11 Cnfirmatin if vertime earned in 2014 is recurring r nn-recurring; If vertime is nn-recurring, cnfirmatin that the vertime will nt recur in the current tax year, and cnfirmatin f the date that vertime was remved/ceased; Where SUSI has made this deductin fr nn-recurring vertime, yu will be re-assessed in the fllwing academic year. Freign incme We will require relevant dcuments fr the tax year, 1 January 2014 t 31 December 2014 i.e. the equivalent f P60 P45 r P21. If yu have incme arising in the UK, yu will need t send us a UK P60 fr 5 April 2014 and 5 April 2015, t cver the tax year 1 January 2014 t 31 December If yu have incme frm any ther cuntry, yu will need t prvide us with the relevant tax dcument(s) frm the cuntry in which yu are/were emplyed. F2: Dcumentary evidence fr Scial Welfare payments Please nte: In the majrity f cases SUSI will nt require a Scial Welfare Statement frm applicants as we have a direct infrmatin feed with the Department f Scial Prtectin. There is n need fr yu t request such Statements frm yur lcal Scial Welfare ffice unless requested t d s by SUSI. If after the prvisinal assessment f yur applicatin yu are asked t send SUSI a Scial Welfare Statement it shuld cntain the fllwing: The name f the persn getting the payment; The type f payment; The amunt received in 2014; If the payment included a Qualified Adult Allwance and if s, the amunt; If the payment included a Child Dependent Increase and if s, fr hw many children and the amunt fr each child; The date the payments started; and The date the payments stpped (if applicable). If yu are in receipt f a scial welfare payment frm utside Ireland, yu must send SUSI the abve details frm the relevant authrity. 11

12 F3: Dcumentary evidence fr payments frm ther gvernment departments Yu may be asked t prvide SUSI with the fllwing dcuments as evidence f payments received frm ther gvernment departments. These may include payments frm; The HSE, FAS, Slas, Lcal Authrities, Pbal, Teagasc, Failte Ireland, Yuthreach, Other A statement frm each gvernment department r state agency with the fllwing infrmatin will be required: The name f the persn getting the payment; The type f payment; The amunt the persn received in 2014; The date the payments started; The date the payments stpped, (if applicable). F4/F5: Dcumentary evidence fr F4(Self Emplyment Incme) and F5 (Rental Incme) If yu have declared yurself, yur parents/legal guardians, spuse, civil partner r chabitant t be self-emplyed r t have incme frm rental f land r prperty yu will be asked t send SUSI the fllwing dcuments as evidence: We recmmend early filing f yur tax returns. This des nt bring frward the date fr payment f yur final tax liability. The earlier yu submit the accunts t revenue, the sner we will be able t prcess yur grant applicatin. If yu wait until the last filing date, this may cause delays in the prcessing and payment f any grant yu may be eligible fr. Cpy f accunts (that is, trading prfit and lss accunt, capital accunt and balance sheet, fr each business fr the year ending in Please nte, that the Frm 11 r extracts frm yur Return f Incme d nt satisfy this accunts requirement. Adjusted Prfit Cmputatin fr incme tax fr the 2014 tax year; If the accunts cntain a wage r remuneratin expense, yu must give SUSI the detailed breakdwn mentined abve under wages and remuneratin adjustment. A breakdwn f bank interest and charges(if applicable) If it applies t yu, a current letter frm the Revenue Cmmissiners exempting yu frm filing tax returns r equivalent frm anther state. If it applies t yu, a ntice f Cessatin f Trading frm the Revenue Cmmissiners. If yur rental prperty is in receivership r with NAMA we require dcumentary evidence f this All pages f the Self-Assessment Statement fr 2014 r its freign equivalent. Even if yur Ntice f Assessment states an incme f 0, we still need t verify this with a set f accunts fr the relevant perid. 12

13 F6: Dcumentary evidence fr prprietary directrships and sharehldings If yu have declared yurself, yur parents/legal guardians, spuse, civil partner r chabitant t be directrs f a cmpany yu will be asked t send SUSI the fllwing dcuments as evidence. A statement frm the cmpany f which yu are directr r in which yu have a sharehlding, utlining the fllwing: The registered name f the cmpany; The registered number f the cmpany; The percentage f vting rights that yu cntrl. As part f the verificatin prcess, we may ask fr cpies f the audited accunts f these cmpanies, alng with a P60 if yu are in receipt f a salary frm this cmpany. F7: Dcumentary evidence fr incme frm pensins ther than State Pensins Shuld yu be in receipt f a pensin ther than a state pensin yu may be asked t send SUSI the fllwing dcuments as evidence: A letter frm yur emplyer(s) r the bdy/bdies administering yur pensin; P60(s) cnfirming the grss amunt yu received in 2014; r P21 PAYE Balancing Statement r Ntice f Assessment fr F8: Dcumentary evidence fr incme frm savings, depsit accunts and investments If yu have declared any incme frm savings, depsit accunts r investments yu may be asked t send SUSI the fllwing dcuments as evidence: Statements f the interest/dividends paid frm the financial institutin r ther prvider, fr example, bank, building sciety, pst ffice r credit unin, cvering the perid 1 st January 2014 t 31 st December 2014; P21 PAYE Balancing Statement r Ntice f Assessment fr F9: Dcumentary evidence fr incme frm maintenance arrangements Yu may be asked t send SUSI the fllwing dcuments as evidence f incme frm maintenance arrangements: Maintenance agreement stating the amunt f maintenance yu receive each mnth; If there is n legal agreement in place, evidence must be prvided such as bank statements detailing the payments fr 2014; Evidence f any bills, mrtgages, rent, tuitin r any ther such payments made as part f the maintenance arrangement. 13

14 F10: Details f Lump Sum Payments frm Retirement r Redundancy If yu have declared that yu r yur parent(s)/legal guardian(s), spuse, civil partner r chabitant retired r were made redundant yu may be asked t send SUSI the fllwing dcuments as evidence: Retirement A letter frm emplyer r the bdy administering pensin stating; Yur date f retirement The grss amunt f yur lump sum The number f years f pensinable service r the number f years f cntributins; Yur grss annual pensin; Yur P45, if applicable. Redundancy A letter frm yur frmer emplyer stating; The date emplyment ceased r became redundant; The grss amunt f lump sum received; The number f year s service with frmer emplyer. F11: Dcumentary evidence fr the dispsal f assets r rights If yu have declared a dispsal f assets r rights yu will be asked t send SUSI the fllwing dcuments as evidence: A cmpleted Dispsal f Assets and Rights Table shwing the prfit r lss n the dispsal f the asset r right. This may be cmpleted when filling ut the grant applicatin frm r dwn laded frm cmpleted and returned t SUSI if requested. A Capital Gains Tax Cmputatin fr F11: Dcumentary evidence fr realisatin f a life assurance plicy Yu will be asked t send SUSI the fllwing dcuments as evidence f realisatin f a life insurance plicy if declared n yur applicatin: A letter frm the financial institutin stating: The amunt received n realisatin; The Irish tax deducted; The amunt(s) paid in premium(s); The date the first premium was paid. 14

15 F12: Dcumentary evidence fr gifts r inheritances If yu have declared a gift r inheritance yu will be asked t send SUSI the fllwing dcuments as evidence: a cmpleted Gifts and Inheritances Table which, if required, yu can dwnlad frm a letter frm the executr/dnr; and Capital Acquisitins Tax Return, if applicable. F13: Dcumentary evidence fr incme frm ther surces nt mentined abve If yu r yur parents/legal guardians have declared incme frm ther surces yu may be asked t send SUSI evidence frm the apprpriate persn r bdy shwing: the grss incme r grss amunt received in 2014; the surce f the incme; a full descriptin f the incme; F13: Incme frm Wdlands If yu received incme frm wdlands in 2014, yu must cmplete the Wdlands Table which, will appear at the bttm f yur applicatin frm. If nt, yu can dwnlad the frm n F14: Dcumentary evidence fr Legally Enfrceable Maintenance Payments If yu have declared that yu make Legally Enfrced Maintenance Payments yu will be asked t send SUSI the fllwing dcuments as evidence: A cpy f the separatin r divrce agreement; P21 PAYE Balancing Statement r Self -Assessment Statement fr We may als ask fr supprting dcumentatin fr the actual payments, e.g. bank statements. F16: Letter frm yur emplyer cnfirming yur current annual salary (2015) Please dwnlad and ask yur emplyer t cmplete the Cnfirmatin f Current Salary Frm frm and return this frm t SUSI F16: Letter frm yur emplyer cnfirming that n vertime will be paid after May 2016 Yu may be asked t prvide a letter frm yur emplyer cnfirming that n vertime will be paid until after May 2016 F16: Ntice f Cessatin f Trading 2015 If yu have ceased trading yu may be asked fr a Ntice f Cessatin f Trading frm the Revenue Cmmissiners. F16: Ntice f Redundancy 2015 If yu have been made redundant in 2015, yu may be asked fr a letter frm yur emplyer stating; The date yu ceased emplyment r became redundant; The grss amunt f yur lump sum; The number f year s service with yur emplyer; P45 issued t yu frm yur emplyer; F16: P If yu have been made redundant in 2015 yu may be asked fr a P45. 16

17 F16: Statement f Mrtgage r Rent Supplement If yu are in receipt f a Mrtgage r Rent Supplement Payment yu may be asked t prvide a statement frm the Department f Scial Prtectin detailing; The start date f payment and the ttal amunt received t date; The weekly amunt received and the ttal in Z1: Evidence f Legal Guardianship If yu have indicated n yur applicatin that yu have a legal guardian then yu will be asked t prvide The fficial curt rder cnfirming wh is yur legal guardian Z2-Z6: Death Certificates (if applicable) We may, in certain circumstance request the death certificate fr the deceased persn. Z7: Declaratin Frm Please sign (all parties t the applicatin must sign this declaratin) and return the Declaratin Frm which yu received frm SUSI. If yu have t prvide ther dcumentatin in supprt f yur applicatin, please ensure it is returned with this frm. Z8: Deed Pll If yu have changed yur name please prvide a cpy f yur Deed Pll frm the Curts in rder t verify yur change f name. Z9: Marriage Certificate Yu may, in certain instances, be required t prvide a cpy f yur r yur parent(s) marriage certificate. Z10: Bank Details Yur bank details are required receive payment f any grant award. T prvide yur bank details please lg nt yur n line applicatin thrugh with yur existing username and passwrd and enter yur bank details. Z11: Verificatin f Persnal Details If SUSI has nt be able t successfully match yur Date f Birth and PPS Number with the details btained by ur external surces we wuld ask that yu please cntact SUSI Supprt in rder t verify that yu have submitted the crrect Date f Birth and PPS Number n yur Applicatin Frm. 17
